Itachi was one of the very few capable of doing such a feat at that time. If it was Danzo that attacked Fugaku then Fugaku wouldn't have just sat there and allowed himself to be killed. Mikoto would've joined in as well most likely if she had the sharingan. She knows shuriken jutsu since she offered it to Sasuke once.
