This is the second one of your videos I've watched about after-care of an injection - cortisone, I think. With the first one I was surprised at the details and realized that was no simple shot. But it was helpful after my own (shoulder) injection, because I didn't get that level of instruction from my doc. I wasn't surprised to find the same level of detail here for a different kind of treatment. Even though I haven't had PRP I am thinking it might a consideration later on (for a different joint!) so it's good to know what to expect. Thank you.

My goodness ! I love your detailed explanation . It’s my first time landing on your channel , and I truly feel it’s god’s intervention. I have been suffering from a rt knee pain for about a year and just recently diagnosed with knee osteoarthritis. The pain and the restrictions on my mobility has really affected me both mentally and emotionally. Both OTC and prescriptionN NSAIDs have not been very helpful. I am now considering one of these injections, and I find your video so informative and encouraging and has given me such a sense of hope that I am now seriously considering the procedure. I am enjoying the information provided for pre/post PRP injection. I have been struggling with Achilles tendonopthy for 2 yesrs and have been recommended PRP by my doctor. I have been doing a lot of reading and have found a mixed bag of reviews concerning PRP and the Achilles tendon. I am curious to know if you have any insight or experience in treating the Achilles with PRP and if you can share anything with me 😊 Thank you!

You’re going to read a lot of things about PRP online. I am very honest with my patients that this is not 100% effective in all patients. Patient selection is key. I tell people it is neither snake oil, nor the elixir of life, but sits inbetween. The vast majority of my patients that I perform this treatment on do show levels of considerable improvement. But this is after many years of experience, and knowing which patients would likely benefit from the procedure. Achilles tendonapathy can be extremely difficult to treat. However, I find that tendon injuries tend to respond better to PRP then osteoarthritis or joint issues.

Make SURE it is a 30 ml tube. Make sure you are in PT while healing. I have excellent success with this method. Jeffrey Peng is great here

I am going in for a consult this afternoon.. I've had medial epicondylitis for 2 yrs - two. 6-week rounds of PT, 2 steroid injections (2 years apart), and the pain is worse than it's ever been. I've been reading about PRP vs. PRP+stem cells (and, perhaps, adipose tissue?) But I recently listened to a video of a Dr. talking about multiple studies that show PRP ($) is just as effective as PRP+Stem cells ($$$). What is your experience/opinion?

I recently had one ultrasound guided PRP injected into my right posterior tibial tendon after 3.5 years of unsuccessful treatment with conventional therapies (brace, rest, physical therapies). It’s been 4 weeks and I am not experiencing any relief from the procedure, any suggestions. My condition is bi-lateral and therefore may require injections into the left tendon

yes I ask my patients to avoid anti-inflammatories, because it will block the PRP process. We want a certain degree of inflammation during the treatment.

Today, I just had PRP injections in my left knee for torn meniscus and arthritis and my post op direction was consistent with yours EXCEPT ONE thing: you recommend against any heat but my Dr encourages it via hot moist towels or heating pad on the joint. Heat is likely okay. Ice is the big thing you want to avoid since it’s anti inflammatory and we don’t want that.

Where did they inject you? I had one the other day for the first time. For starters, they withdrew 15ccs of fluid off of my vastus lateralis just above the knee. Then they injected me in a similar nearby area with my PRP. They didn't inject me in the actual meniscus tear like I thought they were going to🤷♀️

Evidence will likely be mixed. You will hear multiple different view points on them. Theoretically the anti-inflammatory effect may be minimal. I recommend people hold off on them for a few days prior and after, but probably not detrimental if you are still taking them.

It can be done. Likely will be addressing more of the underlying osteo. It will not be effective for treating the inflammatory RA (which usually needs oral pain meds/biologics to do so). However, I have treated patients with RA with PRP.

PRP is less commonly indicated in KL grade 4. Its more effective in mild OA, even moderate OA. However, in bone on bone, it is really a shot in the dark, and not indicated. At that point one would be thinking about surgical options.

@@douss11 It’s been just over 1 month. Pain is still there, but not as much. There is some things I can do than before, like brush my teeth. Shaving is still a issue. My specific issue is turning my wrist a certain way. I was told I have multiple tears in my shoulder, 50%. I believe I caused this injury doing pull-ups, palms facing each other. Bringing my arm toward my shoulder in that same position at rest, or wrist turned a little more clockwise hurts still. Any sudden, fast twitch movement, it’s a very intense pain. I was told not to do anything until my next doctor visit which is in a few weeks. No therapy, or nothing. I think my progress will be slow. Hopefully it’s slow, than surgery. I’ll give an update after the visit on what’s next.

Yes it can. It’s actually quite a common reason will we treat with prp. These areas can be quite tricky to treat as a whole.

I have had 4 PRP injections since 2022 in the shoulder for impingement/bursitis, rotator cuff supraspinatus & infraspinatus tendinopathy, articular surface fraying and glenohumeral OA. It helps for a while but the pain always comes back. My question is; Does PRP help regenerate? I am on antibiotics for urology issue for months, will taking antibiotics interfere with the PRP?

PRP can regenerate tendon, but it does not regenerate cartilage, it can merely stabilize it. That is why you are likely seeing pain return. However, tendons can be regenerated/healed with PRP and in those cases only a few treatments (or one) may be required.

Thank you for posting these videos, as they are truly educational! Would PRP likely reduce further damage and pain associated with hallux limitus / rigidus? Currently I have a painful grinding / grating sensation and limited mobility in my hallux joint. My podiatrist has suggested a cortisone injection and eventual fusing of the joint, to address arthritis pain, but I do not want to lose the mobility in my foot (as I am very active, climbing ladders, swimming, hiking, etc.). I feel that the grating / grinding sensation could be caused by a bone spur (or something similar in the joint) which, if removed, might reduce further damage to the joint, and allowing PRP to be more effective. Would this combination of surgery and PRP likely provide the best long-term option for active use of the joint?

It's not routine to do these injections under GA, at least not commonly in Canada. It's not usual for people to experience much pain with the injection, rather I find pain can get worse afterwards for a few days.

I go to the VA .. and i take a lot of medicine.. I just happened to check my medications 1 by 1 myself.. and about 2/3s had a negative reaction with prp.. but I was only told about the pain reliving meds . Anxiety and sinus meds also were said to have a negative affect. Also for my PRP to my hip.. this doctor went in straight through my hip through all of the muscles and things.. the pain kicked in about 3 hours later. And it was terrible.. I could barely breathe when I had to walk.. the pain was a solid 8 for nearly 20 hours.. I had the shakes it was so bad..

This will not treat the underlying cause of RA. Usually this addresses the local cartilage damage. If you have some OA in the joint, then yes.

Hang in there. The area has purposefully been inflamed with platelets. Do NOT take meds. Pain will go away in a few days. PRP really worked for me. Hang in there. No meds or you will ruin the treatment!

Its not uncommon to give a small dose of high strength pain relievers for a few days. Many patients may not require it, but tendon PRP can be painful which is why its nice to send people home with something incase you need it. However, if you don't require it then thats great as well. Patients just cannot take NSAIDS which is why this is offered.

2 weeks ago had a tenex procedure on medial side of elbow for scar tissue and calcium deposits. One week ago I had PRP on lateral side for torn tendon. The doctor also injected the PRP in the joint as he indicated I had a lot of arthritis/inflammation there. The swelling and pain has subsided though it is still tender. Hot tub acceptable after 2 weeks since PRP? He indicated 6-8 weeks before I can get back to weights and golf because of injecting into the joint as well as the tendon? Sound accurate? I’m a person who consumes alcohol regularly. So after my first PRP injection to my shoulder I was told no alcohol by my doctor. But I wish to know for how many days do I resist alcohol after injection and also my regular exercise routine?

Alcohol makes your blood thinner and induces even more inflammation. This is why we ask patients to avoid it prior and a short period of time after. The other thing is alcohol honestly has no positive effect on the body and is really slightly toxic at any dose, but that is another issue/concern entirely.

Excellent video. I had my PRP into my knee joint and my doc gave me no aftercare notes or instruction. I am 4 days in a feeling no pain at the injection site, can I get back into my regular workout routine and golf? I am still unclear even after watching the video on whether this could damage the results of the PRP.

Thank you! This is a difficult question to answer without knowing age, the type of tear, and the mechanism of injury. Usually smaller tears in the vascular portions of the meniscus and younger patients tend to heal and settle down quickly. On the other hand, bucket handle tears and those tears in older patients with OA may be less inclined to settle down with conservative treatment. The other thing to consider is if someone has mechanical symptoms, like locking, catching, and buckling. In some cases, those symptoms may require surgery to settle down. On average about 70% of meniscus injuries can settle down on their own, but like I said MRI findings, and patient demographics are really important.

The (to be confirmed by MRI) tear limited the range of motion on my right knee towards both ends by a little - maybe 5 degree or so, however it didn't cause any locking catching type of thing. But it is painful, especially after longer runs, turns during a run, standing up after sitting for a while. I'm considering PRP injection, your video is great, useful and easy to understand.

Hello please advise me on what to do My daughter does Rhythmic gymnastics she is 12 years and suddenly had shrapnel pain in feet could be from traning. When we did MRI it showed that there is edema on bone. The feet bone has water and inflammation. Now I am advice to do PRP and traumeel injection. She is also doing magnetic therapy and lazer. Would you recommend to do the plasma injection and the traumeel injection or just one thank you

Hang in there. Doctor said it can take up to 8 weeks for recovery. My pain was gone after 6 weeks. Your area is inflamed and that’s what is suppose to happen. Do not take meds! Pain will start to decrease. PRP really works, just give it time

i advise no NSAIDS for approx 5 days, other pain killers can be taken earlier as they do not effect the PRP

I'll be doing my first consultation next week for elbow and ankle problems - thanks for your helpful tips! I have recently started doing cold plunges, three times a week and only a few minutes each, in a filtered and ozonated tub that I built. Would it be OK to resume them after the 24+ hours of the infection risk passes?

Theoretically, I'd advise against cold plunges for 4-5 days because of its anti-inflammatory effects (not just infection risk).

Adductors can be tricky but yes prp can also be considered in this area. It’s extremely important to make sure you work on an adductor strengthening program as this can be an area easily reinjured

I’d start with PRP personally. Both options have their benefits. Sometimes my patients want to use their drug coverage so we start with HA. But I do prefer to start with PRP

@davidtorna7514 I have the same issue in my right hip. I just received my second PRP injection 2 days ago, one month apart exactly. I asked the physiatrist and he said he injects directly into the joint, and right behind the labral tear if I recall correctly.

I do follow ups at three months because sometimes the responses delayed. I tell all my patients to wait three months before confirming that it did not work for them. Most people get the benefit around 8 to 12 weeks

I do not perform this procedure on pregnant patients. This is one of the contraindications as we don’t have any studies as to how this may affect the baby. Unfortunately, it is an exclusion.

). In general would you put a restriction on driving during phase one? Phase one post PRP relative to being a single father (50/50) healthy co parenship. Days I drop my son off consist of approx 72 miles in total. Surgery 7 months ago for partial Achilles tear and bone spur in my heel. PRP scheduled in a week. Greatly appreciate a response

When I perform a procedure on someone's driving leg, I always discuss with them if I would restrict driving. Usually my cortisone/Ha injections don't have much of an issue. But with PRP, the injected area will be quite sore and extending driving if is the driving foot may not be the most comfortable. I always encourage you to ask this question of your doctor prior to receiving the procedure.

Hello! Dr Sonam Namaste I am from Nepal and have been experiencing issues with my left meniscus for the past four years. Up until a year ago, I didn't have any significant pain, but now the pain has returned, and I am unable to stand on my leg for more than 20 to 30 minutes. I started going to the gym to strengthen my quadriceps, but I now also have issues with my right meniscus. I consulted with doctors, and after examination, they informed me that my condition is not too severe. They suggested Platelet-Rich Plasma (PRP) treatment as a possible solution. However, I am experiencing pain in both legs. I would appreciate any advice or guidance on what to do next. Thank you.

the first question is diagnosing why you have a hematoma prior to injecting the area. I do not inject hematomas with PRP

Dr.Its pity that prp injections has not the same protocol. I did get 2 times a shot based on 20 ml blood and there was no possibility of double spin so the effect cant be very good. The docter here said in three injections you have the same effect as in 1 shot of 60ml. Is that right? In the Philippines there is no possibility to get 1 shot with the very best options.

The protocols are truly very difference. Double and single spin as well. I've used all sorts of machines with different protocols and to be honest the end result is quite similar to them. However, because there is no standardization it will be hard to get a concrete answer.

Each centrifuge is different, the products also change. I've used single and double machines. Honestly, at the end of the day, you are looking for a high platelet count, so some machines can do it with a single spin (they are getting more advanced). Look at the quality of the end product, not the number of spin's it may need. I use a single spin right now and its got amazing cell retention.
