Hi NH, And I wish I had you as a patient! There is such variability in the way chiropractors treat, it can be incredibly frustrating finding one that works best for you. Highly recommend looking to google reviews (actually read them to determine if they are real or fake) and family/friend recommendations. We are out there, I promise!

Cancers or meningitis? In what world should a chiropractor ever mention either of those things?

Hi Barney - thanks for your comment! Prepare for my Sunday-Morning coffee wall of text! In ours! Namely, in the role of being a Primary Contact Provider (PCP - a healthcare professional that patients can simply walk-in to see without a referral). These include professions like traditional family doctors, nurse practitioners, physiotherapists and chiropractors. While both of these conditions are exceptionally rare compared to the number of neck pain/stiffness patients presenting to chiropractors, having a deep understanding of our anatomy and the presentation of various conditions (including those that your respective profession is unable to treat) is important. In fact, I’d argue that it’s actually MORE important than knowing the conditions you can. This goes double for conditions that like to masquerade as problems each profession is able to treat (meningitis and cervical nerve tumours both present with neck pain - the most common reason a patient will see a chiropractor, besides low-back pain). If someone adjusted a neck that was due to inflamed meninges or peripheral nerve tumours they wouldn’t be helping it - they would potentially instead make it much, much worse. I do not treat either of these conditions, and no chiropractor should attempt to. THEY BOTH WARRANT URGENT REFERRALS. See this orthopaedic test/sign - L’hermitte’s - www.ncbi.nlm.nih.gov/pmc/articles/PMC4445188/#:~:text=Lhermitte's%20sign%20(also%20known%20as,and%20sometimes%20to%20the%20trunk. Tension may be applied to both the meninges & spinal nerves through one of two ways - either distraction (“pulling-on-your-head” - less tension) or forward flexion (more tension). Normally, the patient will feel a pleasent stretch/decompression of their neck musculature - as is this patient's case. However, If a patient’s chief complaint (electrical-like pain shooting down the back of the neck) were created with the mechanism of LESS tension, they would very likely also be positive with the mechanism of more (flexion) - a concerning sign. The next step is to then dive into more targeted questions to suspect/rule out the more serious conditions (namely these two). Has the patient traveled recently? Have they sustained any open-wound cuts/scrapes? Do they have a fever or other flu-like symptoms? These are all signs of potential meningitis - which while rare, does happen. Patient needs to get to the ER immediately. Cervical peripheral nerve tumours would present VERY similarly to a cervical disc herniation, so knowing things like if the patient has a familial history of cancer, is a regular smoker, if they are able to find relief in neck “extension” (bending backward) or not, if they can recall an incident where the pain first began in an instant can all be useful pieces of information to make your decision to refer out. Patient needs an MRI to confirm. At the end of the day, you’re watching a chiro treatment video where the intent is to be educational towards both the patient and my subscribers/viewers. These are not conditions that I would normally mention to a patient unless I was referring out for them specifically - no need to cause stress otherwise. I hope that answers your question :)
