I cannot thank you enough for your assistance..I have Stage 4 Metastatic that has raised its ugly head in my lungs.. I was so shocked I had no idea what to ask Oncologist..I have started on Immunotherapy injections but had no idea of the knowledge you have given.. Thank you from the bottom of my heart ❤❤

I'm just starting down this road and am watching hundreds of KZbin vids to educate myself. I meet with surgeon and oncoligist nect week so will fins out the treatment plas. I want to thank you fir showing your emotions, your tears, your fears. Most other YT'ers put on a happy face. While I've gottrn a lot of good info, those happy faces didn't sit well. I'm also a sensitive, emotional person so was glad to see your channel. Thank You!!

Angela, I’m watching this video doing research for a friend who just found out her cancer has spread to her liver. How long has it been for you? Did your doctor tell you it’s treatable after spreading?

@@tiffanym_1155 Hi Tiffany, sorry your friend has to go through this but there´s hope with the new treatments and with good friends like you. My first line of treatment failed (to be precise I had a mixed response, there was some stability in some lesions, some reductions but new ones appeared in my liver). As the liver is the second more important organ in the body, it has to be checked closely, I´m in a trial with Enhertu (I´m Her2 low), I have received 6 infusions and the treatment is working. I still remember what my first oncologist told me in 2006, breast cancer is a traitor, it has to be monitored all the time BUT even if it returns it can be treated (mine returned after 16 years). Since 2015 there are a lot of of new treatments. I wish your friend good luck. Big hug from Madrid.

@yerbba - My mom (63) has stage 4 with 'mild pleural effusion' in 1 lung & most lymph nodes involved. She's on hormone therapy with Letrozole and Ribocolib since 2.5 months. Coulpe of questions: Does this mean it's Oligometastitic breast cancer? Can this be cured by removal of tumor? Is there any hope of being cancer-free? Thank you for sharing your mom’s situation. Oligometastatic breast cancer generally refers to a limited number of metastatic sites. Most of the time, the term refers to 5 lesions (spots) although some studies have included up to 8 spots. Treatment plans can vary. Surgery is usually not the first option in metastatic cases. Rather, the main goal is to usually to control the cancer with systemic treatments. Moreover, removing visible areas of disease does not improve outcomes, such as quality of life or survival. There are some cases where surgery might be done to help with symptoms. While being cancer-free can be challenging in stage IV (4), the goal is often long-term management and improved quality of life. I have stage 4 metastatic breast cancer to the bone. I developed an infection in my breast (cancer side) - I had an odd drainage discovered a small hole - woke up days later in terrible pain, purple breast, much larger in size, orange like texture & bloody discharge. Called Dr & was told to go straight to ER - I ended up in the hospital for 4 days on IV antibiotics round the clock a number of blood tests later they couldn't find a diagnosis other than cellulitis. This has been going on since January I have since had xray, Ultrasound and a punch biopsy which they say showed no cancer they put a stitch in - I was hoping the stitch would also help the hole heal. No such luck I now have a pencil size eraser hole and a slight discharge (cannot go without wrapped) & it bleeds. Just seems like after all the iv antibiotics and 4 weeks on an oral antibiotic that it would show some improvement (at least the color is back to normal & the pain is so much better). I know & understands you can't give exact medical advice but do you feel like I'm on the right path & doing all I can? Thank you for your time Dr. Jennifer Griggs. Sincerely, Juanita G

I am HER2+. I don’t think they tested the tumor profile, it was recommended to have double mastectomy, ovaries taken out and started on the highest dose of oral chemo and hormone blockers. Currently taking 100 mg and an Anestrozol and never feel really great. I have been in remission for almost a year and keep asking if I can just take the hormone therapy. Why do I have to keep taking the chemo? Can I take a lower does? Is there integrative therapies we could do?

Thanks for writing. In general, we recommend starting with endocrine (hormonal) treatment in someone like your mother. Hormonal treatment can be highly effective for many months or even years in some people. How long someone stays on treatment depends on how well it works and, even more important, how they feel. If the first hormonal therapy does not work, another one has a chance of working. Regardless of this, I imagine this is very hard for you and for her. She is fortunate to have you.

Dr. Griggs, please talk about supplementation during hormone-therapy. It is so confuse. If the supplement do not interfere with the treatment, so, why to cut all of them? How can we get nutrients like, digestive enzymes, Omega 3, Vitamin D3, Vitamin C, all of them to help our immune system be strong. It is a dream to get them only with foods, and some of them our body does not produce itself? Impossible, isn't, it? Por favor fale sobre a proibiçao dos suplementos durante a hormono terapia.

We don't generally recommend supplements except for Vitamin D. People who come from extreme economic deprivation may indeed not get enough vitamins from their food. The trouble with supplements is that they are usually not tested, not manufactured according to standard practices, and are highly processed compared to naturally occurring foods. A side effect I had with Xgeva and Zometa was low calcium. I now have to take Calcitriol daily.

my wife has stage 4 breast cancer, liver and bone metastases. we live in Canada. I noticed that the doctor prescribes chemotherapy for many, and for some reason we were prescribed medications - Zolodex - Ribocidin - Ietrozol .. I don’t know anyone else who is treated with pills .. and because of this I am very worried. I would like to do chemotherapy, reduce the tumor, cut it out, and so that the metastases also decrease. why is that ? and a question. you know someone who is in remission from stage 4 cancer. ?

Thanks for writing. This sounds so hard. Your wife is fortunate to have you on her side. As hard as it is to believe, the treatment course you describe is the most active treatment combination we have and is the best treatment for people with metastatic breast cancer that is hormone receptor-positive. People can be on treatment such as this for a very long time before having to change to chemotherapy. Surgery may be a reasonable consideration down the road, but the most important thing now is to treat the cells that are throughout the whole body. I hope this is helpful.

I’m going through tests had mastectomy, I’m older have arthritis in lower back. Spent a long time in bone scan I’m scared but my back is a mess for 30 years

A new diagnosis can certainly be a scary time. Chronic back pain is unlikely to be related to cancer, but having your back looked at with the scan is a reasonable next step. Wishing you the best.

The first step is to obtain a piece of tissue through a biopsy before anything else. Lumps can be from many things. She's fortunate to have you helping her through an anxious time.

Hormonal therapy is generally started after chemotherapy is over. For people having radiation therapy, hormonal therapy can start along with radiation therapy. Some people prefer to wait until after radiation therapy.

Hi I had Stage II breast cancer surgery in mid 2020 followed with chemotherapy and radiation. 2 months after radiation Im diagnosed with Stage 4 cancer. Why is it from stage 2 to 4 just 2 months after treatment?

That must have been a shock. When you were first diagnosed, there was no evidence that the cancer had spread to other parts of your body. At some point, areas of cancer in other parts of the body that were originally too tiny to be seen became visible. While this is not common, it does happen. Remember that the cancer had already spread to other parts of the body when you were originally informed you had Stage II disease. I hope you are doing okay.

There are quite a few medications for people with advanced triple-negative cancer. Many of these are chosen based on the results of tumor markers other than ER, PR, and HER2.

Hi, my mother was diagnosed with Stage 4 MBC last month. She has metastasis to 3 bones (starnum, C7 vertebrae and iliac bone) along with axilliary lymph node with all other organs appearing normal in PET Scan. Her tumour is Er, Pr + and HER2-. She has started her treatment with Ribociclib + Letrozole and a monthly Denosumab 120 injection. She is 54, doesn’t have any pain in her bones and the tumour was Grade 2 invasive lobular carcinoma with in left breast. I am trying to understand if there is any hope for the cancer to go in remission for years and in those cases can surgery be an option to completely remove the tumour if it shrinks? Also, How long does she need to take the injection or can it be stopped after some months if there is no sign of cancer in the bones. We didn’t do the bone biopsy as multiple bones were involved and Dr said not required, but should we ask the dr. to do this nevertheless? Lastly, does she need to cut off sugar completely? She is not diabetic.

Your mom is lucky to have you advocating for her. There are a lot of options for treating bone pain from cancer. If she has breast cancer, being on a treatment specifically for bones, such as an intravenous bisphosphonate, is important. Two bisphosphonates are pamidronate and zolendronate. Sometimes bone pain can "flare" when starting on a new anticancer treatment. This should improve soon if this is her situation. Other things that help include non-steroidal antiinflammatory drugs (NSAIDS), but I would encourage her to talk with her medical team and let them know that she trusts them to help her find pain medicine that will help her deal with the pain. I can't make specific recommendations as you know. Wishing you and her all the best.

Great question. In about half of people with metastatic (advanced) cancer, we see an elevated tumor marker. This can help us monitor the status of the cancer if we see the marker come down with treatment. If the tumor marker goes down and then goes up again, we will usually use that as a prompt to get scans of the body to see if the scans show growth of the cancer. Tumor markers are not helpful if someone has early stage (stages I, II, or III) breast cancer or DCIS (non-invasive breast cancer).

In the case of someone with HER2-positive cancer that is also hormone receptor-negative, the main course of treatment will be therapy directed at HER2. There are currently 16 different options for anti-cancer therapy. We agree that more needs to be done. Remember that the goal of treatment is to help people feel good for as long as possible. If someone is getting treated and feels poorly, a change in treatment is an option. It's important to let the medical team know of any symptoms related to the treatment or the cancer itself. Thanks for sharing your story. These changes in your tumor marker are generally not the changes we consider meaningful. In general, we need to see a continuous rise in the tumor marker level month after month before we would pursue additional tests. Having the tumor marker done every 3 months might be something to bring up to your medical team if they would be willing to decrease how often it's done.

My mother had breast cancer in 2016. In 2022 it reoccur again but we came to know last month it has spread to bones, lung and liver and doctor started palliative chemotherapy one is done is there any suggestions from you mam

Your mother is fortunate to have you. Emotional support for both your mother and your family is crucial during this challenging time. Palliative chemotherapy can be very effective. If available, a palliative care consultation will help her existing medical team manage symptoms and feelings that come up when someone develops metastatic disease. Thanks for writing. I'm not sure how to help navigate you to another place for treatment. Chemotherapy, which is recommended for most people with TNBC, is available in India. I would image that traveling to a major city would be necessary for many people.

Hi mam,i am from india i am suffering from 3rd stage her 2 positive breast cancer.l hava lump,one limfnod positive and one spot in my coller bone.i have completed 3 chemo with fesgo target therepy.ihave done pet scan.neck spot was completely finished.bs exella m abhi hai.dr are saying result is better. Abhi 3 chemo should be remaining.then radiation.then surgery in excella.my treatment is going right or not

My mother has been diagnosed with Her2-, bone mets she has been given Radiation therapy to ease the pain and Ribocicilib and letrozole along with that. How long is she gonna be on these meds?

Thanks for writing. Your mother is on the right form of treatment from what you've written. In general, as long as the treatment is working and she's tolerating treatment well, we would continue this treatment. It is possible that she will need a break from the Ribociclib at some point due to fatigue or low white blood cell counts, and the other medication may well control the disease on its own in the case.

Thanks for the question. There are many treatment options for metastatic breast cancer--almost too many to list. Make sure that she has a good relationship with her medical team and feels comfortable asking about what all of the options are.

Thanks for watching and for your question. Acupuncture is, as you know, an ancient healing art. There have been many studies of acupuncture with mixed results. It is extremely challenging to do placebo-controlled trials with acupuncture. This is because even "pretend" acupuncture, people benefit from this treatment. So there's no real "control group." While some people are skeptical about the benefits of acupuncture, there are enough people who find it helpful that it is hard to dispute its effectiveness. The only downside is the cost, which is not covered for everyone. The other downside would be a situation in which someone declines therapy that has been shown to help and to choose another therapy to treat their cancer. As a supportive measure such as in your case, it's hard to see the down side.

Dear Doctor, Thank you for this very helpful video. I was diagnosed with grade 2 evasive ductal carcinoma in 2016. Last week my CT scan show cancer cell came back in my liver, bone and lung. I am broken devastated! I will start hormones drug next week. Anything I need to be aware please doctor?

Thank you for writing and for sharing your story. We can only imagine what you must be feeling. Hormonal (also called endocrine) therapy is a great option for many people. You may also want to ask your medical team if medication to strengthen your bones would be a good idea in your case. As hard as it is to believe, you will get through this. Please take the absolute best care of yourself, reaching out for support and making sure you keep good communication with your medical team.

It sounds like your mom is undergoing a comprehensive treatment plan, which is a good sign that her medical team is being thorough. Although surgery is usually not done for people with metastatic disease, surgery may be considered after completing chemotherapy if the doctors believe it will benefit her overall outcome.

Surgery ka name hai (left modified radical mastectomy + level 1-2-3 axillary lymph nodes disease + pacooo technically) .mam surgery successful hota hai to . Great question. This report means that the HER2 is negative. The treatment implication is that anti-HER2 therapy (such as with trastuzumab, Herceptin) will not be part of the treatment plan.

Thank u so much for this video, my mother has been diagnosed for stage 4 breast cancer ( bone mestasis in spine) her liver lungs kidneys pancreases and all other organs are fine and no sign of cancer, my question is since the cancer hasn’t spread to anywhere else besides the spine shouldn’t the surgeon remove her breast and then treat the cancer that’s in her spine before it starts to spread again? Your mother is fortunate to have you advocating for her. And such a good question. The impulse to remove the cancer is a strong one. Because the cancer has spread, however, treating the whole body is the most effective approach. It's more than likely that the breast tumor will shrink quite quickly and that the disease will response in the spine as well. Decisions about breast surgery could be made in the future. We do not do surgery on spine metastases except in rare cases because it is not possible to remove all the disease. Radiation therapy to the spine may be part of your mother's treatment plan if she develops pain in the spine because radiation therapy is extremely effective in killing breast cancer cells in bone.

If you are asking about ER-negative tumors, hormonal (endocrine) therapy doesn't work because there is no receptor on the tumor cells to "receive" the estrogen. Hormonal therapy works by either lowering estrogen levels (AIs) or blocking the estrogen receptor on the cells. If there is no receptor, neither approach is beneficial.

I have a friend who has breast cancer. She had a double mastectomy then they performed two scans. Showing a 2cm mass in her liver and smaller one in the lungs. They told her even before the scans because they didn’t get clear margins from her surgery she will have chemo, radiation and hormone therapy. Is it possible that she can be cured at this point?

Thanks for writing. Your friend is fortunate to have you. Without knowing more, it is not possible to know if the goal of treatment is cure. The general guidance is to attempt to biopsy anything that is being labeled as "metastatic disease." There may be other results that indicate this is not metastatic disease. The treatment plan sounds like one that we use when the goal of treatment is cure, so I would be optimistic for that endpoint.

This sounds really hard for both you and your mother. It can indeed be harder to manage chemotherapy and its side effects in someone with severe diabetes. It can be done, however, with close monitoring. We would recommend that you and your mother stay in close contact with your medical team. It is also possible that a pharmacist could be extremely helpful in this situation if you can ask to speak with a pharmacist. Wishing you and your mother the best...she is fortunate to have you.

Thanks for writing, and we're sorry to hear about your mother's suffering. She is fortunate to have you. Breast cancer in the spine causing a lot of pain is treated usually first with radiation therapy along with treatment that goes through the whole body (such as chemotherapy or endocrine therapy). Surgery is sometimes needed if someone has already had radiation therapy or if someone has problems with control of their bowel or bladder (a serious complication that happens on occasion).

HER2 is expressed in all normal cells. In breast and other cancers, it can be overexpressed. We have many treatment options for people living with HER2-positive breast cancer, and there are new treatments being developed, tested, and published all the time.
