Makes me think of Stephen King's Dark Tower series, one of the characters is a heroin addict and it's described as a "monkey" or "gorilla" on his back. For real, speaking as someone who works with both addicts and people with schizophrenia (they often go together), there is definitely an amount of beyond corporeal (whatever that means to you) risk with drug use. (In addition to the bodily risk, of course.)

That’s a Drug Spirit, my stepdad is a medsin man and I’ve heard him talk about how when we do drugs a drug spirit will follow you around. I had one when I was an addict and mine was this huge black mass that would show itself to people around me when I’d be having emotional meltdowns. Scared my cousin pretty bad one night and took me a looong time to get rid of it.
