This is spot on. You can't see how the roof was installed, how the fasteners were used or if the underlayment system was installed properly. A roof inspection isn't a perfect system but it can offer some good insight. If I'm doing an inspection and I see improper applications at the surface, it suggests poor workmanship and shortcuts were applied to the hidden aspects.

Not that I'm familiar with. Normally they're just the opinion of a roofer about the condition of a roof.

You can create your own in Word, or just write it on a piece of paper and copy it. Put down your specs of what you want the contractor to do for you and give it each contractor you're interviewing.
