If you really think about what Tsunade was capable of in terms of even being considered to heal a reaper death seal and the fact that IF she healed his arms she potentially could have led to the destruction of Konoha was a pretty big moment in naruto history. This would not happen because Orochimaru literally states Tsunade is the last person he'd hurt. This was even shown in the past when she stopped him from taking out jirayia. Tsunade would likely end up being branded a rogue Ninja though I doubt Orochimaru would flat out state she healed him during the bridge fight, but Yamato ain't stupid assuming he'd take over the team after Kakashi is down. He'd probably report what he suspects to jirayia who would likely confirm only Tsunade could be so capable. Likely leading her to go into hiding in her summonings land knowing she'd likely be hunted down.

I think there are a few major details you either forgot or left out One She’d never agree too shazunia being killed 2 if Jiraiya is hokagae during either attack on the leaf he’d definitely go into sage mode to stop Orochinmaru three there’s nothing stopping him from sending Naruro to learn sage mode definitely if Kakashi is dead & the village is dangerously low on capable ninja even more so if Jiraiya is hokage & can’t afford to leave the village & most importantly you completely took Might Guy the strongest character in the leaf outside of maybe Jiraiya completely out of this timeline other then a brief mention this what if that had great potential was completely garbage

Orochimaru bodied Jiraiya when he first left the village and is implied to be stronger than him. Sage mode wouldn't help. Gai being out the village fits what happens in the timeline. After the village was destroyed there weren't any Jounin in the village. Jiraiya would have been forced to send them out on those S rank missions back to back to back, just like Tsunade. No Kakashi means Gai is likely out the village.
