Poor training I blame on industry, if they would train people in a classroom environment with lab and certify the splicing, lugging and crimping this problem would be reduced. That's how aviation industry does it, you can't get near an airplane without having been tested. That's what I went through. The MV industry can learn from this, sounds like they don't want to invest in their people!
