Oh wow.. that is a messy situation. That's some deep wounds of rejection the family has created in him. Such a gross invalidation and devaluing of him. And yet the gravity of knowing your sibling could die without your help.. So on the one hand I need to say, it's his organs and his body so no matter who it displeases he has final say over himself. There are risks and consequences to such a thing that he'd have to live with regardless the circumstances. On the other hand, as undeserving and cruel as this family has been to him, he'll have to make a choice that he can live with. When he looks in the mirror would he forever live with shame if his sister died, or would he look in the mirror and feel used if he donated- especially in the years to come if he has an expectation of more love & respect than he's gotten. Like Dr. Deloney likes to say, it's not your fault but this fell in your lap so now it's your responsibility to make the right choice (whatever you decide that is).

I definitely wouldn't suggest just taking a break. Either break up with her or spend some time with her. There are parents who do go on date nights and nake time for each other all the time. She's even helping out. Even if he's the parent now, she isn't. Shes a girlfriend who's been supportive even though he hasn't actually put into the relationship currently it sounds like. Honestly AH move or not he should probably cut her loose so she can find someone that will at least prioritize her some. I get the younger brother being top priority in some ways, but if she's not a priority staying with her is even more of an AH move. Honestly I'd hope that he'd stop being angry about reasonable feelings and expectations, but what's presented here doesn't give me much hope. What I fear with talking about everything she's done and the wishy washy talk is that he'll talk himself into believing and convincing her it will get better, but it'll always be some reason why the brother has to always take priority.

1. You don't let other people in a relationship that you formed with one person only. You created that relationship for you and that one person and when something's wrong, you two fix it together. And you ESPECIALLY DO NOT LET PEOPLE WHO ARE SURE TO GET SUBJECTIVE INTO THAT RELATIONSHIP. A romantic relationship is a two-person circle. Just you and your partner. 2. He is the only one his brother has in his life and his brother is too young to be left alone . His brother comes first, his brother needs him, and his brother was left to his care after his mom died. Try comparing that to what his girlfriend needs. I pointed out his wrongs, but you can't justify wanting attention when your man has someone in his life MORE IMPORTANT THAN A GIRL WHOM HE DOESN'T SHARE BLOOD WITH WHO ACTUALLY NEEDS ATTENTION AND CARE. I agree that the relationship is unhealthy and thus needs to get taken care of, but relationships require mutual understanding and his girlfriend failed to even understand the circumstances he was going through in the first place. He's stuck in the middle of two things that need tending to, and it's only natural to prioritize the one where he's the only one who's mature enough because the other person involved is a literally child. He prioritized his brother because his girlfriend should've been mature enough to understand, but she unfortunately was not and that's not his fault.

Depends where you are. We'd need to know where they actually are before knowing if there's any legal repercussions. Every country has different laws, in the US federally it is 18 and many states have Romeo and Juliet laws. but in some other countries it is 16 or even much lower 😬

I know that this is the morally acceptable way to do it and what should have happened but deep down I disagree with the idea of breaking up to sleep with other people or follow other romantic interests. "In sickness and in health" isn't followed by "except if you are horny" Marriage should mean something. I think people should rarely ever marry. Matrimonial bond shouldn't be a play thing for immature adults. This is why a wedding ring is important. And why I think both partners should give each other something like a wedding ring to each other, not just one to other.
