hi i am a med student a question. when pressure or overload rises.(preload or afterload) heart does either concentric or eccentric hypertrophy and increases wall thickness decreasing wall tension. thus reducing wall stress but in text book knowledge it says hypertrophy increases o2 demand how so ? it is paradoxical.

Hypertrophy = Less wall tension/ Per unit area of muscle. If the question asks about the whole heart (net-oxygen demand), then it does increase the demand (More muscles, more o2). But it decreases the tension if it asks about O2 demand per unit area of muscle (or simply wall tension).

​​@@TinyMedicineSo when there is Left Ventricular Hypertrophy (LVH) the muscles are more thicker so it would be easier for the ventricle to contract against both the pressure inside the ventricle and the afterload it's facing But since it's more thicker (more muscles) mean it requires more oxygen demand. Hypertrophy = less wall tension But more oxygen demand. Is that right? Hoping you'll answer..

you cant give more then 500 mg of lidocaine to a patient in one visit so you calculated 700 mg to 35 ml is wrong

Easy short hand; Bupi w or w/o Epi the patients weight in Kgs is the same as mls if 0.25% and and if 0.5% the total mls is half their weight in Kgs. Yes, the Bupi with epi is slightly under-dosed but is easy bedside mental math, especially helpful for nerve blocks.
