Uterine fibroid embolization (UFE) is an image guided minimally invasive procedure utilized to treat symptomatic fibroids (myomas) of the uterus which are causing pain, irregular or heavy menstrual bleeding, and abdominal/pelvic pressure on the bladder or bowel.
This animation demonstrates how one of the most popular minimally invasive procedure, uterine fibroid embolization is performed step by step through a radial access. This procedure is commonly done by interventional radiologist.
As a first step, the left radial artery is accessed under ultrasound guidance using a micropuncture needle. Once the blood return is noted, a microwire is advanced through the needle into the left radial artery.
The needle is exchanged for a slim sheath.
Once the slim sheath is advanced over the microwire and properly placed, the microwire and inner dilator are removed.
A mixture of vasodilators is injected through the sheath to avoid arterial spasm at this point. Then, normal saline infusion is started through the sheath to avoid possible clot formation.
A combination of catheter and wire is advanced from the left radial artery to the aorta and subsequently to the internal iliac artery. Once, the tip of the catheter is placed at the origin of internal iliac artery.
Then, a long small gauge needle is advanced through the anterior abdominal wall to the anterior aspect of lumbar vertebra to preform hypogastric nerve block. After injecting the cocktail, the needle is removed.
Then, the wire is exchanged for a combination of microcatheter and microwire, which is advanced to the uterine artery.
Once unwanted collaterals are bypassed, the uterine artery is embolized using particles until the blood flow slows down, or in another word stasis is achieved.
Then, the microcatheter is pulled back in to the base catheter which is also pulled back to the aorta and thenadvanced to the contralateral internal iliac artery. The unwanted collaterals are bypassed in similar fashion, and particles are injected into the contralateral uterine artery until slow blood flow is noted.
All catheters and microcatheters are removed. Then, saline infusion is stopped.
The wrist is lifted. Then, a TR band is placed on the wrist, inflated with air, and the sheath is removed. Finally, the amount of air within the TR band is adjusted.

I’ve read some awful women testimonials of post procedure. How are you feeling? How is the pain? And any nausea? Read some women suffered terrible cramps (more like contractions) from day 1 post op through weeks and months! Followed by nausea and vomiting.. I’m about to get mine done in December and seriously considering after seeing these comment.

either options are possible. with the radial artery access, you could be discharged sooner, while with groin access you should be watched at least for 2-3 hours.

oo wish you well on your healing journey. I am due mine on Tuesday 19th March 24 which is earlier than expected. Abit nervous about the post-procedure as we all heal differently and at varying rates hey.

Hi guys I had mines on the 1st of November and I must say it went well.. Just the pains after the surgery is really severe, then then next day you are fine..

You could certainly ask for hypogastric nerve block while undergoing UFE. This could significantly reduce the pain.

I had this procedure done few months back. Totally worth it. I went from very heavy menstruation to normal period . The first 48 hours after the procedure was intense with labor intense cramps. But it was worth it. I show l improvement after my next cycle. The surgery doctor is called intervention radiology

I had mine today. My pains aren’t too severe but I know this really depends on pain tolerance and willingness to take narcotics. I got a bunch which is helping a lot. They won’t refill to make sure abuse can’t occur

Im having this procedure this coming week and know from my MRI scan that I have a few pedunculated subserosal fibroids. I heard I could be at higher risk of infection and wondered are there ways of tackling this once the fibroids start to shrink. I know they don't have anyway of leaving my body.

Same here. Had mine on 26 March 2024 so in post op mode. Rest and relax for a few days whilst the cramps and inflammation subsided although I don’t know if it will fully given the process of shrinkage could take upto 9 month to a year. Speedy healing to you

Fibroid is not an infection. Fibroids are benign tumors made of smooth muscle cells and fibrous connective tissue that develop in the uterus. It is estimated that 70 to 80 percent of women will develop fibroids in their lifetime - however, not everyone will develop symptoms or require treatment.

It looks way more harder to do from the wrist than it would be to do from a closer region like the groin. That's the other area that they could perform the procedure from and so kinda makes sense too

Radial artery (wrist) access for peripheral endovascular procedures appears to be safe and effective and should be considered more often. Complication rates are lower than those reported for femoral artery access. The patient could go back to daily actively 3 hours after the procedure. The wrist access is done through the left wrist and almost there is no risk for stroke. The risk of stroke presents when the right radial )wrist) access is obtained.

The pain is after the procedure. You will be sedated during the procedure so you won’t feel any pain. I just had mine June 27th.

@@pammst52422 thanks girl this is so kind of you. Want so bad to go For but its very expensive here in my country. $15000. So the doctors are suggesting that they should remove my whole uterus since my fibroids are too much and they are at a mature stage. Am just 23 and no kids so kinda worried about my health and the fact that I'm most likely not to be a mother. It's too much Would have shared with you the scan results if I had a way.

Preserving fertility is one of the reasons for this procedure. Almost all my patient who came to undergo UFE to preserve fertility, were able to conceive if desired.

The fertility is NOT true! Oh my goodness! I spoke with an interventional radiologist. The fertility preservation is currently unknown after UFE. It is known please put studies here!

@@CC-kl4nh it’s hit and miss. I’ve just had my UFE and I asked that any intervention near my ovaries would need to be held off on as I wanted to avoid early menopause.

I was sedated by IV in my right hand. The DR did UFE in my left wrist. I’m having my second menstrual cycle now. NO cramps. NO heavy bleeding. ❤️

this is for fibroid. For tumors, pending on the nature and stage of the disease, different approaches could be taken in consideration. but, the gold standard is hysterectomy.

I have a 10.9 cm fibroid myself am refusing as well to get a hysterectomy done am 50 Years old now I already have two grown daughters. Never had any of my organs removed since I've had my daughter's. But am praying for Healing natural from God to shrink the fibroids.🙏🏾🙏🏾
