One thing I think about the startup failure is that it has to be dangerous and fast. One way to stop a black hole is to overfeed it, hover this will make it grow which is not good for the facility keeping the world safe from it. The other option would be to attempt to starve it since this will not result in it growing. I highly doubt the facility would have the structural integrity to keep the black hole contained for… well, I’m not sure how long because they never give us the mass of it, so I’ll just say this: the earth would likely have been destroyed by the sun’s supernova (about 5,000,000,000 -five billion- years away) before this thing is halfway to its starvation point.
