Never heard one acknowledgement of being too low. Not one acknowledgement of being below glide slope. Not one acknowledgement of losing airspeed and being too slow. I did hear the proper “oh shit” as the plane contacted the ground.

that oh shit was actually my grandfather and he survived this along with everybody else he said after they crashed he thought he was in heaven when he saw the emergency services running recorded the plane

Typical for a task-saturated crew. That’s why you brief at altitude and not on final approach.

Also. The ILS was not tuned at they had originally planned an approach to another runway but broke off visually to land on this runway. It’s entirely possible that there were glide slope indications that were low and non-representative of the chosen runway.

Absolutely. Configuration and considerations should have been briefed at altitude not up for discussion on final approach. It could have been the distraction that caused them to miss the throttle swap to engine indications.

I think the majority of the talking is actually from the flight engineers and not the pilots. Because you hear a voice that says airspeed which you basically don't hear the entire recording. The FE on the C5 doesn't have an ASI I believe.

2:03 If these are the real telemetric data links, then it shows that the crew briefly throttles the third engine, the engine started to make power, then they cut it to idle right away. Had they kept the 3rd throttle maxed out, they would've saved it.

The PF had #2 out but inadvertently grabbed it's throttle rather than the good #3 so they were trying to land that pig on two engines. You'd think SOMEONE would have noticed that. It's why you IDENTIFY and VERIFY the bad lump.

As we can see, lack of crew resource management and situational awareness caught up with them. They were heavy taking off on a trip to Germany, there were several IP's into cockpit. I can imagine the left seater is flying Cessnas now if anything.

Not sure why no one looked at engine output N1 when they were not getting power..."that's all we got", no it sure isn't when your 1 and 4 N1 are the only ones indicating useful power. Isn't this really basic stuff? I mean I get the fact that they were on final and focused on the approach but it was evident 60 seconds beforehand that they were very underpowered, Which would have led me to immediately look at engine output.

@@googaagoogaa12345678 I mean, you're not really going to get a response on using them all as a unit when the #2 engine is dead. I guess it's more or less procedure to manipulate the working engines.

@@googaagoogaa12345678 they had an indication that thrust reverser doors were not locked properly and you really don't want a reversed engine at full speed when you are in the air.

"On 3 April 2006, C-5B 84-0059 crashed following a cockpit indication that a thrust reverser was not locked." "The Air Force's accident investigation board report concluded the cause to be human error, most notably the crew had been manipulating the throttle of the dead engine as if it was still running while keeping live engine at idle. The situation was further worsened by the crew's decision to use a high flap setting that increased drag beyond normal two engine capabilities." All survived.

And that’s why once you shut an engine down, you bring the throttle back in line with the rest of them so this confusion doesn’t occur. The engineer and pilots should have looked at N1 indication and immediately seen that they were only using power in two engines. Tragic.

Bradbury Robinson to prevent exactly what happened here. Pulling the throttles back and then accidentally throttling up the dead engine and crashing due to lack of thrust and climb capability.

@dman72 #2 is out, but they pulled the throttle on #3, which meant they were only getting thrust from 2 engines. What you are hearing are two loops. The pilots can't here the engineers chatting. Losing one engine in a C-5 isn't that big of a deal (I've been there before), but losing 2 at high weight is a bad day at the office.

Lack of a "sterile cockpit" was the direct cause - the so-called throttle-swap was an indirect result of this - resulting in mass-confusion on the part of the PIC. He was trying to listen to "too many cooks spoiling the broth" & lost focus on what was going on. I quite imagine that there have been some massive & serious changes in AF cockpit (CRM) protocol as a result of this outcome evidence. Wonder what the crew is doing nowadays - driving busses for NYC or Chicago Transit, I would imagine...

Everyone is critiquing the crew but you have to remember they're reservists. Not saying what they did wasn't inexcusable.. I'm just saying give an active duty crew the same situation (or the vast majority of reserve/guard crews) the same situation and they'd be 100 percent fine. Most of these guys flying isn't their job, it's a hobby. There's a reason why the FAA has rules on proficiency.

What a collection of errors! At 5NM out it is already bellow glide scope but it did not even maintain altitude to intercede it closer to the airport. It kept losing altitude. Then at 0:43 #1, 3 and 4 engines are brought to idle and then, immediately, #1, 2 (which looks inoperative) and 4 are powered up again. The culprit of all. Now the C5 is flying with two engines instead three. Hard to recover a heavy bird that is going down at more than 500ft/m, with full flaps and two engines at maximum.

Yes - and not maintaining a sterile cockpit GREATLY added to the confusion - to the point where the pilot shut down the wrong engine and actually got lost in the process!! As a CFII/Single/Multi/II/A&P Mech of well over 35 years, I should know a thing or two. Maintaining a sterile cockpit is almost mandatory in virtually every civilian and military transport-category aircraft that I'm aware of - I flew P-3(B) Orion ASW planes in the US Navy for 2-1/2 years - NATOPS is quite adamant about this!!

+paraord1410 The two pilots and flight engineer were in the middle of running a checklist when the throttles were swapped. The only conversation which could even be considered somewhat non-pertinent (the speed chart and flaps setting) didn't involve the two pilots, and all of the live mics were merged together on this copy of the voice recording so it only _sounds_ like they're all talking over each other even though they weren't. That's why one of the engineers didn't know where they were. Besides, the conversation happened *after* the mistake was made. The PIC was silent through most of the proceedings, and since the co-pilot was busy flying the plane, I think the PIC is ultimately responsible for failing to notice the throttle swap. His "bring it out of stick shaker" comment as they were passing above electrical lines a *few feet* above the ground was almost laughable and demonstrates his poor situational awareness skills in general.

nobody died in this accident. This was never supposed to get out but was leaked. Their was an engine switch when he brought all to idle. Also their were only supposed to be 4o% flaps not 100

This crew shouldn't even be in charge of a paper airplane, let alone a huge heavy jet. Sounds like complete dysfunction and amateur hour in the cockpit.

How do you loose so much situational awareness to forget which motor you had to shut down earlier? You have to pull that dead motor throttle over a detent to shut it down. You would have to pull out on the stick and back over the detent to make that mistake... Not surprised however, I was a crew-chief and have witnessed pilots reading magazines and taking naps in the flight deck.

3 engine day became the dreaded 2 engine day we practiced in the sims . Not a lot of thrust to keep a heavy weight day like that airborne , then they go to 100% FLAPS . NOOOOOOOOO. THEN RETRACT THEM TO 40% , Noooooo !! Potential Smokin hole on that page of the -1 ( dash one )

i didn't see the crash itself but this happened very near me as i live south of dover. what caused this C5 to crash, it was pilot error mostly, watch the throttles number 2 engine is out and he cuts power to number 3. so the plane is flying on 2 engines and they do the checklist for a 3 engine landing. so he slowed down too much to land safely at the base, the good thing about this crash was the fact that it was good that he was coming in from the south east where no houses were, if he would have came from the northwest, there would have been people killed and a bigger investigation would have taken place.

They extended the broken engine instead of the working one, so they are running only 2 engines and extended the flaps which increased the drag.
