Why second heart sound narrow split It should be Wide Split S2 As the left ventricle blood is emptied earlier due to 2 outflow (Aorta& Shunt) Also the Right ventricle is receiving more blood therefore more time taken for P2 to close That is Wide Split S2

In VSD the physiological split is usually maintained. If pulmonary hypertension occurs, P2 becomes loud, and closes earlier due to pulmonary hypertension, so the narrow split
