what if my 1st set of aligners don't fit because of teeth movement because of tooth extraction, does invisalign charge extra for new scan or new set of trays ?

Today is my first day of Invisalign. My front two teeth hurt a lot whenever I try to take them out to eat my meals. Is this normal? I feel too much pain and pressure around that area.

Very normal (and expected) for teeth to be sore when eating for the first 3-5 days. That soreness means your teeth are moving! Good luck with your Invisalign journey.

Thanks for sending the video! It looks like this case should be corrected with braces. This is not Invisalign software btw. I would recommend getting a second opinion.

3:02 i think this case needed expansion to make the actual teeth go back and keep it tipped forward to keep the angulation in the philtrum and would also preserve lip definition

Good observation! It's much harder to expand in adult because there's no new bone made so teeth must be housed in the bone housing. This would have been best with extraction treatment as a teenager.

My daughter is 6yrs old. She has a high palate and tapered arch, 36mm intraarch distance between maxillary first molars. Her dentist recommended clear aligners phase 1 (spark) for her, but would this expand her palate? Or would a removable palate expander be better to start?

Great question! Clear aligners are not really designed for skeletal expansion. It is standard in the orthodontic profession to use a Maxillary Expander for what you are describing. I do have a new video about Invisalign Palate Expander coming out soon which is a newer alternative to the Traditional Expanders. I would probably stay away from clear aligners for skeletal expansion. You can still do clear aligners for teeth alignment, but not recommended skeletal expansion. Hope that helps!

I just got Invisalign for my over bite and I still couldn’t comprehend how they were ever going to fix it, but seeing this video and hearing from another video that each aligner moves about 1/4 a millimeter, makes sense.

Glad the video helped! In general, it's about 0.25mm or 1 degree rotation per tray. We don't want teeth moving too fast ... that's called extraction. Good luck with your Invisalign.

Hi Dr. My mom already had a partial denture for front 4 teeth in upper jaw for nearly 17 years. Now the teeth near that dentures tips are broken only root is left 5 teeth around those area. Now our doctor suggesting we have to remove those with teeth extraction surgery at one go and later after a month we can have a complete denture for upper jaw. Is this the right procedure??

Great question! That is usually a question for your general dentist. Extractions and Dentures are usually procedures with your normal dentist. If you are unsure about the treatment plan, then I suggest you get a 2nd or 3rd opinion before you remove teeth. Once you remove teeth, you can't put them back. Natural teeth are generally always better than fake teeth. You may see a decline in health with good teeth are removed. Be slow to make this decision. Wishing your mom all the best!

In 1998, I had what the ortho called the "First Phase." In 2008, I should have gotten the second phase, but unfortunately, my parent's finances at the time were going towards my dads cancer treatments. Fast forward to 2024, and my teeth are flared out. I have been teased so badly because of it. I hope i can fix it one day.

If your teeth were like any of the examples, then you can absolutely get Invisalign as an adult. Invisalign is great for reducing flaring. It sounds like orthodontic treatment could be life-changing for you. I hope you can get treatment soon! Wishing you all the best!

Thanks for watching! You must have a cutting-edge orthodontist. Custom braces are the future and only a small percentage of orthodontists are using them. Tricky scenario ... When you close gaps, you are effectively making the arch length smaller which constricts the jaw ---> the opposite of widening. It depends on how much rotation/crowding vs gaps between your teeth. If you have more rotations/crowding, then fixing a narrow smile is more realistic. If you have just gaps and no crowding, then you might not be a good candidate for widening.

@@crayoneater1717 Another good question. Invisalign is not really designed to procline (forward angulate) teeth in the absence of crowding. If you have spacing, then I would say no. If you have crowding, then I would say yes. If you have neither spacing or crowding, then your teeth angulation will likely stay the same. Hope that helps!

Great question! The amount of expansion/jaw widening will be determined on your size of the jaw / amount of bone you have to house your teeth. If you have bone available to move teeth into, then you can absolutely expand as an adult. The issue with adult expansion is that you won't grow new bone like a teenager would during growth spurts. Short answer is YES, if you have bone to move teeth into otherwise you will get gum recession. Hope that helps!

Great question! Orthodontic tooth soreness occurs the most at the beginning of treatment when teeth are being moved for the very first time. As you progress through the treatment, then the teeth are much less sore because the mouth is used to teeth moving. If you are experiencing a lot of pain with each tray, then definitely contact your orthodontist to make sure it's normal. Hope that helps!

Can spark also widen as much as Invisalign or is the plastic weaker? How would you think upper midline correction can be achieved with Spark? I got suggestion to have a beneslider that pulls back teeth but it freaks me out to retract teeth..I don't want collapsed face or breathing issues..

Great question ... There are now alternatives to Invsialign today vs 5 years ago when Invisalign was the main clear aligner on that market. Spark can absolutely do expansion just like Invisalign can. As long as you have space for your tongue, then beathing issues don't usually arise from orthodontic treatment.

Great question! If the arch is narrow, then expansion can also work to retract and re-angulate teeth. If the arch is already wide and no additional expansion can be done, then the options are to IPR or extract. Hope that helps!

It depends ... there's always some relapse associated with the removable retainers. Most of the time it's small / unnoticeable, but sometimes stubborn teeth may want to go backwards and can be out of alignment soon. The permanent retainer wire is 100% effective in keeping the front teeth straight.

Wisdom teeth have nearly no effect on the flaring of the front teeth. I've seen some wisdom teeth alter the equilibrium and cause crowding, but never seen it cause flaring.

Great question. As long as the IPR is done correctly, then there is no downsides. You shouldn't have sensitivity and you shouldn't be more prone to cavities. It's very safe (if done correctly).

Thanks for the feedback. Orthodontic treatment varies from patient to patient and results are definitely not the same for all people. I use Digital simulations to simulate where teeth are going before we start any treatment so we can see if there are going to be negative effects. Orthodontists should be looking at the face, lips, tongue, airway, bite, gum health, etc prior to doing treatment to make sure it benefits the person. In some cases, you can reverse some orthodontic movements so maybe you can get another consultation to see if you can make your situation better. Don't settle.

Good tip! Munchies are also more expensive. I've had patients use pen caps, wine corks, rolled up towels and erasers. Anything can work!

Problem with Chewies is that they change the shape of the aligner and force systems are deranged. Using harder “objects” is even worse 🤬

I’m almost done with my treatment I’m down to 12 hours a day instead of 22. These instructions are absolutely correct. WEAR YOUR TRAYS AND FOLLOW YOUR DRS INSTRUCTIONS ON WHEN TO SWITCH THEM OUT!!! The pul pick tool to remove Invisalign is a godsend. I got several from Amazon for under $10. I soak mine in the crystals every other day to keep them clean and remove any grime. The hardest part of Invisalign is switching trays and teeth soreness as your teeth are moving as you move between trays … otherwise your teeth will be straight and beautiful of u follow directions accordingly

Thank you so much for sharing! My son just turned 11. His teeth are a little crowded and.looks nice. But from X-ray, his airway is very narrow. His upper mouth roof is a little smaller then his lower one. My orthodontist recommended expander, 5-7mm, or even 10mm. But when I check with two other orthodontists, they thought my son only need very little expansion, or if I like, they can help his teeth with no expansion. They said that the airway theory of palate expander is not solid proved. Since my son's teeth looks good in general and he has good bite, they don't agree with our first orthodontist

Thanks for the message. Please do a PubMed search for Maxillary Expander and Airway. You will find plenty of studies showing a positive result of increased airway with nearly zero side effects. It's actually quite amazing what an expanded maxillary arch can do for breathing. Even if the breathing improves 10-25%, it might be the difference in resolving the symptoms you listed. I've had patients do expansion in the absence of a crossbite and still get the benefit of airway improvement. Give it a try.

Custom Braces can be done with both Clear Ceramic and Metal! For Metal, the customization is in the glue base while the customization is in the ceramic pad for the Clear.

There are many ways to reduce protrusion which do include distal jet, extraction, IPR, Space closure, Surgery, TADs, etc ... every person is different and the mechanics differ for each person. Finding a great orthodontist will ensure you get an amazing result!

Great question! Braces generally reduce bimaxillary protrusion better than Invisalign because braces can do greater movements more efficiently. The slide I put up of braces had much more retraction/uprighting than the Invisalign retraction/uprighting. Hope that helps!

I had IPR in my lower and it was a terrible it makes the teeth look too skinny and they are not naturally shaped anymore. Do not let these idiots decide the shape of your teeth by shaving away perfectly good PERMANENT enamel that you can’t get back and do not let them pull perfectly healthy teeth further destroying your arch size and therefore your face.

@@drpassamano just like up until a couple years ago you would have told me that mercury fillings were a very common and safe procedure that have been done millions of times. Except we’ve known that mercury is highly toxic to humans for well over a hundred years and that you were poisoning literally generations of people with this practice that had all the logic of a kindergartener. Not only is that committing a mass atrocity, but those toxic heavy metals get passed down congenitally from mother to fetus, so in order to save a dime you literally subjected the human race to endless suffering. And when you’re questioned about it it’s just a shrug of the shoulders and an oopsies. IPR is the exact same way. It has all the logic of a kindergartener and the fact that you all practice it doesn’t give it validity. It just means that you all are complicit in the idiocy.

Great question. TADs can be used on any case. The question is if it's needed. I only use TADs when auxillary forces are needed to accomplish the goal. These patients in this video did not need TADs. Thanks!!

After getting my braces taken off both bottom and front teeth are very flared out I recently went to an orthodontist and didn't give me any hope since there was no pre molars to take out since I had braces 13 years ago and they were taken out back then. knowing this now gives me some hope to fix my teeth some day 🥲Thank you for the video!

Hello u can remove ur wisdom teeth and create space and use mini screw or TAD to move ur teeth..use that method , u won't get satisfaction from any other methods.

Teeth are not like cars meaning that you can't put them in reverse. The wisdom teeth have no effect on anterior flaring. I have yet to see anyone successfully wisdom teeth space for flaring. This is why choosing your doctor is very important.

I've also done open bite closure with braces before. I use Invisalign more though because it can hold vertical, but with Custom Braces, then it is less of a difference. Damon braces are not custom braces. You'll likely be wearing the up/down rubber bands. Good luck!
