OMG a Apnea Doctor that doesn't blame it on weight and act like if you just ate a little better and worked out a little more. I swear to god. Thank you. I am a wide gentleman. I always have been. A lot of my anatomy is just big. This includes my tounge. One of my problems is and has gotten worse is I move too much in my sleep. I root in my pillow. I kick my legs. I twitch. Last sleep test I had I failed. Why they haven't recommended Inspire is beyond me. Maybe state medication doesn't cover it. Like WTF. This makes so much sense, it is a tried and true technology used for ages on other things. I am wasting away Doc. I will write them.

It’s so brilliant that you examine patients when they are asleep because that’s when the actual problem occurs. I wish more sleep docs would do this

I lay down and try to go to sleep and then I wait to the last second and put my mask on before I fall asleep. That is the easiest way for me instead of having it on for an hour or so waiting to fall asleep. Works for me

After 10 years of dodging my CPAP, I am finally back on it with success. I could not tolerate it in the past. Watching your content and lankyleftys, educated me a lot and had a great influence. I bought myself a philips dreamwear mask, adjusted the CPAP pressure, bought the chinstrap from hope2sleep and it was a game changer. You're a legend, thanks Vik!

the pressure in my machine always felt way too high but the doctor said it was set as low as it goes and i was unable to adjust it at all without going to the doctor and having them do it. my machine feels like a jetstream blowing through my head when all i feel i need is just a gentle breeze

How many thumbs up can I give this video? I’ve watched many of your videos, and I feel like you’re actually speaking to me in this this one in particular. I’ve got most, if not all of the problems relating to sleep apnoea, have been tested, checked, seen an ENT (rude man, told me I have deviated septum, no further advice but just handed me a business card for his gastric surgeon mate). I know I have a small jaw and large tongue, I have to lean forward to get my tongue out the way just blow my nose. No comprehensive advice is given, just directed to the CPAP machine sale people. I really struggled to use the machine, waste of $2k imho, I no longer use it but take a daily antihistamine among other things, and sleep in a semi-foetal position to keep my tongue out the way while I sleep. Thank you for all the wonderful advice

rather than foetal try keeping your neck slightly extended (look slightly up rather than tucking your chin into your chest), it might help. thank you for your kind words.

I am and have always been a side sleeper but I still feel my tongue thrashing around when it wakes me up and I tape my mouth and can still move my tongue. Ugh!

Oh my goodness, I have said that I have this exact problem for seven years to so many doctors and everyone just shrugged their shoulders and said you need to try a different mask, you need to try a different nose spray, you need to take a different allergy pill blah blah blah. Thank you so so very much for explaining this

Another excellent video Vik for which I could write an essay in response, but I won't 😁Best to write a website article and link your video in. In most patients, the nose needs help when wearing a mask all night and don't start me off on the fact that 98% of patients should be issued with a humidifier. For the record, I've had surgery on my turbinates which has helped me breathe so much better in the masks. Also had Drug Induced Sleep Endoscopy and yes, tongue is a problem as well, but always side sleep. I'll stop now before it becomes an essay, but would encourage everyone to address any nasal issues, and thanks for this superb video Vik 👍

Thank you so much Dr. I’m using cpap for the first time tonight (🇦🇺) and after an hour of trying to sleep, having a completely blocked nose and needing to yawn every few minutes but being unable to do so, which is incredibly frustrating, being confused about what is going on, (isn’t the device supposed to keep my airway open - not block it!!!), your advice has settled my raw nerves about it. I had previously tried cpap about a year ago and it was a complete disaster, kept me awake for hours, however, this time I’m determined to make it work. Your advice here is greatly appreciated and has put me at ease somewhat that there is a good chance things will get better.

Hey mate. Fellow Aussie here. I got diagnosed with mild OSA mid 2023 and it took near on 6 months to start feeling comfortable with the mask and wearing it all night. Still not perfect and I have some crap nights sometimes but overall it's good. Hang in there. Habits are hard to break but can also be hard to make. Wearing the mask will become habit and normal if given enough time. It's no small task but you can do it. Persist

The is all very interesting. I was diagnosed with moderate sleep apnea about a year ago. I opted for a mandibular advancement device in lieu of a CPAP. It's almost adjusted out to the full setting to keep my airway fully open. Took a little getting used to at first but because we travel often (don't want to lug around a machine through airports) and the fact that I was already wearing retainers at night for my teeth, switching over to the MAD wasn't that difficult. Love watching your videos to see what else is new in apnea treatment.

My sister is mild and has MAD. I'm moderate but willing to pay out of pocket and get one. I know my problem is tongue based..I wake up feeling it thrashing in my throat. I have a dentist appt in Feb. I'll prob wear the mask and MAD.

I absolutely love my cpap machine, have had it for 8 months(ish). I tried the humidifier on my airsense 11 first couple months and felt like i was being waterboarded so I decided to just not use that and not had an issue since. Never had an issue wearing my mask. I did the smart thing and asked lots of questions regarding the mask. I asked which ones are rated higher for comfort and then which ones get a better seal. Ended up getting F&P vitera. Its a full face mask and its very comfortable. I was concerned about comfort as i can be a light sleeper. That and was worried about the frame for the mask as it had a strip of plastic that comes straight up in between the eyes because sometimes things near my eyes bother me. But its great. Made the right choice for myself.

I'm pretty sure I need full face but hate the tube coming from the front. I'm a side sleeper. I have to tape my mouth so full face might help but I'm kinda claustrophobic and prefer nasal and pillows.

Thanks for encouragement. Im going to try again tonight. I have to admit I became scared of my CPAP machine. Last couple of days it wakes me again with high blood pressure somehow - I feel really unwell with feeling of lack of oxygen and have to walk it off for at least an hour. My night is ruined. I dont know whats the nature of my sleep apnea just yet but when I swim and Im oxygen deprived after a sprint when I try desparately to take quick breath sometimes it happens like the flap is suddenly shut and I cant take a breath at all. Dusturbing especially when in water and desperate for precious air. Swimming does help though - at least thats my subjective observation. Please keep bringing these vids. One of them just ticked all the boxes and sparked me seeking help which turned out to be 100% correct. Thank you again and if you ever needed a lab rabbit Im happy to assist.

I cant believe how my provider had me do a home test n ordered a machine without even examining me or discussing the results with me. The machine leaks and I am so frustrated. From watching this video, I think my problem is enlarged turbinates because my nostrils get blocked even when I am awake. Thanks for helping me understand how CPAP works and how to fix the leak problem. Either standards have changed or the US is different.

I know it's your job, but it blows my mind how simply you explained why I have congestion when I use my cpap, and for some reason my sleep doctor doesn't know why. It seems so simple.

Neither my cpap company nor my ear nose and throat dr. told me any of these things. The main focus the company has is getting 2200 bucks from me for a machine.

Just started cpap 2 weeks ago. Havn't been able to get past 5 hours yet, most nights i seem to rip it off after 3. I am very hopeful though that i can get used to it and do full nights because it can explain basically every health issue i have been having for years. Possible weight loss will be a bonus bnecause I have struggled with that too. I don't think i have had a refreshing nights sleep for over 15 years, but I am determined to figure out my sleep apnea whether its cpap or maybe a MAD.

You're a brilliant doctor. Do you have a video on CPAP causing tinnitus? Should I try to find an ENT who can help me adjust the settings more precisely. CPAP was working just fine for 9 months, and now tinnitus has appeared all of a sudden. Understanding that relationship would be very helpful. Again, you are a Godsend.

I started the Bleep and bought the V-COM and now I sleep and get very very low leaks if not any. My pressure is set at 6 . I also have a deviated septum. But The Doc taught me to sleep with tongue on roof of mouth.I practiced and it has worked. Most nights I get 20/20 for mask leaks. I stopped using the cpap with all different face masks. I quit till I found the Bleep ! Thank goodness. The V-COM makes the air go in slower so you dont have leaks, IT WAS A GAME CHANGER!

Use CPAP gradually and often for short periods whilst trying to remain awake. Do not aim to sleep if you cannot wear it while pleasantly distracted; that’s never going to happen. Wait until you build comfortable acclimatisation, most patients can get established if they about fighting with it and take things gradually.

My dad may have sleep apnea as well but never got tested. I got tested and had an AHI of 85.2 (I stop breathing or have partial stops that many times an hour on average). For me I think it is my nose. I am just glad I do not have my dad's nose and palette as he was born with orofacial clefts (Cleft lip and cleft palate). It has caused him enough issues with his dentures as it is. I do however have huge tonsils that had been scarred so much through years of tonsillitis that they have holes in them. Right now they are inflamed and I haven't had tonsillitis not for 20 years.

I've finally got a CPAP after 8 years of waiting since diagnosis and I can't get on with it. I'm devastated. Autism and face masks don't mix - I feel like I'm suffocating. My only hope is that an M.A.D. or something can work instead. :(

I have the humidity on the lowest setting, otherwise, I can't breathe! However, with low humidity, I often wake up with crusted bogeys and sometimes hardened blood that I have to 'dig' out so I can relieve the pressure and breathe again! My AHI is on average 7 ish so the CPAP is working but could be better. A GP once diagnosed Rhinitis but never recommended meds or sprays, so I'll watch your Spray video and give them a longer period of time to get used to, thank you.

I'm waiting for my appointment for my machine , I told my GP back in September 2010 that I had sleep apnea and he said it was panic attacks, I recently had total hysterectomy for cancer (which can also be caused by untreated sleep apnea and type 2 diabetes ) while in recovery i had sleep apnea and now just being taken serious for it . I am really angry that I was fobbed off and then developed type 2 diabetes and endometrial cancer) you can look this up , both diseases can be from undiagnosed and untreated sleep apnea .

Using my cpap for the first time took a little getting use to and I’ve experienced the things he has mentioned about being woken up for the feeling of being unable to breathe. I’ve been using it pretty consistently for about three months but I feel like I’ve gained weight rather than lost weight. Was really hoping to lose weight

I read that it does make you gain weight. I've gained as well. I was shocked when he said it would make you lose. Google says you don't burn the calories using Cpap like you would if not using it while sleeping

This is a fantastic video... thanks so much. I've been using Cpap for 6 months. I know my tongue is my problem. I wake up with it flapping almost. I also have sinus issues and i do kind the humidifier for helping to keep my passages open. I'm thinking of adding the MAD plus wear the mask. Some say it hurts their jaw and makes it out of shape etc. But Vik, thanks for such an informative video

For me it has been the constant dripping running of my nose throughout the next day and on 2nd day I add sneezing. Cleaned and replace weekly daily doesn’t matter

Interesting - I'm in the process of seeing a new sleep doctor and an ENT so it will be interesting to see if they have similar findings. I do know there's a slight deviation in the septum and my sinuses are always inflamed so it would make sense that that is why I have never been able to tolerate the CPAP. Hoping that will change soon!

I agreed to do a home sleep test, woke up the first time having ripped off the tubes that were taped down to me, the second time I woke up with the nose tube wrapped around my neck chocking me. I sent back the machine and said that I would never do the test again. I was convinced to try a sleep study in a sleep clinic and had the worst sleep of my life but told I have severe sleep apnea and insomnia. I have never woken up laying in the same spot that I've fallen asleep in. I have woken up 180º turned around though. I take clothes off in my sleep, face masks, bandages... I've woken up sitting on the edge of the bed, in the bathroom. I describe my sleep as very active. I explained this to the sleep doctor and he was dismissive and said CPAP is the only option (I'm suspicious that he gets a kickback for selling CPAP, in Canada only a portion is covered by healthcare), I initially agreed to a titration sleep test but cancelled it because I don't see how it's remotely plausible that I would use CPAP without ripping it off my face as soon as my consciousness is asleep, or yank it off the nightstand and break the very expensive machine. I'm hoping the ENT I'm suppose to see later this year will take my concerns seriously and have solutions other than external equipment that needs to be in my mouth or attached to my face, as these are not solutions for me, and even attempting to use them to date has been mentally stressful. I'm even more frustrated after hearing your explanations, as the doctor noted that as it is my nasal passage is nearly completely obstructed and my GP prescribed nasal spray until an ENT visit. How did the sleep doctor think I would be able to use CPAP for that alone!

Get a mouthguard! Tell the sleep clinic you need a mouth guard because the cpap crap doesn’t work

i was diagnosed with sleep apnea over 25 years ago,and I can’t remember seeing a doctor after being diagnosed and given a cpap machine,the older I get the more it seems my facial structure changes,teeth loss mainly,the less the seal to the face.at present i wear nasal strap across the nose ,widens the nostrils as I’ve always had this gristle bit in the front of nose ,which blocks over half the entrance of 1 nostril.also having to clear my throat a lot,so it’s probably an age thing eh .

I guess it depends if you feel the CPAP is working for you? you might get some benefits from an examination.

@@VikVeerENTSurgeon He probably lives in the U.S. so will get the brush off and told just to be more determined and keep at it (he's already 25yrs into it) and maybe change pressure settings, no looking at anatomy to see if that might be the cause. Much cheaper to just tell them to work harder at it.

Not being able to breathe through my nose at all, eight years ago my hospital allowed an unqualified surgeon to remove my uvula leaving with a mild sense of the feeling of a finger being pushed into the back of my throat and having always been a very itchy skin person five years became very bad allover my body I seriously contemplated suicide which I did recover from. I still have skin break outs - all making it hard for me to relax to sleep and that was before the nightmare of wearing a CPAP mask. On getting the mask October 2022 for two months I suffer very badly, trying to get use to sleeping with it on my facemy mask, often waking up from dreams that I was suffocating and getting very little sleep, often going to bed at 4:00am in the morning to get if I was lucky 3 hours sleep. Now I put the mask on and usually I am fast asleep in under a few minutes, 3-6 minutes, even less. My solution, one year ago I started an intensive exercise routine initially power walking (3 Miles), indoor rowing and cycling every night at 10:00 pm for an hour followed by a hot bath with Epson salts. TThis has now also become dumb and barbell weight lifting, boxing exercise plus stretching from 9:00pm to 11:00pm - which being 70 years old 'knackered' me so much I went to sleep very quickly. I have now found that even if I have a break from exercising, I believe the habit has got my body into going to sleep and even waking up once a night to urinate, I go straight back to sleep. The by product of all this exercise, I have gone from 122kg to 93kg, my wife a long term Tradition Thai Massage specialist is amazed how firm my muscles have become and continue to improve (Note - I don't do it, to build muscle but to stop my old muscles declining) plus I now eat a very healthy diet which with losing weight, exercising and very healthy food, overall l feel so much better and not the old man I was becoming I also hope and soon to take my yearly Sleep Apnoea sleep test that as some people in the world doing what Ihave done, they no longer need the CPAP machine - fingers crossed.

Hi doc. There is something important I want to tell you. You may already know about this being a problem for some patients, but if you don't, you might find it informative. I have been using a nicotine vaporizer (e-cigarette) for 12 years. I needed a CPAP for sleep apnea (not related to vaping) and I absolutely could not tolerate it because it was too dry, even while using the humidification reservoir. I don't know if other people who use an e-cigarette have had this problem, but it has been my experience. I'm not looking for answers because I think the only answer is to quit; I just thought you would find the information useful.

I tried CPAP. The rush of air CLOSED my throat. This would happen on bikes or even if wind came in car window as I looked out. Do you know what this is?

Dang! I am definitely struggling with my deviated septum and turbinates / allergies. The problem I run into is one side of my nose shuts down, and I wake up with my CPAP on after 4 hours. Also, I have never had my tonsil removed either. The other issue is, I've had two different deviated septum and turbinates surgeries in my life. I guess I should probably look into a third opinion? The referral for an ENT in USA can take months...

Sleeping with a cpap machine is not so bad, once you have the right mask and pressure, ive used it for 5 years and i cant sleep without it.

thanks for this wonderful video. I've a tracheostomy for sleep apnea which I received in 1986 at aged 11. have you ever had a patient be switched fro tracheostomy to cpap? I'm in the process now and my apneas are very erratic. once again, thanks so much for the video.

Mouth tape is perfeect for closing the mouth with a full face mask. Chin straps never worked for me.

I absolutely cannot sleep with the warm air setting, it reminds me of the suffocating feeling of summertime with humidity. I have asthma and I need the air to be cool in my room, I feel like I'm suffocating and I looked in the masks as size small. I don't think the technician fitted me for the right size. And then I get blowout everytime I try to exhale.

I find the nasal bandaids are effective for opening up the nasal passages, using nasal sprays is not good for the nose.

The sad fact remains that some of us cannot fall asleep with masks or cannula on our faces. I was diagnosed with severe sleep apnea over 15 years ago, and tried for months to get used to a CPAP device with various settings and masks. I would lie there in bed for hours waiting to fall asleep. I couldn't. All I got from doctors was assurances that "Everyone else eventually finds it easy, and the wooshing air lulls them to sleep." Recently I tried again, this time with an APAP machine, but it's the same as with the CPAP. I have a choice to either continue lying awake and getting no rest with the device, or returning the APAP machine and taking my chances.

Retired nco army USA. And one of my side jobs was a NBC Trainor. And it occured that a big accident had happened and we went in to mopp level #5. Then we had to swap our mask for filter replacement. And I went 52 hours w/o sleep. So now some body asked me to cpap. ??? New record 100 hours w/o rem sleep. I'm getting quite edgy. Tks

I have moderate sleep apnea. I wish I knew of a doctor in California as good as you. Is there anyway you can recommend anybody in the Southern California area?

Very informative video. I am not overweight, tonsils, adenoids, uvula removed, (maybe some tissue too I think). Deviated septum addressed as well. My interruption rate is way down post surgeries to only 14x per hour. I still feel horrible when I wake up. Literally exhausted. I have CPAP, use the pillow mask, just so happens I typically sleep on my side but still typically wake up with my mouth open and the air rushing out. Nose is always restricted upon waking whether I use CPAP or not. I am now wondering if I possibly have a tongue blockage as you suggested. I guess it wouldn't hurt to try a chin strap with the CPAP? Also, while the effect of keeping the mouth shut is obvious, I am not understanding how the chin strap stops the tongue from falling back into the throat? Does keeping the jaw closed prevent the tongue from being able to fall back? TY for posting this info. You seem sharper then any ENT or Sleep Doc I've seen here in N.Y.

Thank you so much for taking your time to produce and publish these brilliant explanations. I am recently diagnosed with OSA, just got my CPAP think I might have a big problem and am struggling for some expert guidance. I basically have a hole in my nose created as a new channel for a blocked tear duct. The CPAP is blowing air and tears back into my eye and causing major irritation. Would massively appreciate any advice on best solution, quite concerned CPAP will not now work at all. Thank you.

Nothing like having problems with or without a CPAP. The CPAP making me even more angry with being overly complicated. My sleep is screwed either way. The CPAP arguably makes things worse. Especially when you are lying in bed for 3 hours unable to sleep. Take the mask off and fall asleep quickly.

Why hasn't someone designed a CPAP machine to humidify and warm the air being forced into the lungs ? I've Googled this and there's an accessory machine to do just that. Does it work well ?

Everytime I wear my cpap after an hour or two. I wake up gasping for air because I can’t handle the pressure gashing in when I swallow. And that makes me feel like I’m drowning!!! It just make me wants to take it off instead of killing myself!

AT 42 i am diagnosed with 93AHI. Verry high sleep apnue. I am using Resmet with level 11. The pressure is so high, i cant sleep. Even the slowdown thingy doesnt help. I get awake from the big pressure (lvl11). What to do?

I got my CPAP machine a week ago and it’s so difficult for me anyway they gave me a nasal one and that is not working at all. I want to get a mass but I have to get a referral sent back from my doctor which I’ve been trying to do three days now, so I haven’t used my machine at all and over in a week or so.

Well, I'm back again after 10 months. I started watching this video not realising I have already watched it (& commented) The company that sold me my machine has basically abandoned me )I've not heard from them for over 8 months) I've made my own adjustments to my machine though i am struggling to maintain using it nightly (more like every 3rd or 4th night) but I feel I'm doing ok. My sleep doctor doesn't want to see me anymore, she was happy with my progress & even after having a lung test (because there was a question there) I'm on my own. The thing I would like to know is: if by using the cpap machine you have managed to correct your sleep apnoea problems, how do you know?? I spent a lot of money to buy my machine & though it would be great that my sleep has improved, the machine does wake me a lot, so I still feel like I have disturbed sleep, even though I'm not regularly using it. Buggered if I know but to be honest the full mask isn't much fun to use which is maybe why I've cut back on it's use but I have no further guidance from any professionals.

Great info Sir! What if everything feels great, mask is fitting great, can breath no problem but yet I just lay there wide awake? Then when I eventually give up and take it off, I fall right asleep?

I agree with the information, however none of the methods fix the underlying problem that causes sleep apnea. A CPAP isn't a cure, it's a lifetime bandaid. Your sleep may get better, but you still have apnea. Which to me is wrong, while on CPAP professionals should be trying to solve your apnea. CPAP should be a short term tool heading towards a personal cure.

mouth tape works wonders to stop breathing thru your mouth……I don’t understand why doctors are not recommending it? stops snoring……easier than a chin strap…..works great w/nasal mask…..

sadly mouth tape isn't particularly good (or safe). i'll do a video on it one day. chin straps are more effective (and safer), and are more of a temporary intervention whereas mouth tape seems to require you to use them forever.

I am learning so much! I took a sleep test and my apnea is very mild. Mine is due to tongue hump. If I use another device that helps with that then I wouldn’t need my machine- is that correct? What about just raising my head while sleeping?

I have what I believe is post nasal drip which means I have constantly thick mucus at the back of my throat especially when I lie down at night. Saliva is the only way I have to clear it (I have never smoked and am.only slightly overweight). CPap dries my mouth which causes me to choke more at night even with the dehumidifier) so I can't use it. I'm really struggling. Any suggestions please?

I have a question. I get some weird pressure build up that feels like it’s in between my eyelid and my eye on my left eye. This same eye I had issues with a tear duct on when I was about 3 years old. Do you think that it is the CPAP causing this pressure? Or do you think it’s some weird kind of sinus leak I may have due to my surgery? They had to put a tube in my tear duct to try to get it to create another one on its own since my original tear duct just kind of came out. Sorry if I’m not making a lot of sense. I’m just going off of what my mother had told me about all of this from when I was super young and I have very little understanding of it. Anyways though thank you for any help or advice you may give! I also have a known deviated septum.

My problem is that I lost 60 lbs since starting my CPAP. My machine is set to automatically adjust to my breathing so it is automated. I had to discontinue my machine bc even at the lowest pressure it is TOO much air. I would wake up feeling like I was suffocating or simply wake up in the morning with my stomach full of air and gas. It was so uncomfortable. I found that I was more tired using the Cpap than I was when I didn’t. I thought maybe with the weight loss I didn’t need it anymore but I am still snoring, not like I did before at all but enough to bug my partner. I don’t feel anything like I used to during the day. I’m not randomly and suddenly falling asleep. I can make it through the day without a nap or nodding off, but I do still have some episodes where I feel very sleepy. Mostly the snoring is the major issue. I can’t use the machine again bc that seems to make me feel a lot worse the next day, what can I do??? 😩 I am trying nasal sprays and strips, I am now going to try an essential oil throat spray 😬 I don’t want to pay 3000 for a mouth guard that I won’t need long since I am still working on losing weight and I also wear aligners at night to straighten my teeth so the mouth guard isn’t ideal. Any suggestions?!!

Been using a cpap for a week (nasal mask), and I feel like crap the next day. My mouth kept leaking, so I used a chin strap. My tongue moves back, so I moved to my side, but one of my nostrils gets congested when that happens, so I feel like I just can’t win lol I guess I need a MAD?
