This machine just saved my brothers life. My brothers organs were shutting down from the delta virus. There was no chance he could recover the doctors said. @@Tanman96 I did not experience pain but it is uncomfortable that you can't really move because the tubes where by my bikini area (one on each side). The removal was a little painful. I didn't wake up until 10 days after being intubated so I'm just sharing what I experienced after I woke up. My breathing wasn't good when I woke up but it progressively got better. I'm so thankful to be alive

One year ago today, my husband (age 63)was on ECMO for severe Cardiogenic shock-post MI, He had right vent. failure - which led to some Left vent. failure - had a right pump which failed - then a left pump failure - with stent placement X 3 with no improvement. Cardiac arrest X 3 - Surgery 4 times within 24 hour time frame - was placed on ECMO in Huntington, WV - transported via helicopter to Ruby Memorial WVU Hospital in Morgantown, WV. Excellent group of ECMO technicians and medical staff - placed on VA ECMO - then to RVAD - and the rest is golden. Spent 1 month in hospital, then to in patient rehab, and finally cardiac rehab. No previous cardiac problems and is still alive and doing very, very well. Keep on teaching the wonders of this life-saving technology. It has taken me this long to really research what he survived. Thanks so much for the overview. TDL

This was the most informative yet. I'm currently taking anatomy & physiology for nursing school. We are on chapter 22 about the lungs. This video really helped put everything together as far as the importance of knowing about the heart and the lungs anatomy and physiology components . Just hearing you talk about the valves etc gave me confidence to one day explain the flow of the heart to a patient and why they may need ECMO when I become a nurse. Thank you. 😌

I have been aware of ECMO for decades. It is an amazing technology but technically difficult, expensive and not widely available ( in the sense that not every hospital has it, or has multiple machines, or techs that can run the machine). Glad we have it, but better to avoid Covid infection. Retired RN

Thank you for this video, it's been a big help and very easy to follow for someone like myself with very little medical knowledge. We currently have a family member waiting to be transferred to a hospital that can provide ECMO and I am their primary contact. Although the doctors have explained briefly, I was still somewhat unclear and this has been a huge help in allowing me to understand exactly what ECMO is and will really help me to explain to other friends and family who don't quite understand how it all works.

Great video, thank you! So if the patient is on ECMO and they go into vfib or pulseless vtach that doesn't convert with defibrillation after the first shock, do you start CPR or would you continue to let the ECMO pump circulate the blood alone?

It depends if they are VV or VA ECMO. VV only supports the lungs so in this case you would perform CPR. If they are VA, the pump is providing both pulmonary and cardiac support so CPR wouldn't be required.

R C beat me to it and is correct. To add just a little... VV ECMO is dependent on a functioning heart to pump the oxygenated blood around the body. So if they loose a pulse, you would initiate CPR but keep the ECMO going. Shocks, compressions, the whole thing. VA ECMO still functions with no cardiac activity and is providing the perfusion and oxygenated blood for the body. You more than likely won't identify a pulseless VTACH since we will have a MAP going from the ECMO. That said, any lethal arrhythmia we would attempt to shock them out of it. As long as we are maintaining good flow and MAP from the ECMO, we would not do compressions. They can persist for a long period of time in a lethal arrhythmia, but at some point it will either become asystole or it would become futile if they didn't go back to a rhythm that can support life. If we lost flow or pressure from VA ECMO, then we would clamp out (stopping ECMO) and run a full code as normal. Doesn't happen often, but when it does its a bad day. Hope that helps! 😊

Def not something to start with (usually) as it is very invasive and has a lot of risks. That said, the evaluation for patients to go on is often done late, and more a result of the lack of familiarity with it. In large ECMO centers the evaluation often happens much earlier, but they are also very familiar with it. That said, its also resource heavy and not always available even if someone truly needs it.

My sister spent a few weeks on ECMO and survived. She was an adult that had lung damage from a fire. She entered the hospital June 17th, 2017, placed on ECMO on June 21st, 2017, and removed from ECMO on July 5th 2017.

I was on ECMO for a week after going into cardiogenic shock after years of heart failure. Afterward I had an impella and then the lvad. Hospitalized for two months and inpatient rehab for 2 1/2 weeks. I still have some nerve damage and neuropathy but otherwise doing well and working towards a heart transplant after 2 years of therapies. Very good description!

That’s wonderful. I had the same. I was fit and healthy and a virus ruined my heart. I was on ecmo for a week in ICU until I GOT AN LVad. I spent a month in ICU and then went to a ward and waited three and a half months until I was fantastically lucky to get a transplant. I’m so grateful to all the doctors nurses and staff for saving my life. I’m now three years post transplant and baring complications, I am getting by fine.. I’m so glad you are doing good too. a week and then

So I have been asked to take a class for VV ECMO. This video was a helpful introduction to the concept of ECMO before I actually take the class. It has decreased my anxiety. @@treeceysfatcatenterstheroo5622 Thanks. Yeah my purpose in this part was to explain the how behind the gas exchange. Many staff new to ECMO are coming from CRRT management and understand the concept of fiber use for flow and diffusion, though that is blood in the fiber for CRRT.

I was on ECMO in November/December 2020 after multiple heart failures,a stent, a bypass,a mitral valve replaced, and multiple organ failures. Without it, I wouldn't be here today

In really enjoyed seeing how ECMO has progressed over the past 30 years. I was a cardio-pulmonary tech @ WHMC from 1973-1990. Drs. Robert DeLemos and Don Null were some of the early neonatal pioneers in using ECMO. The typical patient was a premature newborn, usually weighing less than 2000 grams and delivered because they were in fetal distress. When that happened, the baby would release it's meconium plug (essentially inutero stool.) It often had the consistency of tar. The baby would inhale this while still in mom's tummy and require ECMO. Wilford Hall's ECMO device looked somewhat like a Rube Goldberg device, but it worked fantastically well. When I retired in 1990, WHMC had the only ECMO that a neonatal patient could be placed on and flown across the country. The reason- at that time the standard aero-vac plane was some version of a Lear Jet. Not a lot of room inside for all the personnel needed. Thankfully, Uncle Sam had a large fleet of C9 air evac aircraft that could carry ECMO device, patient, 3 RN's and 2 technicians and a respiratory therapist.

How cool! Wow, thank you for sharing. Its always amazing to see how things progress over time and to hear about how things were before. Now-a-days, it has become such a small "manageable" device. Moving around with it is so much easier than even the setup we used just before the new Cardihelp pump, which is quite small and very portable. Add to that the increased availability and usage across many populations and it has become such an incredible tool to save so many people. I'm always in awe of the technology and it will be interesting to see how it advances in the future!

Ecmo saved my daughter's life. She was put on it when she was less than 8 hours old. She is now almost 20 and this machine saved her.

Hi Eddie! The weirdest thing happened - I watched your ECMO videos this afternoon, and several hours later, I happened to care for a patient who fit the v-v ECMO indications to a T! She's on her way to the ECMO center as we speak! Thanks for giving me the knowledge to be confident in my assessment that "she needs ECMO".

Really excellent presentation and a very understandable breakdown of the ECMO components and processes. I was curious about how ECMO worked and when it was indicated since the use of ECMO appears to be so critical in the tx. of advanced CV. I'm not a doctor, but I teach future RN's and Doctors at a High School pre-med program and the students like to throw random questions my way concerning current med issues and tx. so I just want to be prepared! Thanks again!

My daughter was placed on VV ECMO using a dual cannula when she was 24 hours old due to pulmonary hypertension from Maconium aspiration. Her lungs recovered and she was off the machine in 5 days. She is doing well and expected to be discharged from hospital in a week. You have beautifully explained the procedure. It makes a big difference for parents like us. Thank you !

I had ecmo treatment.recovered after 1 month in hospital. 2 weeks in coma ,week in icu and a week in general ward .lucky I survived .

but why multiorgan failure or renal failure is a contraindication against using ecmo? won't using it relieve that failure by increasing blood perfusion and oxygenation? especially when it was the reason of organ failure in the first place. What exactly ecmo does to the organism except helping the heart and oxygenating blood?

Your channel is amazing! I start my flight nurse position Monday and this really helps me fill in the gaps on a lot of things. My ICU didn't do things like ecmo, impellas, lvads, etc but now I will definitely be transporting those patients so this is super helpful! Thank you for putting this great content out there!

My friend was on ECMO for 31 days. She died. She had a congenital heart defect. Her mitral valve was damaged. She had 3 previous procedures. The went to have a 4 procedure to install a mechanical valve. Due to scar tissue they were unable to put it in so they inserted a pig valve. After her procedure she had a heart attack. They did a single CBG. She arrested but they got her back and they put her on ECMO.
