Everyone is critiquing the crew but you have to remember they're reservists. Not saying what they did wasn't inexcusable.. I'm just saying give an active duty crew the same situation (or the vast majority of reserve/guard crews) the same situation and they'd be 100 percent fine. Most of these guys flying isn't their job, it's a hobby. There's a reason why the FAA has rules on proficiency.

2:03 If these are the real telemetric data links, then it shows that the crew briefly throttles the third engine, the engine started to make power, then they cut it to idle right away. Had they kept the 3rd throttle maxed out, they would've saved it.

The PF had #2 out but inadvertently grabbed it's throttle rather than the good #3 so they were trying to land that pig on two engines. You'd think SOMEONE would have noticed that. It's why you IDENTIFY and VERIFY the bad lump.

3 engine day became the dreaded 2 engine day we practiced in the sims . Not a lot of thrust to keep a heavy weight day like that airborne , then they go to 100% FLAPS . NOOOOOOOOO. THEN RETRACT THEM TO 40% , Noooooo !! Potential Smokin hole on that page of the -1 ( dash one )

And that’s why once you shut an engine down, you bring the throttle back in line with the rest of them so this confusion doesn’t occur. The engineer and pilots should have looked at N1 indication and immediately seen that they were only using power in two engines. Tragic.

Bradbury Robinson to prevent exactly what happened here. Pulling the throttles back and then accidentally throttling up the dead engine and crashing due to lack of thrust and climb capability.

In the aftermath of this incident if your focus is on who should get fired you are focusing on the wrong thing. You have a lot to prove before you sit in the cockpit of an airliner so incompetence is not a factor [ what causes a crew to act at a level below their normal performance level, maybe fatigue, and can you recognize when that is happening to you. If you want to develop as an aviation professional or enthusiast focus on Prevention and Situation analysis.

United made the best of a bad situation by Fessing up as to their navigation error. They did not try to protect themselves by minimizing their confusion and reported their known position. US air 2998 acted decisively in protecting their aircraft. Very important to have a Airport diagram handy and visible immediately after landing [ especially in low visibility ] . You really cannot make any assumptions about the airport taxiway signs and the visibility of markings and placards on the airport under those conditions without an investigation. One pilot can make a difference. United Airlines has a good population of female pilots an I am disturbed by the KITCHEN comments that someone made.

Never heard one acknowledgement of being too low. Not one acknowledgement of being below glide slope. Not one acknowledgement of losing airspeed and being too slow. I did hear the proper “oh shit” as the plane contacted the ground.

Typical for a task-saturated crew. That’s why you brief at altitude and not on final approach.

Also. The ILS was not tuned at they had originally planned an approach to another runway but broke off visually to land on this runway. It’s entirely possible that there were glide slope indications that were low and non-representative of the chosen runway.

USAir 2998, proactively well done captain and crew. im curious if there was any punishment or verbal reprimand for the ATC. i know these controllers have intellectually high octane jobs, but in this case the controller was about to allow a take off by USAir 2998, immediately after united declared confusion for their intended turn and present whereabouts

i didn't see the crash itself but this happened very near me as i live south of dover. what caused this C5 to crash, it was pilot error mostly, watch the throttles number 2 engine is out and he cuts power to number 3. so the plane is flying on 2 engines and they do the checklist for a 3 engine landing. so he slowed down too much to land safely at the base, the good thing about this crash was the fact that it was good that he was coming in from the south east where no houses were, if he would have came from the northwest, there would have been people killed and a bigger investigation would have taken place.
