yea, it happened twice, but this only time i reminded myself to capture it. both times were the follow up attack of axe variant yharnamite. i tried to recreate it but it seems random and i can't even think of what could be causing this

It is probably a well known tune through out Yharnam, it works on Gascoigne because it simply reminds him of his young daughters/past. Well that is the theory that i liked the most atleast :D

The only link I found between Gascoigne and the lullaby is when you beat Mergo's Wetnurse the corpse of his wife dissapear.

I always thought her Blood Vial is there for gameplay reasons honestly, Or like a "Hey! Remember this place?" Type of thing. But! It could be just a reminder for you to revisit the Clinic in case you didn't already, Otherwise, you'll miss out on her plot and Cainhurst Castle. Although, i like your theory way more than mine since it's a strictly lore theory :D

My god, finally another person who pointed this out, nobody EVER questions why her blood vial is basically at the apex of mergos loft. I know damn well Miyazaki is a stickler for details and to have that blood vial was intentional, had they wanted to give you health they could have simply gave a good amount of blood vials. The only thing is, my theory is kinda weird and less dramatic. I think Iosefka's blood vial is there because maybe its a vial of blood very reminiscent of her's to the point that we just assume its her, but actually maybe the vial wasn't hers at all and she had somehow found(stole) this blood that was potent and refined that would heal and invigorate you...what the blood was originally meant to do, meaning the very culmination of all blood research. Most of if not all of the blood that was administered from the Healing Church backfired except whatever they achieved with the Choir, now think about what happens when the imposter arrives. Whoever she was, I don't think she was the one who had the blood to do her experiments so maybe Iosefka had stole from the healing church and this other person knew about it as well and wanted part and Iosefka denied her, causing the imposter to eventually turn on her. I believe these two were part of the Church but left for some reason or knew what they was up to because why else would one small clinic house some of the most powerful healing blood in Yharnam?

Nah, Yosefka was with the choir, Mergo was captured from the Labyrinth by Mensis. They're were opposing factions that got to the point of being violent towards each other.

Iosefka wasn’t with the choir since the Imposter Iosefka, who kills real Iosefka, was confirmed a choir member. My personal head canon is that Iosefka sent it there when she was turned into a celestial emissary by Imposter Iosefka (the one in the clinic that drops iosefka’s blood vial). Why? Eh, who knows. Accident, forced by Imposter, doesn’t matter. That’s the only reasonable lore explanation in my head, at least.
