I just got out of a 5 day hospital stay with both arms full of bruises from multiple blown veins. The only vein that didn't blow was the one they did with a sonogram, which was also the one they refuse to do until the last day. Both of my arms are extremely sore and I can barely move them. I have had issues with ivs every time I have needed blood drawn/iv fluids, but this is the first time so many blew. I had one girl try to draw blood in my hand and she inserted the needle and decided to dig around with it. That's my biggest bruise. 😪

I had wrist surgery on R wrist with a brachial block 12 weeks ago. 4 weeks after surgery I began to experience burning pain in my breast and tricep and under my arm as well as behind my right shoulder. Recently the burning pain has progressed down to my wrist and hand with increased redness and swelling of my hand. Conduction and EMG tests showed interference in the Brachial Plexus. I’m waiting for Plexus MRI results at this time. Do you have any idea what may have caused my issues? Thank you very much for your informative video.

Also, sell your car and never ride on an airplane again. There was an interesting Reddit thread with some anesthesiologists and crna's discussing the topic. One mentioned that that results of the studies on the environmental impact were pretty weak. It would be nice to see a video from the other side.

Cannula quality questionable , after successfully inserting a cannula reinsertion of the stylet is not recommended as it may cause a double prick messing the all good end

In Australia, many hospitals are removing desflurane entirely from their machines. Personally, I've always been a bit skeptical about environmental impact of TIVA - There is alot of plastic waste, especially for the longer cases and that needs to go somewhere, right?

Great vid to think about further impacting the environmental impact of anesthesia. Unfortunately not consistent when giving examples of climate impact (m3 of molten ice vs. l of gasoline... ) and no relatable real world examples like "if you do 1 hour of minimal-flow anesthesia with this agent you could drive from A to B, while using another agent your distance would be...". It would also have been great to tell what applications remain for Desflurane (and not just wave it off as "should be reserved for cases where it´s clinical benefit is undeniable"). Also regarding TIVA: there were only papers shown about PONV, not about its overall resource consumption/climate impact; given that there is a lot more plastics and packaging involved the overall climate impact is about the same as minimal-flow anesthesia with sevoflurane...

While talking about using TIVA, many patients don't tolerate it, also third world countries can't afford the exuberant price of the propofol, there is money making involved in these drugs. TCI pump's cost is very high. There are too many hurdles to cross. But yes, the issue is real. And not just it's anaesthesiology responsibility, it's the pharma company's responsibility and biomedical engineering product company's responsibility too. We can all be aware, but sometimes it's the broad society involved in healthcare services which needs to take responsibility too. Thanks for the insightful video.

If a pot smoker who has a 3X faily (edited: to daily) over many years, and is facing surgery with requirements on abstaining and the patient is incapable of the this, try therapy and Marijuana Anonymous and stop daying "you can't stop." If you're not addicted psychologically or physically, you CAN stop. If you can't literally follow surgeons orders to stop in the time frame presented, then get over yourself & get to get help from a pro, because you can stop with help. If Eric Clapton and Stevie Ray can quit shooting up and boozing it which causes physical addiction, then people that are pot addicted can stop. If you can't stop, youre addicted. If anxiety keeps you smoking, get help with that. Start being grateful you found a surgeon willing who cut you and put in a new joint, or whatever surgery you need to give you quality of life. Pot is not quality of life. Its a crutch. You're not that special. Many people do this, they abstain to get the surgery. Or forget about the surgeon & keep smoking your selfish pot. Choices are possible.

Question, when you know you have adequate position of your needle tip by visualization of the tip in the lumen on the long and short axis using ultra sound, but do not have flash or see draw back, what could be wrong with the insertion or art-line? I have many times encountered when my needle tip is lumen on a very hypotensive patient and I can't get the art line to work, when if I get draw back of blood with an empty syringe? Just need some help :/.
