Just a recommendation: If you're looking for modifiers for the wheel, there are plenty of options to choose from the difficulty modifiers in the Downfall mod. One in particluar is Neow's Lament where you draw 1 less card and start with a max of 2 energy. Of course the flip size is, you gain a HEAPING amount of max HP each Act you clear so theres that!

Go ironclad. Stack strength, energy, get whirlwind, and get the thing that doubles your attack if energy > 2. Use pandora's box glitch to remove all attack defends. Every turn becomes whirlwind = automatic win. Use every campfire to heal.

Hey frost! I was thinking for the wheel maybe a reasonable time limit for a run and you lose the run if you pass it? Another idea is a sectioned time limit where every time 10 minutes pass (in game) you get to spin the wheel of change. Love your content!

What if you started off with a 10 card deck of your choosing, but could only add cards from the shop and boss rewards? Alternatively, you can choose what relics you get at the end of the act from the entire pool, but you have to add a Necronomicurse if you do. I have a few other ideas, but those came to mind. I will go back to watching your poggy content now.

Personally, instead of ascension 0 streak. I'd like to see your attempt at an ascension ladder. Basically, beat ascension 0, set it to ascension 1, beat that, go to 2 etc. But any time you lose a run, it's all the way back to ascension 0. The aim is to beat all 20 ascencion stages in a streak. Idk if it's quite the sort of thing you'd do, I first saw it does as long form co-hosted videos on KZbin and you are, primarily a streamer. I don't know that it would translate well.
