This is kind of common in anime adaptations of manga, in manga form is less of an issue because the speeches seem more instant and you figure out the timing of the events in your head. It would make more sense for the Okuyasu vs RHCP fight to last the handful of seconds it took the rest of the guys to show up but since everyone is talking it stretched up to a couple of minutes like if Josuke and the others weren't in a rush at all, I guess most of these problems are solved if the dialogue were thoughts instead of speech but mangaka have the bad habit of making them scream out loud every bit of info

My headcanon is that Joseph is the same troll he always was and that he's faking being helpless for fun. Also that he's immortal because he has Dio's blood. He still looks very similar to how he looked in part 3, just a little less muscular... and he was able to walk for a bit without a cane in this episode, and was actually pretty serious in a few cases. I bet he "tripped" on purpose and dropped his cane just so that Josuke would be forced to hold his hand.
