Although I am in remission from Crohns, I am experiencing more severe symptoms than ever. This condition is perplexing, and I recognise that I am not the only one facing these challenges. Meanwhile, others in remission seem to be thriving, which is as it should be. Its hard to keep hopefull. Thanking you for everything your doing🙏

Lots and lots of exciting new drugs available indeed. Im led to believes anti tnf's are still the main ones to try before jak inhibitors, yet i dont understand why ive only been offered to choose between aza or rinvoq. Id like an anti tnf before turning to jak inhibitor, or even aza but not been given that choice :-(

Dr. Lees, thank you so much for your passion and dedication to this horrible disease. My Story. Canadian born and raised. Mom was Ukrainian and Dad was Scottish. 2010 was given Clindamycin to prevent infection of a root canal. Developed UC type symptoms shortly after for the first time ever. 2016 at the age of 50 started to show signs of UC. Diagnosed with UC shortly after. In and out of UC flares controlled with steroids. Transitioned to Biologics Jan 2020. Humira and azathioprine not working. March of 2020 too sick to work. lost 40 lbs, critical. Looking to cut my Colon out. Moved to Xeljanz. Still not working. Meanwhile, changed my diet radically. No processed foods at all. Changed to high fiber, fermented and gut friendly foods. Before they were going to cut my colon out Dr. suggest we try steroids one last time to see if will help the Xeljanz work. After weaning off of the steroids boom, I went into remission. I firmly believe diet change and the meds have kept me in remission for 4 years now. Last Colonoscopy on Dec 2023 showed no signs of disease. Quality of life has been awesome. Thank you for what you do!

Hi Dr. Lees, I’ve been an Ulcerative Colitis patient for 5 years, living in the Netherlands and from a middle eastern origin. I got diagnosed five years after my difficult but successful immigration to Europe. I’m an engineer by profession, so understanding details is in my nature. Your videos always give me a dose of optimism, and I really appreciate how they provide information I can research further. It helps me better understand my condition and communicate more effectively with my gastroenterologist. Please keep posting new videos-they are truly helpful!

What studies have been done to evaluate the efficacies of these various medications for individuals who have had Crohn's for many years? Early detection of Crohn's seems to be the exception rather than the rule unfortunately.

I’m sure that Eye and many other IBD patients, would love to know which emulsifiers are the least harmful, as for those of us following as healthy diet as possible, still can’t avoid them sometimes,.

I'm about to begin Ustekinumab. I have Crohns and Ankylosing Spondylitis, I'm hopeful that it could put me in remission.

I'm keen to hear your thoughts on the research published in early June about macrophage inflammation through ETS2. Maybe it will be a decade before new treatments may come from this, but it does still seem like big news for IBD?

Biologicals have a low success rate, are VERY expensive, and have adverse side effects. Diet is a bigger factor. Whole foods high in phytonutrients has worked for me and many others. I followed Dr. Chanu Dasari's protocol (on KZbin - but his book is better) and had good results. If you can make a difference with diet, why deal with the meds. Not only can improve your Crohn's but also improves your overall health for a better life and longevity.

kind of sucks that in Germany (where I currently live) it's not really like this. Patients here have to "earn" their way to biologics with being diagnosed as having severe Crohn's...

Interesting video. One of the most exciting use cases I can think of would be to use AI and the massive amount of undigested (pun intended) data on the field of microbiomes to spot something new, as its very likely the main clue to treat the root cause of this disease is hidden somewhere in there. Many things indicate this, including how EEN is one of, if not the most effective current treatment for Crohn's. There was even a recent study on home-made EEN-s showing similarly impressive results even though every single ingredient different from commercial formulas. The most likely explanation hence is not what is in these formulas, or whats not in it, but either the texture, or the monotonity of foods. Leaning towards the latter if you think about how other diets work but for significantly less people - sticking to one of those diets like CDED or SCD just makes it more likely that an individual would accidentally find the right level of monotonity for them, that sustains a microbiome that keeps a potential pathogen in check in the context of their specific body. For many of us with Crohn's, diet has an immense role to play, and I have learned a ton by checking my calpro on a weekly basis and switching between radically different and very strict diets in few week intervals. It is so incredibly frustrating to see week over week how my diet changes have a very direct effect on my calpro but not having the tools to generalize these insights despite having a degree to basically analyse data - if this could be measured on a daily basis e.g. via a wearable that can produce a new biomarker, or with some daily calpro arranged, and then you could just tell to GPT 4.o what you ate, then you dont even need 100s of people like me willing to adhere to strict diets, because the data would be there for an AI to find hidden in people's regular dietary patterns! Ofc even better if some participants are willing to stick to some system and both of these trials could in the end complement each other. There must be a few more people like me willing to sacrifice some to contribute to curing this disease, there is just noone who shared such a plan so far. Then if all of this could be complemented with regular microbiome samples during such a trial and ran through the same AI, then the results could be absolutely game-changing. And even if we dont have the tech now, in a few years someone could take those samples, create detailed microscopic images, and have an AI mass process them. Imagine that if I could discover solutions to keep my calpro below 250 relatively consistently, when it was 1000+ beforehand, with weekly measurements of a single biomarker, and keep myself in clinical remission, wouldn't it be reasonable to assume that if we multiply the data samples with 3-4 other dimensions, 100s or 1000s of individuals, and then have it processed by an AI instead of me with an exercise book, we would get closer to a cure than ever? Or at least to a consistent way to put people into at least clinical, or indeed even deep remission? Perhaps I am naive but in the fintech world where I work, we would jump to much smaller opportunities much more quickly, and win battles that seem just as difficult as this one. P.S.: it is GPT not GTP :P

My IBD team's version of monitoring is a phonecall every so often to see how I'm getting on. So far, it's been put back multiple times and is now six months from my initial phone call appointment. So, to sum up, the NHS is on its knees and your ideal utopia of 'monitoring patients' really doesn't exist in my and a lot of people's cases. I just ring the IBD team if I get symptoms of UC occuring and that's it. Then once they get in contact after 48 hours at the earliest, they'll arrange a calprotectin and blood tests.

Hello Charlie! I was diagnosed for Chrons disease more than 2 years, I have manage to keep it in remission as of now, with low terrible days, i changed my diet completely, I have no bad habits, I drink azathioprine, i drink a lot of vitamins and use organic anti-inflammatory foods for my self, and i also see that sports made a huge different for me and for my bowel movement including gass . I can mentioned a few meths but also videos like these helping a lot

Ive had an adverse reaction to Azathioprine Infliximab caused Demyelinating disease Primary none responder to Vedolizumab Just going to start ustekinumab So this is fantastic news as im fast running out of meds

Hey Dr Charlie! Thanks for the Atomic IBD Uncut series, very useful! I'm a 28 yo ileal Crohn's patient from another country who has the HLA-DQ2.5 phenotype. I have been managing my Crohn's with strict dietary therapy mixed with occasional EEN-s and regular calpro monitoring since my diagnosis 18 months ago with relative success. My doc wants me on a Humira biosimilar, and I also wouldnt't mind getting my social life back, but I have read many studies about how my phenotype will make secondary treatment failure very likely. I have also seen that after such a failure other drugs will be less likely to work afterwards. I have sent my doubts to my doc a few months ago but so far no response. In a month I will have to go back and decide but I am very reluctant unless we are going to do proactive drug monitoring / escalation, which I have read has a good chance of preventing the treatment failure in my case. Not asking for medical advice but would you mind sharing your view whether my approach makes sense, and whether this proactive drug monitoring is a common thing in Europe and is it reasonable for me to expect it? In case its not a possibility for me, but its common or expected to be common in clinical practice elsewhere, then I wouldn't want to exhaust this treatment option here, well, at least until I am able to keep my disease more or less in control without it. Thanks in advance if you can share your view!

Thank you for mentioning Meat consumption and how it relates to IBD. Is it the same for eggs, dairy, or fish? All are good protein sources that I have but I'm now concerned they might be not as good for me.

Brilliant well done and a well deserved award! I've developed and delivered courses in behaviour change for healthy eating for the NHS/dietetics and I think a support group programme offered to IBD patients covering and including everything you've mentioned would be such a good thing to offer on the NHS. I know how useful I and my fellow ibd peeps would have / would find this. Funding funding funding hey, ha.
