Great video, been contemplating going down this path for a little bit now. And I feel you thoroughly explained the good and bad. The bad... thinking about not saving an animal and then dealing with an angry owner has to be incredibly tough. But like you say at the end... even if it is 25% I bet everytime you save one makes up for 100 bad ones. Thank you!!

I would love to know the consequences and treatment of each arrythmia. I was watching your video a few months ago because my bulldog had 2 episodes that caused him to collapse. His body was limp but he would track me with his eyes. It took 10 to 20 min for him to return to normal. We did and ecg and ultrasound and sent to a cardiologist. They said they noticed and arrythmia on the ecg,did not identify what kinda, listed ones common to the breed but recommended a holter monitor to determine the frequency. His ultra sound was normal on every aspect of the assessment. He passed away before we got the holter monitor. He laid down while walking through the kitchen and was instantly non responsive. He was 13 and the love of my life. I tried so hard to learn so i could ask the right questions. Your videos are quality and helped me when i was helping him fight for his life. 😢

I’m so sorry to hear this. It’s harder than words can express to lose one of our family. It sounds like you did everything you could for him and he was lucky to have a mom who fought for him. It sounds like his work up was appropriate. Arrhythmias can be tricky, they so seldom look textbook like the way you learn about them. And they can be very intermittent. I’d be worried your guy was going into episodes of v-tach based on what you’re describing, and it sounds like the holter would’ve been the appropriate next step. Most acquired arrhythmias can’t be cured, we just attempt to control them for a time, but they’ll eventually stop responding to treatment. I hope you find solace knowing you did everything you could for him.

Are there any jobs you could work with a bachelors degree in veterinary technology that don’t include emergency medicine / having to work weekends and holidays? I’m considering getting a degree from Penn Foster but it seems like no one is hiring Vet techs where I live, I’m not sure if I could get any other jobs with that degree or if it’d just be useless unless I move somewhere else.
@jenniferlyonsvts
@jenniferlyonsvts Ай бұрын
Penn Foster is great if you want to take the program to become eligible to take the VTNE. It requires a very strong will for independent studying and research. If you do better in a class or group setting, this program will be a struggle. Also, I believe the degree from Penn Foster for vet techs is an associates. And, many of the courses will not transfer to any other schools. It looks like ten of their classes might transfer to another school for academic credit elsewhere. If transferring your credits doesn’t matter to you then this isn’t an issue. In general, the program is meant to give you background info on vet tech related courses, and to make you eligible to take the VTNE to become certified or licensed in your state. It depends on the state and area you live in if that will give you a big hiring boost or not. Some regions or states care and have strict laws about what licensed vs un-licensed techs can do, and some do not.

Typically, as it is harder to staff and has a high degree of turnover therefore compensation tends to be higher. Also these days many ER’s are corporate which are known for higher pay and better benefits (although of course there are some exceptions).

What are your landmarks on how far should the picc line be ? Gage I feel comfortable but length is always my question
@jenniferlyonsvts
@jenniferlyonsvts 4 ай бұрын
Great question! Unfortunately, since there are not an endless amount of diameter length combos, I usually end up picking my diameter as my priority. If you rely on too large a diameter you’ll be dealing with painful foot swelling, and I try to always prioritize patient comfort. For length, ideally a PICC will sit in the caudal vena cava, so measuring just caudal to the liver would be ideal placement. But, often times these catheters end up being a little short depending on your patients conformation, so as long as it samples well I’m also totally happy with my PICC terminating in a femoral vein.

For oscillometric readings in hind limbs, do you place you cuff above or below the hock? For Doppler readings, you place your crystal below the hock, so I typically place the oscillometric cuff right over that artery below the hock as well. Would you agree with that placement? For the Doppler, is it more accurate to place your cuff above or below the hock? I have seen it done both ways.
@jenniferlyonsvts
@jenniferlyonsvts 5 ай бұрын
There is going to be some natural variation in blood pressure readings taken at different places on the limbs, but the variations should be relatively negligible. So, it is appropriate to place the oscillometric cuff above or below the hock and I will usually choose based on patient conformation (I.e. really obese patients, patients with short/stubby legs, or patients with excessive fur above the hock I pay choose the place the cuff below). For Doppler the cuff is simply occluding the artery, while the crystal is taking the reading. So, placement of the cuff shouldn’t effect your readings if you are still getting good occlusion and release of the artery. I would place the cuff wherever it seems to fit best.
@bethanycrissman5152
@bethanycrissman5152 Ай бұрын
I was taught that if you can palpate a femoral pulse then your pressure is at least 60 . Is that true? Also , conversely , aside from issues like obesity or too much pressure being placed on artery, does the lack of a palpable pulse mean the bp is below 60?
@S1g15
@S1g15 5 ай бұрын
I have been measuring cats' bps forever with a size 2 cuff based on the 30-40% of the limb rule. Over the past year, I was introduced to the reference lines you mentioned inside the cuffs. When you use the reference lines, many cats will be right at the high end reference line on a size 2 or above it, making me wonder if a size 3 is more appropriate. I see the kitty that got a size 2 in this video looked to be close to the high end of the reference lines once placed. What are your thoughts on this? Do you rely more heavily on the 30-40% of the limb rule or the reference lines inside the cuffs to ensure appropriate sizing? Also I saw a ce that considered <120mmhg systolic a hypotensive reading for anesthesia. I have always used 100mmhg as my cut off for hypotension with systolic so seeing you use 90mmhg made me feel that I haven't been taught incorrectly and allowing inappropriate readings to persist.
@jenniferlyonsvts
@jenniferlyonsvts 5 ай бұрын
I usually go based on the reference lines inside the cuff, and I agree that some big or chubby kitties end up being a size 3. I think a lot of us were taught different reference ranges, and even when you read different textbooks you will see variation in listed normals. I honestly don’t get super nit picky about which reference people use unless it seems blatantly incorrect. I would argue that a systolic of 100-120 is pretty normal in a cat, and I think treating hypotension with a systolic below 120 is a little too aggressive as there are negative side effects to aggressive hypotension treatments as well (fluid overload, risk of arrhythmias from pressers, etc.). But honestly I usually only treat the MAP in regards to hypotension. I worry about the kidneys once the MAP drops below 70. If I had a MAP of 75, and a systolic of 89, I would be more worried about how accurate that reading was due to how close together those two numbers were. I would probably try my reading again, and check a Doppler if I was still worried. But, in the end, I am going to be primarily focusing on the MAP, and treating hypotension once that falls below 70.

I’m so sorry to hear that! It depends on what the underlying condition is. If it is for a toxin then very very good, and for treatment of IMHA usually pretty good, but prognosis can go down depending on how systemically sick the pet is. I hope she does okay!
