Quite impractical to visit all these places in the time limit which u mentioned ...u will be in complete rush rather than enjoying the places. I would suggest to visit a few places and enjoying the moments there.

its really upto you, mode of transport, stay, food all has its different price categories but it shouldn't cost you more than 2k(hotel) + 2k(food) + 2k(travel) = 6k for 2 person

yes, i cannot tell you exact hotel name but i can suggest you that look for hotels near ghantaghar or pichola lake it'll be easy for you to visit whole city from same hotel :)

These are less visited places by tourists as most of them come just for weekends but If you get enough time surely you can visit these places also. :)
