I remember failing by disconnecting the tanker by mistake. Driving down a backroad, going downhill, accelerating for more speed (which I shouldn't have done while driving a 18 wheeler) and made a huge turn, thus caused the trailer to swing and jackknifed the trailer and disconnecting it.
