What the Biden-Trump Debate Reveals about their Mental Health

More than 50 million watched the recent debate between Biden and Trump, and countless more have been talking about it since. Speculation about the political blowback from Biden’s weak performance runs rampant. The president’s defenders excuse his verbal fumbling as a single bad evening. But we have extensive recent and historical video evidence to provide context, and to document how both men’s brains are functioning in 2024.
In terms of psychiatric diagnoses, the debates supplement a large body of evidence that:
both men show decrements in verbal abilities in excess of healthy aging
both men continue to display extensive symptoms of ADHD
Trump takes stimulant medications to enhance his performances
Biden displays multiple symptoms of Parkinsonism
Trump’s excessive falsehoods are consistent with narcissism
But why is it ethically appropriate to comment on these psychiatric conclusions?
The American Psychiatric Association (APA) and other professional groups claim that publicly discussing the mental health of public figures is unethical without an in-person evaluation and the individual’s consent. But no confidentiality is being betrayed by discussing information widely accessible in the public domain.
The APA asserts, despite evidence to the contrary, that an in-person evaluation is the gold standard for a psychiatric evaluation. Research shows that viewing videos of an individual’s behavior can result in accurate and valid psychiatric diagnoses. Compared to what one can obtain in an in-person evaluation, we have far more hours of behavioral evidence on video for both men, in a variety of settings, interacting with numerous individuals, at different times of day, over a more extensive period of time. We have far more corroboration of this information, from far more sources, than one can accumulate with an in-person evaluation.
Furthermore, diagnostic criteria for ADHD, unlike other mental health conditions are based entirely on observable behaviors, without requiring knowledge of a subject’s feelings, motivations, or other internal mental states. Thus we can attain greater certainty in making an ADHD diagnosis than for a personality disorder. Similarly, many of the symptoms of Parkinson’s disease are observable behaviors.
Although medical professionals’ primary obligation is to their patients, it has been long recognized that doctors have social responsibilities as well. It is a disservice to a democracy to allow ignorant and partisan voices to dominate the conversation about the health of its potential leaders. Psychiatry has an ethical obligation to say what it knows, how it knows this, and its degree of certainty regarding conclusions about the mental health of presidential candidates.

I think you are right on with your discussion, I have been practicing for 25 years. Mostly in the psychiatric mental health field. For me, Narcissism and dishonesty,/criminal behavior is disqualifying for me while Biden should be turning the nomination over to his vice president is my opinion, because I can’t see him continuing and his current status , though far less grievous then a threat to democracy itself. I would vote for Biden in a nursing home before Trump as long as we don’t lose our constitution and something is done about SCOTUS!

I’m very nervous after watching the debate AND the ruling brought down by the Supreme Court. Biden was very slow developing his thoughts, or even arguing about Trumps stupid ideas Now I would love to see Newsom and Whitmer. They are both young and energetic. If I were to advise Democrats that would be my pick If Trump gets in all hell will break lose. He will fire all lawyers in attorney generals dept, FBI, release January 6ers, and he’ll go after everyone he feels has ‘done him wrong’. I cannot believe Biden is going to stay in this presidential race!! Biden has a form of Parkinson’s. I’m 44 with young onset Parkinson’s and it’s like looking into a mirror. I don’t know why no one else is covering this. Like Biden, some days are bad. Some days no one knows I have Parkinson’s. It’s an unpredictable and dreadful disease. When Parkinson’s is onset at later age, the decline is faster than young onset. He’ll get worse fast

I appreciate your comment very much, bc when I think of Parkinsons, I think of our very beloved M. Fox. I didn't realize the symptoms could be so relatively mild, & to hear that from someone who actually suffers that awful disease is very helpful. Actually, I should say I FORGOT the symptoms could be so mild, bc I'm old enough to remember Fox being able to simply keep his hand(s) in his pocket in order to conceal early signs. GD: You have my full empathy. May you have good luck and have many sources of joy in your life. My father had Parkinson’s for 17-plus years. Music therapy (going to hear harp playing and walking) was super-helpful. Massage therapy and also improving and promoting his gut health and diverse veggies and fruit and bone broth. And listening to classical music. He ultimately succumbed from cancer. Best of luck and hang in!!! 💫🍀🍀🍀❤️

Doctor, I'm worried that Joe Biden has Parkinson's disease because he's showed many of the neurological symptoms such as staring without blinking for thirty seconds with his mouth open during the debate, and a slow shuffling gait which are all symptoms of Parkinson's. Biden also has a chronic throat clearing problem and a softer raspy voice which can also be a symptom of Parkinson’s. I definitely won’t vote for Trump no matter what, but I’m worried.

I’d like to add to the previous comment that the “tone of a nation is set from the top”. Leadership is a skill. Every word you say as a leader has the weight of influence. This is not a wooey sentiment about some vague notion of leadership, but a tangible concept that leaders are taught. You could see how just a few angry hints from Trump sent a riot to the capitol. Every word you say has influence. You want someone skilled at responsible leadership, who demands loyalty to be reserved for ideals/principles not people. Someone who understands their role as BOTH a leader and public servant.

Who in their right mind would want their future to be determined by that insane non-intelligent unwise man, and have their children influenced by his vile nature? "Leader" indeed, showing everyone the way to hell on Earth: coming soon to a planet near you. And you will find peace of mind only when you know what I know: that ALL is part of GOD's perfect plan to save us from ourselves. And EVERYONE WILL be saved, after many many visits to this planet. I watched Trump speak for 2 hours with no teleprompter seamlessly transitioning from topic to topic. Even him talking questions from a hostile press day after day is a stark contrast with Biden. Just because you don't agree with the decisions does not mean they weren't made. You certainly can have your own opinions about his character, but I don't think anyone can honestly say that he shows signs of dementia. Personally, I think biden shows signs of lewey body dementia. Gait changes, mask like facial expressions, initiating movements. And I feel sad watching him. I may not have voted for him, but he is my president and a fellow human.

It certainly can, but doesn't always. Depression is considerably more common among those with Parkinson's, as are certain forms of dementia. But many people with Parkinson's don't develope those "complications".

Trump didn't pass any cognitive test. He reported that he was administered a cognitive screening tool, that normal individuals are supposed to "ace". And from everything he has said about "Person. Man. Woman. TV. Camera." he was incompetently administered an invalid test, so even "acing" it counts for nothing.

Thanks for the video. I strongly suspected these conditions of both of them, as well as rheumatoid arthritis in Trump (because he said he has a history of hydrochloroquine and his tanning bed use...which relieves pain in RA, as well as his gait). Very informative.

I lived in NYC back in the 80s when T was always in the news. To me he seems like the same but then again I filter out most of what he says hahaha. B, on the other hand, is just….not there anymore. Muscle memory, canned phrases and teleprompters carry him along 😢

The challenge is separating out what might be ADHD and what might be from dementia. Tangential speech and jumping abruptly to other topics are classic ADHD symptoms. There is far more overlap between ADHD and dementia than most people appreciate.

Is it possible for phsychiatrists to have deep Christian beliefs? Afterall, psychiatry is the study of thee most complex of human organs created by Jesus. Only he knows what is/has been "in the minds of, oh, let us say, 25 billion humans created since Adam. It is a rhetorical question, as nobody can answer it, only speculate. I think that psychiatrists can have deep Christian beliefs or Hindu beliefs or be atheists or agnostics. Psychiatry (usually) doesn't attempt to offer spiritual explanations.

IBiden’s behaviour reminds me of my mother’s decline after she started taking a statin medication. I thought she had Lewy Body dementia ( a form of dementia which presents with Parkinsonism features -reduced facial expressions, a degree of rigidity in her gait, taking short steps etc). When the statin medication was discontinued the Parkinson’s-like features disappeared and her cognitive function returned to its normal baseline (she was 80 at the time.) I think those P features were due at least in part to statin’s adverse impact on her balance. Statins have been shown to improve cognitive function in some people, worsen cognitive function in others and have no impact on cognition in still others. My mother’s capacity to maintain her balance was again gone once she’d been off the statins for a couple of months.

So true. But we need to see who brings who to the Party. Trump brings Stephen Miller, Laura Loomer, Steve Bannon, disgraced Rudy, Roger Stone and more. These people are all criminals. Including the justices. They permitted themselves to take gifts and bribes with no impunity. We have the House of Clowns who have done nothing constructive but shut down the govt every time they didn't get their way. Biden is not perfect, but at least he has Kamala (like her or not) and people who don't want to destroy democracy

The alternative is a convicted felon, rapist and fraudster who has 3 more criminal trials hanging over his head. The choice is pretty obvious. Apropos of my last comment, re, the stroke, I failed to mention that I suffer chronic anxiety/panic attacks. I am not healthy, smoke 30 cigs a day, am all "stented out", have angina, by NEVER GET A COLD, started a trade marketing business with absolutely FA, in fact, less than FA. I will put this entire story in an email to you. WTF am I still alive, as I yearn to be off this planet!!

I swear and blaspheme badly, as I blame Jesus, as the creator and architect of " all things" human. I have opinions on everything as can be seen from the many comments that I have sent, and will continue to send, and they are SINGULAR, although, IMO, well reasoned out and logical. I am 69, have had a rough time of things, am with a stroke, and when I see petulance like Drumpf exhibits, I lose it, and start having bad thoughts about what should happen to him. I know this is wrong, as " shit happens" to all at some stage. The huge difference being is that I have more INTEGRITAS in my small toenail,and more courage and honesty than most, and I just cannot handle people born WRONG!!!
