2:10 I can only guess you got hit by two explosions and possibly a third damage source on the same frame, and since ultrakill's just like that, it couldn't handle so much knockback or something, either that or a physics prop(a virture piece) got double whammy kablammo'd and ultrakill doesn't like that. It's also possible that 4-2 is just a terrarium for all sorts of bugs, especially moths, since they tend to crash into things so often.

The in-game-timer stops when you pause the game, so the reset doesn't actually affect the time that much.

Ive seen TAS speedruns less insane than this. How in gods name are humans capable of anything near these things? I stopped understanding what you were doing when the checkpoint button was involved.
