I just developed this condition after my body cleared COVID. I have an appt with my GP next week and my ENT the following. I also have a headache associated with the swooshing in my ears. We don't routinely do T1 precontrast images of the spine, as the fat is what allows you to make out normal structures. Natural contrast, so to speak. Regardless, even with fat saturation epidural collections can be hard to see as they are often mimicked by artifact including motion, incomplete fat suppression, and CSF flow

Ah, sometimes if it's fast maybe I forget. There isn't much of a differential because this is classic for an adenoma. A broader differential includes meningioma, sarcoidosis, germinoma, lymphoma, and metastatic disease.

I only have 1 lesion on the Sri they think from migraine. but I almost have all the symptoms of systemic MS. what could it be?

A single lesion is not enough to make a diagnosis of multiple sclerosis. If you have other features, such as positive findings on a lumbar puncture, that would have to be weighed against the imaging findings to see what they think helps make the diagnosis. You'll have to talk to your doctors about it.
