But homogenization does this without an emulsifier, by making the immiscible droplets _reeeealy_ small. Why does this work? Why don't the lipid droplets "group" together, if you reduce their size beyond some specific limit?

Because of addition of an third agent i.e. emulsifying agent that lowers the energy barrier b/w two immiscible liquids !
