Most charting systems have smart/dot phrases (may also be called macros). You can simply copy the smart phrase from the Comprehensive List of Smart Phrases and paste into your EHR. Over charting can lead to redundant or inaccurate information. Also, if you mention something in the HPI but do not give a proper workup throughout the rest of the chart note, you will be held liable. I have another video coming out tomorrow that further explains over charting!

I do have a "normal exam" on a template within my charting system. So I use the "normal exam" and change for anything abnormal. I am not sure if your charting system allows this, but might be helpful to you!

@@DekkuRen because it’s free, for a simple soap note which is what freed does you can even use ChatGPT with the transcript anonymized. Also nabla too, but nabla removed their free plan, the only free now is Heidi, I think they are trying to do the same Nabla strategy, the only worth for me to pay is deepcura, but that’s only if you need specialty specific notes

Hello! I am not a psych provider, but I do see comprehensive patients that address multiple issues. Sometimes the longer SOAP notes get a little out of order and I do have to edit. I do make sure that I am clear with transitions. For example, I will say, "now lets talk about your diabetes." Freed AI also has a button to push that will concise the chart note, so it does make it more clear. I have also seen a couple AI companies that are specific to psych practices, so you may check those out as well.

Freed has some helpful scripts to use when notifying patients. You can find this by clicking on the "Get started" and then there is a link "How do I tell my patients about Freed."

Agreed, this is my favorite part of Freed AI! Before I would chart as the patient was talking, it was distracting for both of us! Now I love being able to focus on what the patient is saying while Freed AI writes my notes for me!
