Unfractionated heparin is a strongly anionic anticoagulant that forms a salt aggregate when exposed to the positive cationic arginine peptide of protamine.[6] The salt aggregate that forms is inactive and has no anticoagulant properties. Protamine has a rapid onset of action, neutralizes unfractionated heparin within 5 minutes, and has a relatively short half-life of approximately 10 minutes. There is very little known about the metabolism of the heparin-protamine salt aggregate. Some studies have described the metabolism of the salt in the liver, while other studies have reported that the salt is metabolized and excreted by the kidneys.[7][8] Care is necessary to administer the proper dose of protamine, as excessive doses, protamine has been shown to possess anticoagulation properties and may cause bleeding and increased transfusion requirements.[3

Heparin is a anticoagulant which reduce the clotting ability of the blood in the blood vessel and if the patient is receiving high dose of heparin the patient will have excessive bleeding as heparin will stop the clotting of the blood .So to stop the excess bleeding protamine sulphate is use which is a antidote of heparin and it's action is to reverse the effects of heparin by removing the blockage of the blood clot which was created by admininistation of heparin.The protamine sulphate is administered 0.5mg-1mg per 80-120unit of heparin and it's effect is seen within 5 minutes and it's last for 2 hour.And in addition protamine sulphate should be administered slowly or else the patient may have hypotension, bradycardia, dyspnea
