Just as @Mattjohnson5192 says, partners make mistakes, misjudge the situation or are insensitive sometimes. So maybe, and you get to decide if a behaviour is a dealbreaker of course. That said, if you've agreed to forgive but find yourself obsessing about her past and can't seem to mentally let this go, then we could be in the realms of retroactive jealousy. Sometimes intrusive thoughts are all made up in our minds and sometimes they're based on things that actually happened, mistakes that were made. I hope this helps clarify - it's a good question!

That's my two cents about the issue as a person who struggled in the past but finally got the relief. Maybe the picture is as follows. You are the guy who was raised by books and movies romanticizing relationships with women and "love". Because of that you had some lack or deficiency of sexual experience, or you think so. You meet girl and soon realize, she is "that girl". "The special one". As consequence of this you overcommit in the relationship - maybe not even in money but in attitude. Some day you realize that other guys rode your girl on their hairy motorollers and she was happy about it. And, what's interesting, these guys DID NOT COMMIT. And you feel like a total loser who pays full price for something others get for free. Two things can help. First, realize she is not special. Take a look from bird's eye view. Some people on some territory meet, fall in love, sleep, get married, born kids, divorce and break up with each other. Each person pursues their own goals and your girl too. So, she is not "that girl". At best, she is good and suitable for you. Second, stop overcommiting. Don't give more just because of "thatness" of the girl. Think about yourself too. Well, not "too", think about youself in the first place. Maybe this advice will ruin your relationship if it was backed only by your overcommitment but so be it.

I’m a soldier at war and see my wife once in a couple month. My problem became so bad i can last like 1 second the first time we meet. Second round would be couple seconds more😢. Due to your circumstances, have you considered a delay spray to give you a bit of a confidence boost? It's not a long-term strategy and by all means work on your relaxation too, but I appreciate you might not be in a place to do some of the 'training' methods right now. Interesting content. The topic of PE is, I think, highly mysterious. It could simply be evolutionary wiring, where quick ejaculation had survival benefits, and some people are just more primed towards that. Or it could be that this primal pathway is activated whenever we are carrying a lot of unconscious stress, which basically pertains to the perception of threats, but in a more abstract sense, and so dealing with lowkey anxiety is important to it. It could also be due to physiological training, as in squeezing the PE during sexual experiences, we train ourselves to ejaculate as quickly as possible, and if we've been doing that since we first become sexually cognizant, then it has become a lifelong habit, which in turn could be reflected in other areas of life. The PC muscle can trigger the ejaculatory reflexive system, which in turn is the activation of the sympathetic nervous system, which pertains to our fight or flight response, hence its overarching relationship with anxiety and fear. I've heard of cases where a person ejaculated simply from a sexual massage from their partner (without sexual stimulation), or from viewing certain explicit content, without any sexual touching occurring. What would such cases indicate? We know that they are passing from a parasympathetic system activation and arousal state into a sympathetic nervous system ejaculatory state without sexual stimulation. Could there be the association of arousal with stress? And so when aroused, there is this attempt by the body to escape stress or this fearful situation?

I completely agree, and I share your curiousity. For a presentation that seems so straightforward on the surface, we're still trying to understand the many biopsychosocial factors that contribute to PE. In most guys presenting with PE, I tend to see a combination of excitement and anxiety. Closely related emotions and both drivers of sympathetic activation. And just as you suggest, stress is a factor too. Both latent stress and performance stress, which keeps men stuck in a cycle of disappointment and worry about the next time. And, again as you mention, our natural wiring. PE does have some genetic inheritance. Some men are wired to ejaculate sooner than others. We all have some degree of agency over this, but stress and excitement make it difficult to exercise this agency in the moment. We panic or get carried away, or both. For cases of ejaculation without touch, I suspect these are acute experiences of going from parasympathetic to sympathetic engagement due to purely psychological/perceived stimulation. Us humans seem to be the only species that can do that, for better or for worse!

@@LastingLongerLab Thanks for the response, insightful as always. There is one exercise I've been experimenting with and using to help notice my own physical response. For many years I've been interested in, and using, cold showers/ice baths to help train myself in relation to anxiety. There are a lot of good studies on that and I think that I can say that it's helped a lot, being able to stand in the cold shower while your body enters into a sympathetic nervous response state. But then, learning to be calm within it, almost as if perhaps transitioning back to a parasympathetic response. In regards to PE training, I notice that it is very hard to simply not constrict or squeeze the PC muscles when the cold shock hits the body. But I notice that the more I train to relax the pelvic floor through reverse kegels, the easier it becomes, although still not at a level where it's comfortable. Do you think this sort of training can be effective based on the theory? It might be worth investigating as a type of training for future use cases. So far I haven't seen anyone investigate this, but it certainly does highlight the relationship between sympathetic nervous system activation and pelvic floor activity.

Though I'm not aware of any contemporary research (except related to Wim Hof method, but that has quite a different objective) and I haven't investigated it up to now. The pelvic floor response you describe sounds completely plausible to me and I would consider it worthy of investigation. Thanks - for my part, I'll give this more consideration!

Well, it's quite normal to be able to last longer in round two - and thank goodness for that. You might be starting out a bit over-excited or aroused and there are lots of options for addressing that. See my videos about excitement for starters and I hope it helps!

Anxiety after sex, including panic attacks, can still come under the definition of postcoital dysphoria. But not necessarily - it can also be more related to the postcoital drop in hormone levels, and individual factors such as trauma and latent stress levels. And I do agree - sadness almost feels preferable, doesn't it? If this persists, breathing and relaxation techniques can help and speaking with a therapist with relevant experience can also be beneficial. Thanks for sharing!

Hello and good question! Prostatitis is probably the most common - an inflammation or infection of the prostate. But less common in younger guys. Lower body muscle tightness can contribute to PE too, though unlikely to cause it.

Interesting and useful video, doctor. I think we men should create a more relaxing atmosphere and spend more time speaking to our girlfriend, before starting to touch each other’s body. It’s also clear that the more excited we arrive at the meeting, the sooner we ejaculate. It also depends on personal feelings and reactions: even a kiss on the neck or a moan of pleasure by our girlfriend may take us near the coming. In my opinion, masturbating before having sex is not helpful. I speak considering my experience as a boy and young man. Masturbation, if carried out correctly, requires lots of energy and in case we have sex shortly after, it may happen that we feel not completely satisfied. It’s like the case when we masturbate twice in a short time after a period of abstinence: during the first one we feel stronger, more desire, more satisfied and we last shorter; the second time we may last longer because we have no urgency, but with a loss of energy and imagination.

Good question! Different guys will want to practice this method more than others, just as some guys masturbate more than others. And similarly, it's possible to overdo it too. As a general rule, when guys ask me how often they should 'train' I suggest every other day or three times a week. Some guys with lots of energy will go for every day. Multiple times a day is overdoing it IMO. As for duration, anything from 15 mins to 30 mins is sufficient.

I take your point. I'd say that like most obsessions or OCD-type thought, RJ isn't completely irrational. It's quite natural to some degree. But the doubt and obsession becomes turned up to the absolute max, making our partner's past feel like a major emergency. Being able to focus more on the now is key, absolutely.

Well I did this breathing technique at the gym doing squats and blasted through to ten reps every set. Also I tried this casually during the day, by simulating poping my ear drums without popping them. Instantly this felt like the best reverse kegal I ever felt. Normally I just used my abs to do the RK but this method recruits the diaphragm and the push is fuller, stronger and easier to hold. This alone is amazing just for the use in the gym. That's a really good question, so thank you for asking. I agree that these two forms of stimulation are opposite in a way - one targets the most sensitive area and the other purposely avoids it. When formulating a treatment plan for PE, we often introduce PRM as a starting point (along with a breathing technique and lower body relaxation). Many men with lifelong PE have very little experience of receiving penile stimulation while erect, so PRM brings useful experience and insights. Sometimes a true 'wow' moment. Then when we're able to stimulate in this way for 5-10 minutes and acclimatise to sensation, we can start to introduce more intense stimulation, with a device or manually. So working towards 10 minutes of PRM and then 3-5 minutes of glans or frenulum stimulation - reverting back to PRM when/if necessary but not stopping stimulation completely. And gradually, we can shift this ratio to 1-2 mins of PRM to acclimatise and 5-10 mins of intense stimulation. This takes time and patience to work towards, of course. Research suggests that PRM alone (typically for 15 mins) can be beneficial, but we've had best results with this mix so far. I hope this helps explain things and thank you again.

And the other method just stimulates the base. This second method is good to start with. It gets us used to staying hard and stimulated for longer. Then we can add in more of the intense stimulation as we become more confident and relaxed. Hope this helps!

So working towards 10 minutes of PRM and then 3-5 minutes of glans or frenulum stimulation - reverting back to PRM when/if necessary but not stopping stimulation completely. And gradually, we can shift this ratio to 1-2 mins of PRM to acclimatise and 5-10 mins of intense stimulation. This takes time and patience to work towards, of course. Research suggests that PRM alone (typically for 15 mins) can be beneficial, but we've had best results with this mix so far. I hope this helps explain things and thank you again.

Yes that's true for most guys, including guys with PE. But for a significant number of men with PE, it doesn't make any difference and then feels a bit hopeless. But you are absolutely correct!

The way porn helps to cause PE is like this. Before, when watching porn I would last long(as well as during normal sex) but because I masturbated so often to porn, waiting an hour to ejaculate wasn't cutting it. I then started putting myself in an overstimulated mindset by which i would cum in 5 mins. Now it seems I'm unable to reverse this as sometimes(if its with a new partner) I can cum in 2 strokes.

Is this like Box breathing? Count 4 seconds breathing outwards, hold for 4 seconds - then breath in normally, repeat?

Yes box breathing has a similar effect. Any technique with a longer exhale and not too much emphasis on breath holding can help keep the parasympathetic state sufficiently engaged - and that's what we want during sex!

I hate the idea of using a term as a blanket for men who care about their partners past. In this case, "jealousy". I think it's natural to be repulsed by a cuffing a woman with a "rich sexual history" because theres a tendency for it to be a sign of masculinity in a female (likely via birth control pills). Also, it ruins a woman's ability to pair bond and why would I wanna invest my time, effort, and energy building a life with someone who doesn't appreciate it because they're too busy crying about their ex/exes?
