Dear doctor Strickland. In sept 2023 an idiot hit me in traffic and almost killed me. Result: acl, lcl and a fragment in my fibula came off. First surgery in oct 2023: they put the fragment back + lcl reconstruction. Then i had to train for4 months and a half and then acl in march 2024 because during my pt i had a lot of instability, pain when i walked and also experiencied this weird rubber band feeling when i walked. So acl reconstructed in march. Now i am 4 months into pt and my muscles become stronger and stuff. However... i still have this elastic feeling that starts from my lcl and going to my tibia it seems. And its really annoying and instable when i walk. Have you ever came across such a casus? Here in Belgium we have a very good health care system and doctors but still. It is interesting to ask other surgeons their opinion. Greetings!

Dr.Strickland - Do you use the hamstring or the qaud tendon as the graft to replace the mpfl? Seems like most use the hamstring, but in my research, the quad tendon is preferred for high level athletes.

You should be able to play in 4-6 months with the right rehab; consult your surgeon for specific guidance on your situation.

After 8 dislocations of my right patella from hockey I finally had MPFL surgery 2.5 weeks ago. At age 53 I’m sure it would have been wise to have done this 15-20 years ago after the 4th time. Recovery is slow but I’m through most of the pain now and working on the ROM and building the muscles back up. Bending at 45-60° now with a bit of pain at 60° and quite tight. Hopefully will be driving in a couple of weeks. Hopefully be able to play golf in a couple of months. We will see if I play hockey again.

Just over 4 weeks post opp myself and the pain has definitely started to drop. Some days better than others but overall it’s bearable. I’m back to doing mostly everything I was doing before the injury besides being able to jump or sprint. Part of the reason of that is also bc I’m told not to do it yet but i currently am able to do a slow jog and do some yard work here and there. I had some fractures and the ligament literally pulled a piece of my knee apart when i dislocated it so I’m sure you’ll recover sooner than later. Goodluck!

I have a question.im 17.i have dislocated my left patella 2 times and may have subluxated it once in between thez two times..last time i dislocated my patella was 2 yrs back..recently i went for a checkup as i was experiencing pain in the knee when i played football..the doctor suggested an mpfl reconstruction but my father is hesitant and he says i should carry on without it ..i fell very imbalanced and feel pain usually AFTER playing soccer . I also have knock knees ..how much would the surgery cost in pakistan and how much time would it take to fully play football after surgery ..also will knock knees play any role in complexifying the surgery?

Yes knock knees might increase risk of failure but it is about 100 percent chance that this will happen again. I have no idea what cost would be in Pakistan and in the US it is typically covered by insurance.

Using the information you provided, without surgery, you will continue to have these problems. For this reason, it is worth pursuing with an orthopedic surgeon, even though knock knees increase the risk of failure.

I’m having this in 12 days.. strange, I don’t have instability or arthritis.. I have a lateralized tt with a tt-tg of 20.5mm.. diagnosed as PFPS. Pain didn’t pop up until about 2 years ago from overloading in sports.. I do worry that this will make my knee worse.. however I trust my surgeon as he is the best in my city. In fact he actually worked for the military and preformed thousands of these. Pray for me?

I play hockey and I had dislocated my knee multiple times. Doctors said mpfl. I played on it popping in and out for a few months until I couldn’t stand the pain any longer. I have surgery in a few days and I’m terrified. I’m not sure what to expect or what’s going to happen. But I found this video and it’s comforting that I’m not alone and how Liam and I have a very similar experience. But my question is, after the surgery, at what point am I able to walk around freely or bend my knee to an athletic position? Is it possible to practice things like shooting and stick-handling off ice after a few weeks? Thanks

I doubt youll see this tonight Im 38 had this issue since 8th grade its calmed down since Ive been in my 30s due to not playing sports. The new dr i have when i went to ask for a knee replacement due to the amount of arthrise pain i get at night that keeps me up at night they suggested this. Im mot sure this is the correct procedure for the up all-night type of pain i have due to arthritis and maybe the way my knees dont move in the groves because im very knock knee. Im just nervous and hoping im doing the right thing because both knees will need to be done.

Had surgery 4 months ago for meniscus and acl (posterior lateral root). Also had an MCL tear that didn’t need surgery. 2 months later had another MRI and the acl and everything was fine, mild scar tissue forming, meniscus was still torn with partial radial tear. Twisted 2 days ago, acl graft has a tear and meniscus is completely torn with a flap hanging, suspected cyclops lesion, arthofibrosis and a few more things including my MCL tear is still grade 2. Any advise? Will I need surgery ?

Hi Kelsey, You will definitely need surgery if you want to go back to cutting sports, and certainly you need a stable knee if there is a chance of repairing the meniscus. I doubt that you have arthrifibrosis if you had good range of motion before this recent fall. Finally, a torn ACL looks like a cyclops - a cyclops just means that there is soft tissue in the front of the knee joint (in the notch). It sounds like you need to rehab to get your full range of motion, and then the revision may be a bit more complicated, depending on the graft you choose and whether or not you have a lateral extra-articular tenodesis.

I'm 39 and just had a knee scope to remove several large bodies from my knee (a half dozen of 1cm diameter). This is my third surgery, second scope (originally surgery at 16 yrs old). Doc after my recent surgery/scope said it was worse than he thought and I have Stage 3 and 4 wear of my cartilage on the underside of my knee cap and on the groove where my knee cap rests. I don't have instability, but they believe it is not resting correctly. They said a possible option is a "Tibial tubercle osteotomy" to get better knee cap alignment with the groove. I'm told I'm not a candidate for cartilage transplant given I have a 'kissing' wear problem. Anyway, my question is this "Tibial tubercle osteotomy" the logical next fix? If so, should I do it sooner than later so I don't wear away what cartilage I have left?

Hi Matthew. This is a complicated question to answer without actually seeing your MRI. Bipolar or kissing lesions can be treated with an cartilage transplant on both the patella as well as the front of the femur. This is virtually always done with a tibial tubercle osteotomy unless your alignment is optimal. At 39, we certainly try to do everything possible to avoid a partial knee replacement. I don't know if you live anywhere near New York, but I'd be happy to see you. I also do telehealth for presurgical visits or you could see a patella specialist if there is one in your area.

Hi Doc. thank you for the videos - I have been told (following MRIs) I have severe patella alta (insall salvati ratio 1.8) with some lateral tilt, Q angle is increased bilaterally, lateral subluxation of the patella three fourths laterally & medially, dysplastic trochlea, TT-TG distance of 26mm, severe chondral damage to the patella & to the dysplastic trochlea, Clarke's test is positive, tibial tuberosity placed laterally.... I have been advised to not take part in any physical activity that puts force through the knee & have been referred to be seen by a specialist surgeon, but the wait time is long as I am going through the NHS (UK) - I know it is hard to say without images, but I'm guessing this will result in surgery- so my question is, what sort of surgery would typically be performed to fix the above?

In the United States we have different option for cartilage repair than the UK. It sounds like you would need a tibial tubercle osteotomy with a cartilage transplant for your patella and trochlea. You should be able to be active once this is fixed. Best of luck finding treatment.

I used to perform extreme and intest jumping with my left leg, and I developed knee instability, patella alta because my patello-tibial tendon streatched, and also chondromalacia. I can't run anymore, much less jump, and I can only go for short walks every like 3 days. I am limited to working out my upper body and calves without bending the knee at the gym, and I also have a partial schapolunate tear in my hand and probably have to get surgery for that.

Hello Alexander. Pain and swelling with retear is variable and dependent on mechanism of injury. You should get an MRI if there is any swelling.

Every surgeon rehabs their osteotomies differently. I would use a lot of cold therapy and make sure to fully straighten your knee and bend to 90 when sitting. Also, quad sets help keep your quad from atrophying too much.

I’m two weeks post op. I’ve had dislocations since age 11, I’ve dislocated my left patella 4 times and right patella 1 time. I’m now 17 the reason I didn’t get surgery earlier is because we always went to a clinic and they never mentioned surgery. But this year Feb while playing at my netball club I had to go to the hospital because it was the closest and the doctor told me if I don’t get this surgery I have a risk of loosing my leg because of how many times it has dislocated.

That sounds like a lot to go through. While you wouldn't lose your leg, you are at high risk of arthritis if you continue to suffer from patellar dislocations. I hope you are doing well now.

I am 15 and I have the exact same problem from when I dislocated my kneecap as my MPFL is torn and I have the patella Alta. I am getting surgery for my MPFL but I can’t get surgery for my patella Alta until I am fully grown which would be 3-4 years.

Hello Parker. That isn't completely true. We check bone age with hand X-rays and then determine when surgery is possible.

Usually one can skate in four months - if you're strong. Otherwise it may take more time.

@@jochyrod as someone got MPFL almost 5 years ago, keep working on your workout to build the legs muscles, strength training, jumping, box jumping, hopping etc at the gym and keep doing rehab or recovery. Normally is 1 year, but full recovery is 18-24 months.

Doc saved my knee with an MPFL. When other surgeons told me I’d never ski again, never exercise again. 8 years post-op I’m a ski patroller and skiing at a higher level than I did on the race team in college. Thank you Doctor Strickland!

I had mpfl and tto surgery in November 2019. When I kneel down the more pain I feel since after surgery in the screw areas. So i plan to remove the screw of tto of two screw and two small screw on top to knee for mpfl. Is there any problem to me after removing the screw.

On 2-12 I will have anteromediale tuberositas tibiae transposition. And on 6 oktober my left knee will be done. My shoulder orthopedic surgeon had her studie in Boston.

How about if the patella tendon pull a peice of the tibia out. What is that injury called?

That is a patellar tendon avulsion. It pretty much always needs surgery.

I wish this wasn't true, but these osteotomy surgeries ruin people's health and that of all the other people who have them done. The bone where they sawed afterwards doesn't stick properly anymore, it gets weak and it will slide little by little and sink inwards with time. I believe that a bone scar is left or for some other reason I don't know. Maybe it is left with little room to regenerate and becomes weak or another reason. It is not the same as a normal fracture where afterwards the bone becomes normal again. When you sawed it, or break it with a chisel, in this case it is different. The bone is left with a problem afterwards. Then the person gets all twisted because of this, as the bone sinks, it ends up twisting or sinks all the other bones of the body. If they do it in the lower limbs. But everywhere, it ends up disturbing people. Besides that, the nerves around where they do this also sink and leave their original place, and then the person starts to feel everything altered and only starts to feel bad things afterwards. Both in the external environment and in the body itself because of this. This surgery is a mistake in medicine, just as there are mistakes in any other profession. I don't know how it hasn't been forbidden until today, with so many people getting bad afterwards, I'm pretty sure all of them. It doesn't matter if it is done on the nose, leg, foot, heel, people end up getting bad the same way. I hope this comment helps. Take great care, health is very precious. Hi dra, how are you? They performed an osteotomy on my heel and this happened. Have to be very careful. With some people I met, it didn't work out too well either. But I don't know if this happens to everyone. A hug.

Hey Doc Actually i hit my Tibial turbercle with another boy in volleyball almost 10 years Ago .. when i was 15 . Now i am 25 .. i still can feel the Pain .its so frustrating .. i can't Excercise ... i just walk alot these days and when i don't .. i Feel my knee is Jammed.. a Doctor told me the surgery would be 6 to 10 hours long and post surgery complications could be worse.. guide what should i do ?? Should i go for the surgery ??
