The aim of this video is to explore some of the options for lumbar spinal stenosis. What it is, why it happens, and what surgical and non-surgical approaches exists, so patients are better informed to make decisions with their doctors. Please note an error in the recording. I state erroneously that it is 2013. It is not. This is recorded in 2023.

I am a 28 year chronic back pain sufferer, and I just turned 55. I’ve had 4 lumbar back surgeries including 2 fusions. I’ve never had a good outcome from surgery. I’ve been told this is all from a congenital condition. I recently had another MRI which showed more stenosis and a few more herniated discs. Went to a Neurosurgeon here in Orlando to talk about an Intrathecal Pump. The surgeon said I need too have further surgery which includes fusing S1 all the way to T10. And then probably have the Intrathecal Pump put in. Back pain is absolutely horrible and very debilitating. Anyone that suggests pain patients are making it up are very ignorant people. Prayers to everyone that suffers from chronic pain.

I'm so sorry for the pain you're feeling and having, I'm a 19 yr suffering chronic back pain, L3'L4 & L5, I can relate and agree.. those who have never experienced this type of pain has No Idea and should never speak on or against anyone who's suffering.

Peggy. I see you too are suffering, At 19?!!? It's a terrible thing, I have some pretty serious back issues going on. severe, chronic, for the last 6 years. In all my years of spinal problems I have never had the pleasure of meeting a Doctor so open and honest about the possibilities of available treatments. We currently have Kaiser insurance in Ca. They will not even have me meet with a surgeon, rather the "gatekeeper" will only allow epidurals, that over the last 15 years have never worked. The current injection is scheduled for Dec 20th, after first seeing the doctor about it in June. At 71 I don't have 40 more years to deal with this, like I have so far. Dealing with the pain for decades has made an old man of me.

I have stenosis too and also have Kaiser in California. I’m in Southern California near Los Angeles. After going through 3 years of various pain management procedures I’m getting a referral with a surgeon with Keck usc . It is possible ,just keep pushing and change doctors or locations if you must. Don’t give up.

I appreciate your honesty. I, for one, am beyond tired of surgeons jumping right to "we've had great success!" Without ever mentioning the numbers and the fact fractures, erosion and other problems that potentially caused by the surgery alone. I've been in and out of physical therapy since my late teens, now I'm nearly 48. I'm also in pain management on morphine ER, Norco, Lyrica and Butran patch. I've maxed out on everything. I'm just about out of cares. To put it nicely.

Thanks Doc. I’ve been suffering from back pains for over 20 years after a failed lumbar disectomy . Stenosis set in over five years ago. I’m in agony. In pain 365 24/7. Very little sleep. My neurologist prescribed gabapentin. I’m at the highest dose with very little relief. Save for the mercy of God, I would take my life. Matter of fact I have no life, the pains consume me.

I know exactly what you are going through, and so sorry to know anyone else has this pain and nothing in the horizon to look forward to. Pain Mgmt doc has helped some with shots, at least it is somewhat relieved for a couple of months. It is worth the pain to get there, and then ask for any kind of pain relief. Prayers for you. Meditate apps help me a lot also.

@@12221943 hi, I had a laminectomy in January . Five of my vertebrae were affected and my spinal cord was released. Two titanium rods were implanted, held in by ten screws. Thing is I continued feeling intense pains even more severe than before the surgery. After weeks of agony, and repeated testing it was discovered that one of the screws was impinging on my nerve. (This pain is beyond description, I prayed for death but it seems God was not ready for me) everyone in the hospital including my neurosurgeon thought I was going crazy, doses of morphine was given to me without any relief. The surgeon decided to review the surgery . It’s now six months and I’m still in pain and unable to walk independently. I’m told it could take up to a year for the nerves in my legs to regenerate. This surgery is not for the faint of heart. It’s a very slow and painful recovery.

Thank you so very much for taking the time to present this thorough and informative video. I have an 85yr father and 70yr old friend suffering from this issue, (I'm 65 and am already experiencing the first signs of Spinal Stenosis symptoms) and will definitely share your video with them. Hopefully we can find other alternatives to avoid a Laminectomy.

PLEASE! Especially ladies of "a certain age" : You MUST be sure you get yourself a bone density test done BEFORE any back surgery! Not doing so prior to lumbar fusion will most likely cost me my life. Long (very long story, but readers digest version is....Screws, rods, the whole set up failed. Bone quality was not sufficient to hold the devices and I have zero stability. The cages put in place between the vertebrae have long migrated out and have now made contact with the main arortic artery and a large vein. No Dr wants to do any revision and all consisder it very high risk to even try . Will have to travel out of state for any attempt. There is no such thing as "minimally invasive" surgery when it comes to the lumbar area. Please don't risk your own life. If your surgeon has not brought up the subject of a bone density test, RUN!!! And yes, I AM trying to scare you. I pray it works. No one deserves the hell my family has gone through in this siutation. I know how desperate one can become when you are in constant back pain, but go slow. I would trade my old pain back in a second to be out of this situation. All I can do is hope this saves someone else.

Sharon here: I am so sorry to hear what you've been through, and continue to go through. I have stenosis in my neck and now have been diagnosed with lumbar spinal stenosis. My ability to walk and go through normal chores has been very limiting. I feel for you.

I too have spinal stenosis in my lower back and cervical spine. I am a woman of 83 yrs but I walk so bent over that I look and feel 110! Injections didn’t last even a week.

By far the best video and explanation to the anatomical makeup and and understanding of the physiology of the spine. I’m about to have second major surgery on my back at 67, and have a much better understanding of my diagnosis and coming surgery. Thank you so very much Doc.

Thank you Doctor my wife has spinal stenosis.she had a spinal cord stemlater witch doesn't really work. No doctor has really explained listen to you has been more informative than any doctor she has went to. Thank you

I am a 74 year old male with degenerated L5-S1 and mildly herniated L4-L5 and level 1 spondylolisthesis and neuropathy in my feet, and persistent low back pain, but I swim a half mile every day and walk 1.5-2 miles every day. A surgeon recommended I have both disc spaces fused by ALIF combined with laminectomy with plates and medical screws. I think that is way overkill for my issues and I fear I would never recover from that much surgery..

This was so informative. I've been taking epidural shots for 3 years now and as time goes by, they get less effective. I'm thinking of having surgery and want to discuss the minimally invasive surgery options with my doctor.

The doctor is ripping off the insurance company and not doing you any good. They forget to tell you that these shots weaken the bones if you get too many. They tried this on me in 2001. They aren’t very effective, at least they weren’t for me.

Pregabalin is helping some. I push through my pain and walk as much as the pain allows. I was in a car accident where I literally was thrown out of the vehicle and rolled head first down a cliff. I have lots of arthritis. I am not going to give up.

Thank you for the great explanation. I need relief of the L1-L2 new condition in the last year I fell to floor and back hit door and doorjam, talking to me about the MILD. I have old damage at L4 L5 that already fused by itself from herniations 30 years ago from a fall down 18 steps. I have been advised by neurologist never let them touch your back. But with the minimally invasive the procedure has been told to me can safely go in to L1L2 cut some bone. No fusing. I will still walk with an upright walker because of the old injury. But the new injury having MILD might aleviate burning down back of left leg and cold water feeling going down right leg, and lots of pain and numbness in the legs and feet more so than in the back itself. I am not diabetic. This is the first video that has actually given me a good vision of what can happen, I have many spinal benign tumors and they tell me the spinal ligaments are stiffening which I did not exactly hear that verbage in this discussion. I am having to have a repeat MRI because I had some other short term situations with my health that meant we could not move ahead. Will then return to Neurosurgeon to determine surgery and if any changes. They told me I would be in a wheelchair 20 years ago and I have done shots, physical therapy, many times when thing s would get worse. I also had a fall 2 times in one week directly to sitting on my pelvis and now have problems with S1 S2, I am just fighting to keep walking. Do know the smal risk the surgery could have that I could lose my ability I still have but the sensations would be great to eliminate.

Right on point with the explanation of the problems of spinal stenosis and the primitive procedures at this time to alleviate the pain and cure of the problem, the future seems to look a little brighter but will have to wait longer. It is refreshing however to listen to a doctor actually speak truth, wish the surgeon I spoke to had, though I declined the surgery and after listening to this man I'm glad I did. Thank you.

This brings back memories of a consultation in 2005 with a neuro surgeon. My symptoms were extreme pain, numbness and weakness in my right foot, as the result of a ruptured disk at L5 S1. He recommended PT and chiropractic, before considering a surgical solution. Both helped, at least well enough that I could return to work. Now retired at 73 I still have some weakness in the right foot and a bit of numbness. GP believes its permanent nerve damage. With the surgical advances today maybe a minimally invasive solution will work for me, if needed.

Mine was caused by an aggravated assault and a psychiatric facility I’ve been like that since I was 54. I’m now 70 and I’ve had surgery. But the stenosis is coming back. And I’m falling down more. 🤨 I’ve had the laminectomy, L5 S1. I’ve had the shots they don’t help. I put myself in a swimming pool with the tube around my waist and I walk. Few and far between that I can get to the gym and do that for myself. And yes, it’s painful the sciatica. The pain in my legs spasms. To the point where I am bowel and urinary incontinence. Great explanation & demonstration! In the next 20 yrs plus. Better for patient & skill of physician 👨‍⚕️

Thank You Dr. That was the best explanation of what I call one of the worst chronic diseases a person can have. I have had 3 spine operations and a fusion at level 4 and 5. Level 3 and 4 have deteriorated so badly even my buttocks and upper legs are partially numb and I suffer with severe pain. I wish there was a surgeon in South Africa that could explain this you have and to be able to this minimally invasive surgery

Excellent explanation of mri since I am seeing surgeon tomorrow. I was hoping to hear more on the various MIS including microscope use versus tubular endoscopy unilateral and biportal and where that sits in current understanding of risk benefit versus age old McCoullough technique and recently a mild approach directly through ligaments between the spinous processes by shaving each spinous process and inserting a tube to then go in only splitting ligament, no muscle resection. Theres alot going on in mis as talked about in Beckers Spine Review. The cutting edge so to speak but what I hear you saying with all of these is can one get enough decompression without hitting nerves causing damage. Thanls!

I am so thankful to you for your explanation. Because of a really bad car as a teen, I have been suffering with spine (the whole spine) all my life. Surgeries, procedures and injections have been my life. Pain pills have been my saving grace. Thank god I only can take a couple a day so I have not had an addiction problem. I am perhaps facing a lumber surgery which I do not want to do. So, you have very much educated me and for that I am truly grateful 🙏

I am an 80yo and for about 30years ago started having back pain off and on. Last year was worse. Thanks for the clear explanation of pros and construction of all the different types of surgery.

Thanks for the information. Explained very well. My doctor, like most all doc's, are being limited by what the insurance companies tell them they can do. So we all have to jump thru the hoops, while time ticks away, until there is no other option, except surgery, which should have been done to begin with.

I appreciate your clear description of the problem, anatomically, and the implants that havd been used over time. I am very interested in the idea of reexpanding the discs so they can serve their original purpose again. The intervention I read about is called Discseal. (?spelling).

Best explanation we have seen. We saw a video on ultrasonic bone removal tool rather than a burr to open the canal. Does not damage the ligament/soft tissue and no fusion required. Have you looked into this? 85 year old wife has severe stenosis that looks like arthritic blockage in addition to the disc shrinkage. Shots did not help. We have an appointment in January with a neuro surgeon. We are trying to find out what the latest options are. For some reason I am not having problems.

I'm 76 and have always been active in walking and swimming and am now suffering from lumbar stenosis and thank you for your thorough explanation. What do you think of ultrasonic tools/procedure used to ream out the spinal canal? That seems to me to be the most natural without needing further surgeries.

Praying for you!!! Lord you're spine issues dwarf mine. I used to be able to tolerate the pain and push through it pretty well but I think I've used it all up. This past year I spent more time off my feet then on just because even fixing a simple meal I then on just because even fixing a simple meal I get such severe back spasms a nerve pain down my leg that I have to take a break and lay down to get relief. If I I tried to stretch the muscles pull back even harder. Recommended fusion surgery years ago but added, eventually the next set will have to be fused, and the next as you get older. I just can't see myself going under the knife when I hit my 70's I'm just going to be screwed!

Im 68 years ols and on 6/20 i am having minimal invasion surgery for L2 S1 . i a have spiinal stenosis and spondylolisthesis . I am at the point where i can barely walk and i have pain in my lower back and groin. hopefulli get throught this ok . Bob M .

Had 8 surgeries I have stinosis congenital narrow spinal canal and rumitoid arthritis of the spine. I'm 66 my problems started at 38 . There's no cure for chronic pain you might find something to take the edge off . What ever you do don't have surgery at any VA hospital they did my last lower back surgery now I have drop foot and severe nerve pain with stabbing shocking pain to my feet. Haven't slept in a bed for 3 years. It's only a matter of time before I will be at my wits end with this and permanently end it . Good luck to the folks that are dealing with these problems.

What a great explanation of spinal injection using fluoroscopy.. Thank you 😊...you have made it clear what's involved and how it works,or in some cases doesn't work. Obviously having a clinician who is both knowledgeable and invested in his/ her practice makes ALL the difference in the outcome. So my question is this, if the nerve is highly irritated,swollen, will the administration of the dye or the steroid be more likely to cause pain?...as I would assume there would be less space available for the fluid to travel if nerve was more swollen (?) Thank you for the time and effort and thought that was put into this video. What a wonderful learning tool for patients and medical staff alike❤!

In March 2020, I had a CoFlex device installed at L4-L5 because of severe spinal stenosis that caused leg weakness without pain requiring me to use a walker. Surgery was out-patient and took an hour. No post-surgery care or therapy needed for me. just "get up and go" and without any pain. My result is walking strong without use of a walker. It is now May 2023 and there is now some weakness at backs of both thighs when walking. MRI shows severe spinal stenosis at L3-L4. Ortho surgeon who did my CoFlex installation plans to do the same again, and I anticipate that the same great results are realized!

thanks for your time ive just gota execise and do heaps of crunches only had mild problems when i smoked pot ive stoped smoking pot now for two years and its been a crap of a ride through codine going back to pot and exicise it works for me keep fit

I'm happy to find this video and researching for my mother has found out she has this at 72 years old with daily pain, she is seeing a surgeon for the first time in a couple days/I really hope they can help her, and also she says something about discs...

im a 35 yo female with a congenital narrowing of the spinal canal, Early multilevel lower thoracic spondylosis, and C2-3: Normal C3-4: Mild congenital canal narrowing and uncovertebral arthropathy. Normal left foramen and moderate right foraminal narrowing. C4-5: Mild congenital canal narrowing and uncovertebral arthropathy. Normal right foramen and mild to moderate left foraminal narrowing. C5-6: Uncovertebral arthropathy without canal narrowing. Mild left and mild-to-moderate right foraminal narrowing. C6-7: Uncovertebral arthropathy and minimal left foraminal narrowing, otherwise normal but my Neuro said she read it and im fine. however i cant walk without a cane, im incontinent. my hands barely functioin properly and ive also been told i have Aphasia. its a struggle to get anyone to care when im in so much different pains from back to neck to migraine in the head. thanks for showing me how to read my MRIs better so i can educate myself.

Thank you for that thorough explanation. It gave more information and answered more questions than I have ever received from several doctors. Now I know the questions to ask when I look for options to relieving my lower back pain. Physical therapy has not work for me.

P.T. didn't work for me, either. Glad to hear I wasn't the only one. I often felt worse the next day, so stopped after nine sessions.

Excellent, understandable presentation. Apparently, the technology for treating spinal stenosis is moving rapidly, thankfully. I'm ready for minimally invasive and this video makes me look forward to meaningful relief!

I am Curious if you have heard of the TOPS procedure manufactured by Premia Spine. They are located in Norwalk, CT. The procedure was just granted a "Better than Fusion" Designation by the FDA and was given the coveted "Breakthrough Therapy" Designation as well for patients with moderate to severe lumbar spinal stenosis, degenerative spondylolisthesis, and related spinal conditions." I would put links in this comment but You ube eats the comment when you do that. The procedure has been used in Europe and Isreal for well over 12 years

I am truly sorry to hear this for you and who ever has it implanted. How long does it take to know if it works. Is there immediate relief when affective? Also, do you think it is because of the brand you chose? May I ask why you didn’t choose Boston Scientific Wavewriter Alpha?

i have had four spinal stenosis operation 2002/2005/2013/2017 the second one was done by a neuro surgeon which was successful for approx 8 years.All i think were Laminectory.The one that helped for 8 years was done by a neuro surgeon. Is the advance in these ops worth another op as i am gradually getting worse with pain if i walk any distance of more than 300yds i'm sitting or bending over,i have to really walk through pain to get anywhere.I excercise a lot and although i am 80 years old i take care of myself but if i give in now i will be in a wheelchair which will finish me,i just want to walk.

You did an excellent job explaining the different types of surgery as well as explaining this device. I think you said the BURR? I am trying to find a hospital near me that uses this sort of instrument and does this surgery. Because I will be having surgery soon, a fusion of the L4 and 5 and I have already been told by my doctor to expect more surgery in the future. I am elderly and if I can get this type of surgery done I am hoping I may not have to have anymore in the time i have left. Just need some pain relief.... Thank you for your video
