If you go to a store that has plumbing supplies. They should have them. They are adjustable.

What keeps the jam nut from coming loose over time due to vibration, etc?

Nothing really, as long it is tight I've never seen one come loose.

With your hand on the way, the camera not zooming extra close, the shaft not in position to show how the thing works, no explanation about how much dripping, teflon cord options, other stuffing box designs, this really isn't of much help.
