Short answers: Because soldiers are idiots... Through the history soldiers were used like toys, while the "rich" stayed safe far away. Well except a few kings who marched with their army, respect for them. But even nowaydays a lot of stupid folks following the orders of megalomaniac assholes, and willing to die if ordered to. In regards to your conclusion, have you ever looked into the 9 Years War? A key aspect of that particular war was the fact that the Irish Covenanters relied largely on modern European style pike and shot formations over the more traditional Irish troops that the English were used to fighting: this accounted for a number of their successes in the war, which they actually came close to winning at points.

My father would break down in tears as he talked about his fellow Marines on Iwo Jima who were ordered over the birm into withering Japanese machine gun fire and certain death but they went anyway. They knew they were going to die as soldiers did on the beaches of Normandy, the trenches of Verdun, the fields of Gettysburg and so many other battlefields throughout history.

"Soldiers' lives are sometimes worth less than time." Actually, in this period almost always, because the soldiers are probably dying in large numbers of disease. Taking Badajoz by siege would probably cost a lot more than 3,000 soldiers. Wellington hated sieges and I'm guessing this is a major reason why.

Wagenburg by Czech Hussites was essentially the same. Ukrainian Cossack relied on the same tactics. I believe the root was that the army was more alike peasant militia. The troops were light infrantry with mittle armour. Hence a traditional pike ranks were not possible

But what if the first person climbed and then died? Would the next person get the award or no one? Because if no one would get it, there isn't really a huge reason for everyone to rush to the ladders. Not only that, if one person already reached up through the ladder, wouldn't others be more demotivated to go up. Although in the 2nd case you can make an argument that they went up because there were already soldiers above to back them up. I am seeing this from a more modern viewpoint and I am clearly quite uneducated on this topic, so I would love to know the answer.
