Sorry if this is slightly off topic -- I had a pulsed field ablation, and my EP had recommended (not required) a cardiac MRI. I declined because of the gadolinium exposure. Can an effective MRI be done without gadolinium? Thanks for what you do, I love this channel.

I’m an EP cardiologist too: What I don’t quite understand is the reason why your electrophysiologist is requesting a cardiac MRI after an ablation with PFA. In any case, a good MRI should include gadolinium contrast, as it not only provides structural information about the heart but also adds functional data. Moreover, opposed to undergoing an MRI with gadolinium, I would suggest having an echocardiogram instead. If an MRI is truly necessary (and it’s not just a matter of them wanting to bill you more), proceed with it in its complete form. Regards

Nice to see this. Other physicians at other hospitals bounced me around on an afib treatment plan. My wife and I met Dr. Haines in 2013 and he did my ablation in 2014. We stayed in touch. He was amazing and we miss him.

There is a basic defect in study design, i would like to ask how mow much was the af burden after af ablation, and there should be a one more arm which should be not on anticoagulation and didn't recieved watchman device. As the risk of stroke reduction may have been because of af ablation and not bcoz of your watchman device than in that situation your watchman device would not have been of any use.

I just had a PFA on 08/29/2024 in Miami, FL. What an amazing, painless, & quick procedure. If for any reason I have to have re-done, hands-down, I will!!! I was able to discontinue the Amiodarone immediately following the procedure (mainly due it messing up my thyroid) without any adverse reactions. I am so pleased so far with the outcome of my PFA and would highly recommend it if your Electrophysiologist suggests it.

I think decreased time under sedation, less exposure to radiation are key benefits. If safety and efficacy are equivalent then the only concerns would be unexpected longer term negatives which are unknown unknowns. Cost is a fair point but what proportion of the total cost is just equipment?

This is such an important topic and I'm thrilled to see HRS tackle it. This is especially important for people with implanted heart devices. The recommendations from the 1990s are to "avoid contact sports". This recommendation was created in an era of Fidelis and Riata lead failures, but devices and leads are much more robust today. Combined with better implant techniques and products designed specifically for protecting implanted devices today's athletes are much more protected than years past. I love the physician's shift in attitude from protecting the patient to returning the athlete to play. This is absolutely the right course to take. Thank you HRS for researching this important topic in detail.
