From the minimal information that you have shared , it's really difficult identify the root cause. Hi sir I was conducted Microdiscectomy for L5/S1 Disc herinated pressing right leg nerves ... Immediately pain got reduced for right leg after surgery.. everything was fine ... Now am post 20 days of surgery, From 14th day am experiencing pain left leg ... It is new ... And back pain from left side .... Kindly please let me know what will be the problem

I am understanding a bit more what this means and it helps. My MRI said that at c3-c4 there is minimal disc osteophyte complex which indents the ventral sac without abutting or deforming cord. Facets are normal. No significant spinal canal stenosis is identified. There is minimal uncovertebral joint spurring. The neural foramina are patent bilaterally. My question is the arm weakness and constant numbness in hand caused but the indent in the ventral sac ? It has been very miserable i wake up through out the night with numbness along with pain in my arm. My gp tried to tell me it was just a strained muscle i believe that my MRI says differently. Am I wrong ? Colleen

Hi Colleen, GPs aren't really experts in anything. They will happily gaslight you to hell and back. Trust your own body, your own symptoms. You need to see a specialist and go off their opinions. Even then, get a second opinion. All the best to you.

Greetings Mr. Raghuram, It's a really interesting suggestion and I will surely work on that. Frozen shoulder have been explained variably as periarthritis and adhesive capsulitis as well. I'll give a glimpse of the pattern of anatomical arrangements that makes the shoulder highly mobile and also the changes that brings in the restricted movements. The presence of osteophytes at C3-C4 levels need to be correlated with much more factors like : Age , the extend of thecal sac compression, the extend of central canal narrowing, the presenting symptoms as of now. Greetings @thespandilizer6562, From the report, it is evident that the pressure on meningeal covering is causing most of your symptoms. But considering your age, I strongly feel like there will be a good improvement from the condition if you focus on neck muscle strengthening exercises. It's a must that you should be choosing the exercises only after a consultation with a physiotherapist. Sir I have mild disc bulge at C4-C5 and C5-C6 with flattening the thecal sac and on C5-C6 also have annular tear also this problem is from February 2023 due to injury and still have pain in shoulder and finger of left arm ...and I wanted to go gym so can I join gym back or not because from last few months i discontinued but now I wanted to go so can I join gym with this pain or not 😢

Greetings Mr.Raghav, It's evident that there's a noticeable compression on your spinal cord, and that it clearly causing the radiating pain and probably numbness at times. Since upu were restrained from going to gym. I would particularly suggest you to start with "isometric neck exercises" in the beginning for atleast 2 months and later after a discussion, you may give give the gym. And regarding your question: it's not easy to define the solution for this problem as there are a lot of underlying causes that can trigger this condition. Starting from a disc bulge to an injury happened earlier. Apart from that, there is also some contributing factors like body weight, age, occupation, postural stress etc. I can give you a suggestion only after a thorough understanding of all these facts and medical history of the patient. I am wondering whether you think a small posterior disc osteophyte complex present in C5-C6 with minimal indentation on the anterior thecal sac without significant spinal canal stenosis could cause any neurological symptoms, such as vision changes? Twenty minutes after tweaking my neck I developed visual snow over the course of a few minutes last February. It's a mottled pattern of clear gray and yellow that is overlayed across my whole field of vision. I also noticed my very near vision is noticeably worse, specifically anything that's less than ~20 cm from my eyes. Optometrists have given me a clean bill of health, and are suggesting visual snow syndrome, a neurological issue with no clear cause or treatment at this time. I'm trying to fully rule out a neck injury as a potential cause. Thank you so much for taking the time to talk to me about this. I really appreciate it.

Greetings, As per the opinion from the Optometrists , you need not worry much. And I could tell you clearly that the chances of neck strain and it's involvement in the same is very minimal. But still in certain cases - long screen time , long work hours in a bad neck posture can also possibly trigger the visual snow syndrome. May be because of the alterations of blood supply to various parts of brain against gravity. Hence as a trial you can actually try improving your neck posture by utilizing the "isometric neck exercises"(make it a routine) and also reducing the screen time or by shifting yourself into an ergonomic work environment. As a personal suggestion, I also advice Yoga practice and meditation for atleast 20 minutes a day. It will also help you. Regards Dr.Manu krishnan.K Founder: Proanatomy

Thank you for these wonderful recommendations. I'm currently on a physical therapy regimen to strengthen my neck, including isometric neck exercises, so I will continue with that. I'll also look into the yoga and meditation. I greatly appreciate all the help you've given me.

sir my mother have vertebral column compression fracture,,in comparison the fluid cleft the fracture of D12 vertebral body has increased, The fracture extended to the posterior Cortex which is seen to bulge in to spinal canal indenting the thecal sac.Cord edema is seen at this level more on the left side.sir this is her MRI report and my mother can't move left leg, doctor suggested spine surgery and vertebroplasty and fix screws and rod in spine,Is this the right option sir, kindly suggest Thanks her age is 70 and she have DVT subcute thrombosis of distal left superficial femoral vein and left poplitcal vein and perinacal and solcal vein

Greetings @ratanexotica6499 From the facts you have shared here it's evident that there is a clear involvement of mechanical compression over the spinal cord and related structures after the injury. Considering your mother's age and the associated health issues mentioned. It's risky to go for surgery without a thorough evaluation of her health status. As far as the condition is concerned, you have to weigh the amount of risks and benefits out of the surgery and choose for the right one. Greetings Mr.Pramod, As an explanation to your Enquiry, the thecal sac is the outer sleeve that covers your spinal cord and in most of the cases it will be compressed by the disc bulging into the vertebral canal of your spinal column. And when you are mentioning only a mild disc bulge and pain on touching your spine. It might probably be an inflammation of your supportive ligaments of spine. I could roughly suggest that in case of a central protrusion of disc compressing the ventral/anterior thecal sac can produce symptoms as follows; # Numbness radiating to both arms # Stiffness of the neck region # Intermittent headaches of unknown origin # tingling sensation on the inner aspect of upper limb And to have a better look at the extend its always necessary to correlate the findings with many factors like : Age, Gender, Serum calcium and Vit D3 levels in blood etc. Regards Dr.Manu krishnan.K Founder : Proanatomy

Greetings Mr.Navi, From the minimal information that you have shared it's indicative of a postero lateral disc bulge causing thecal sac indentation as well as a probable left nerve root compression too. That is the reason why you have pain radiating to the left hand and also I am suspecting some early degenerative changes happening with your vertebral column. In order to look into it further it's inevitable to have a look at your MRI report films. Yes, But it depends upon the structure that which is compressing the thecal sac. Most commonly the disc herniations are the cause for thecal sac compression. In such cases, a minimally invasive procedure called "endoscopic discectomy" is done. K I had a medical emergency the other day where I went quadraplegic for a little while, I ended up going to the er because I was scared, they did a ct scan and found that cervical thecal sac is being compressed in the c6/c7 fused vertebra that hasn't fused properly from my acdf back in 2016 will I have to deal with this the rest of my life?

Hey doc would a indentation issue on the cervical area cause vertigo or balance issues? Have had tons of tests and MRIs and always have neck pain. C5-C6: Mild disc bulge with minimal central disc protrusion indenting the ventral thecal sac with mild central canal narrowing.

Greetings, As per the shared details of your MRI report, there's an evident disc bulge that is causing a compression on the covering of your spinal cord. Along with that there is a reduction in the diameter of the canal through which the spinal cord passes. These two structural changes itself can cause the following symptoms, 1. Stiffness of the neck 2. Numbness or weakness of hands either one sided or affecting both the hands 3. Radiating pain along the shoulder extending till your elbow. 4. And rarely as you informed there can be vertigo and associated giddiness. 5. In some other rare cases there has been spontaneous episodes of headaches without any known cause. It will be much appreciated if you could mail me the MRI reports ( including the MRI films). It would be helpful in explaining your condition further. Here Thecal sac refers to the covering of spinal cord. In most of the cases this covering will be compressed by a protruding disc(intervertebral disc prolapse) or some other masses. The cure depends upon the severity of the condition. In case of mild Thecal sac indentation, moderate spinal muscle stregthening exercises along with medications can give a considerable amount of relief and surgery is opted only for the cases with severe indentation and associated spinal canal stenosis. C4-5: left para central disc protrusion causing thecal sac indentation with mild narrowing of left neural foramina & mild indentation of left exiting nerve root.."THIS IS THE MRI REPORT would you please EXPLAIN THIS IS SAVIUR OR NOT.

When the posterior disc bulge presses on the Thecal sac covering the anterior/ventral aspect of spinal cord, that is known as anterior Thecal sac indentation.

@@proanatomy Hi sir I have mild disc bulge causing mild indication of anterior thecal sac .t C3 C4 and C4 C5 and C5 c6 level I can feel irritation in toungue and sharp pain in head and hands is this from. Disc indicating thecal sac ? Requesting you to answer ..sir

CT scan does cause a considerable amount of radiation exposure when compared to MRI. But there's no studies showing that a single exposure to radiation by CT SCAN to cause any side effects as u mentioned above.

Greetings Miss.Jyoti Verma, The recovery from the thecal sac indentation depends on many factors. 1.Your age 2.The extend of indentation and the site of indentation. 3.Wether it's a disc bulge or annular tear 4. Wether it's caused by an injury over the region. So I may be able to predict the prognosis only after knowing these facts. If you need to discuss upon the same. Because there are a lot of factors that have precipitated over time to cause this condition and its reversal is never that easy as you said. Hi doc,I have compression of the thecal sac at L4/5 from an accident I had last year,I have a bone fragment trapped in the foremen at L5 with bilateral recess. I have a slipped disc at L4 and osteoporosis. Would the mild procedure suit me? Thankyou

The mild procedure is a minimal invasive lumbar decompression procedure done as an outpatient with special tools.

It depends on the extent and the structure that is affected. If there is a profuse posterior disc bulge then it can compress the thecal sac along with the spinal cord within it.
