Oh wow.. that is a messy situation. That's some deep wounds of rejection the family has created in him. Such a gross invalidation and devaluing of him. And yet the gravity of knowing your sibling could die without your help.. So on the one hand I need to say, it's his organs and his body so no matter who it displeases he has final say over himself. There are risks and consequences to such a thing that he'd have to live with regardless the circumstances. On the other hand, as undeserving and cruel as this family has been to him, he'll have to make a choice that he can live with. When he looks in the mirror would he forever live with shame if his sister died, or would he look in the mirror and feel used if he donated- especially in the years to come if he has an expectation of more love & respect than he's gotten. Like Dr. Deloney likes to say, it's not your fault but this fell in your lap so now it's your responsibility to make the right choice (whatever you decide that is).
