Thank you Ardra and Dr. G. For the last four years, being 18 w/RRMS years MS; I felt I was staring down the barrel of SPMS. Learning that what it really comes down to is my neurological reserve has helped take some pressure off. We keep going despite the labels! 🧡 (Totally sharing this in my Facebook Group, think it will be helpful knowledge for a lot of folks!)🙌🏼

I was diagnosed with RRMS 13 years ago which changed to SPMS 3 years later although it was very slow progression but it has progressed rapidly in the last 6 months especially in my legs and balance even though the last 5 years of MRI scans including one a few months ago have shown no new lesions or inlammation/flare ups. My neuroligist said it must have burned out so its frustrating for me that i am getting physically worse going from using 1 walking stick to 2 walking sticks to a wheelchair in such a short space of time I am 54 now so maybe that has something to do with it.

That sounds incredibly challenging. Thank you for sharing your story. What you describe sounds similar to 'smouldering MS' that Prof G talks about elsewhere - symptomatic progression without new lesions. Hi I’m in the same situation as you. I have no new lesions or inflammation found on my MRI, therefore my Neurologist refuses to give me a DMT, because I transitioned from RRMS to SPMS. This is absolutely ridiculous and I’m only 45, and I can’t believe I am being refused a DMT.

My wife is exactly like you , she is using rollator but i fear wheelchair will be sooner than later

Hi Tuan, secondary progressive MS is so-called because it is the secondary stage of relapsing remitting MS (RRMS). Primary progressive MS (PPMS), however, is different to RRMS because there are not periods of remission in between MS relapses. Rather, symptoms gradually become worse. Therefore, SPMS will not become PPMS. Hope this helps! ☺️

The reality is that all MS is progressive, regardless of the subtype you have been classified as having. As said in the name, DMTs can help to modify or slow the course of your MS, but not cure it. Also, the individual variation is huge which presents more challenges for those working to improve treatment. DMT are a Joke only hsct can result in normal brain atrophy. see "normal brain atrophy after hsct for ms"
