In Das Boot (1981), the whole overboard scene was not originally in the script. It came about because one of the actors was accidentally washed off the studio set of the submarine tower by an artificial wave and all the others were so top-class in their roles that one of them actually shouted "man overboard!" and everyone behaved as real sailors would in such a situation. The guy actually broke two ribs during the filming of this scene and scenes were subsequently added in which his injuries are treated. Also when the boat finally pulls into port at the end of the film he is taken off board on a stretcher.

In the 1978 film Convoy, actress Madge Sinclair's truck turns over rounding a curve. Sam Peckinpah in an over head chopper with both truck, ground and helicopter cameras filmed the accidental turn over and kept/added it to the script. The chopper camera captures both truck driving actors and production crew running to the truck to check on her. She was ok.

There was the helicopter crash in Attack of the Killer Tomatoes. The helicopter ACTUALLY crashed, but it was so amazing (and expensive) that they added the line about a tomato flying right at it and left it in the final cut. Monty Python and the Holy Grail. The famous "Tim the Enchanter" scene. John Cleese was SUPPOSED to give this really long and complicated name, but when the time came, he forgot and adlibbed Tim on the spot.

When Leo DiCaprio in Django with him slamming his hand on the glass because he misjudged slaming his hand on the table and got the glass instead and you can see surprised look in the faces of the rest of the cast. not sure what was improved with the blood on the face of the women but i know that some of that scene was inproved and the blood was real in those scenes as leo really did cut his hand bad.

There is a scene in Kubrick's "Dr Strangelove" where General Turgidson trips over a carpet or something during a heated debate in the War Room. Actor George C Scott stayed in character and continued Turgidson's rant. I don't have a source, but I'm convinced the tumbling was unplanned.

Also...Scott's whole performance was "overdone" by him as he thought/was told it was just a rehearsal test filming. So he hammed it up more than he originally planned but Kubrick liked it so much that he kept that version in the film.

It's possible that she did the original "Kevin!" as a cover but that they liked it enough to film it again but from the angle we see in the movie. Which I just watched for the second time since it came out. Kinda sucked except for her, Joe Pesci and Daniel Stern.

Believe it or not the bit where the white truck rolls when it’s going through the four way intersection in the film convoy was not planned at all, turns out the tire blew out as the white truck was going round the corner and that rolled the track resulting in the crash and the black female truck driver I’d imagine improvise the line where she says damn piece of white junk I know I should’ve brought myself a black truck and then she said let’s haul ass you just got yourself a passenger. which explains why she’s riding with the two white guys for the rest of the film and of course they left it in the movie.

You are right, Marge Sinclaire improvised it and was really mad. Also, the scene when Dirty Lyle crashes threw that barn roof and landing on the other side wasnt planed either. Ernest Borgnine was supposed to drive down the slope into the barn. However, due to his exeleration he crashed into the roof. Both scenes happened so fast and only the quick thinking of cameraman Harry Stradling made it possible to be part of the movie.

The flag, maybe the first time, but they went back and fully wrote it in, it's in way to many shots to be a first take accident and the cameras set up to catch it perfectly. As for Casino Royale, no chance that was an accident, it mirrors Ursula Andress way too closely and it's not the first time a later Bond director copied, seem to remember 'oh it was an accident' being said about Halle Berry did the same thing to dodge critisim at the time as well since it's so blatant

You definitely have to look up the scene in goodfellas where Henry Steps right on karen's hand when they get up from the bed, in the " gun in the face" scene In the bedroom. You can see she holds in her scream and Then what she screams "I'm sorry" you could tell she's in real pain
