I was a heavy drinker for 20 years and now have hip pain in both hips X ray showed moderate arthritis. I'm wondering if it can be confused with AVN? My doctor does know my drinking habits and that I have stopped drinking 8 months ago. It seems that this hip pain has gotten considerably worse in the last 6 months. Also if one stops alcohol use does AVN continue to get worse? Thank you for your time.

Good question. The only way to confirm the diagnosis is to do an MRI scan. Reducing alcohol consumption does reduce the risk of future AVN but it can take a long time for the damage to become obvious.

i’m 26 with osteoarthritis since i was at least 23 in my right subtalar ankle joint i think i got it from working like 50 hours a week. anyways. the only relief i get is a cortisone shot twice a year for the last 2 years.

Hopefully that will work for a few more years. In my experience it usually requires a fusion procedure eventually.

My grandma has Parkinson’s and she fell and broke her hip a few days ago. The doctors says hip surgery is risky for people above the age of 50. I’m so anxious. She had trouble talking and walking as it was, but her breaking her bone made it harder. If you have any advice please tell me.

I’m sorry to hear that. I can’t give patient specific advice without an assessment. In the UK patients who have a hip fracture usually have surgery within 48 hours of the injury. A multidisciplinary team manages the medical care and helps to optimise the patient’s condition before and after surgery.

I stayed in for1 night which suited me really well. I had extra pain relief during night and felt ready to go home the following afternoon. I love these videos, seems like a great, compassionate surgeon, just like mine!

@Lathamhipsurgery o had to pay privately because I had waited for the nhs, I'd have been in a wheel chair! So went to Spire, Wrexham. And had an incredible surgeon, I trusted him completely. He changed my life as you are for many. The lame can walk again!

I had hoped to be out the same day but my op , done while conscious , wasn't started till about 1130 am. The physio staff couldn't fit me in on the afternoon round therefore I had to stay overnight. I was assessed first thing in the morning, passed with flying colours and left the hospital about 1430. Had someone stay the first night but I managed from then on independently. Had friends to help with cleaning the house and walking my dogs but all went really well.

Your comments on differences between US and UK health services is spot on. In the research I did I learned that in the US most surgeries are posterior. There are huge adverts for a robotic machine used in the posterior approach. I opted for anterior. The surgery was done in an ambulatory facility associated with a large teaching hospital. There were issues that prevented me from going home the same day. It was even in doubt that I would pass the "exit requirements" the next day. I finally "passed" late in the afternoon. Even so going home without the support I had would have been impossible. Nearly a year later I am still working through the bills. In the US medical debt is the leading cause of personal bankruptcy. On another note, I later learned that a representative from the prosthetic manufacturer was present during the surgery. Is this common in the UK? My guess was that the presence of this person had something to do with the crazy liability issues in the US.

I am on the waiting list but have been told 1-2 nights. I am in 2 minds about day case surgery which seems so major. On the one hand I would love to be able to get home as long as I can control the pain, on the other, as a nurse I have seen too many things go wrong in the post operative period (with various procedures) not to have some caution about it. The hospital I have chosen is 35 miles from home, so I think I will feel more comfortable being monitored for the first 24 hours or so.

I agree. I have a lot of patients with multiple medical problems. They need to be carefully managed in hospital for a few days. The thought of them languishing at home worries me!

I shall be going to have my THR done in about 4 weeks in an elective surgery centre in Nottingham. Most patients are day cases but I will be staying one night because I have Type 1 diabetes and AF. Quite happy about this but if I feel well enough later in the Dayi could be allowed to go home.

This is reassuring thank you for the advice I’m 3 weeks post op today and I know my body is limping unnecessarily it just does it ! I have to think walk tall and feel my feet placed in the floor while I transfer the weight. I walk so the one sticking the house sometimes none when I’m close to the kitchen units. Outside walking I take both sticks just incase. But it feels so good to not be in pain and hobble about. Good luck to all. X

Thanks for sharing your experience. It takes a few weeks to get to grips with the sudden change in your body's biomechanics. In a few weeks you will mostly have forgotten about it!

Hi Dr. I broke my hip almost 11 weeks ago and had to get total hip replacement. I've improved a lot since however I still have a very slight limp and some little pain when walking? Is this normal? Do I need to wait more time or consult my doctor? I'm 20 years old btw, thanks

That’s really tough at your age. The physical and psychological trauma can take a year or two to deal with. The limp and discomfort will get better but I’d see a physiotherapist at this stage. Hope it goes well.

Doctors, nurses are so overworked. I'm a double heart valve recipient. Also I have hip replacement surgery. Plus I have atrophic kidneys. I try not to bother them then I wind up in worse shape.

@@Lathamhipsurgery Thank you! May I ask you a question? My first THR was this March and was anterior. The revision he had to perform was in May, lateral. He’d told me ‘no restrictions’, but I was terrified. At 8 wks post-op now, is it safe, in your opinion, for me to bend forward enough to put on shoes, shave my legs etc?? I’m so nervous that my regular physician has me on anxiety medication. I read a lot but it’s very mixed! Some articles say ‘no restrictions’, others say don’t bend for 6-12 weeks? It’s very confusing. Thank you for your guidance!!!!!🙏😌

Hi Jeremy, I hope you are well, I went abroad to Lithuania four months ago for a dual mobility hip implant. The surgeon seemed evasive when I asked questions about the implant, before and after the operation. Anyway I just got my x-rays after 3 months from the NHS which I have just sent to them to satisfy their garrantee. They seem poor quality, and I can't help thinking they have put in a standard prosthetic instead. Would you be interested in taking a look if I sent you the x-rays. If they are not clear enough I will go private for better x-rays.

Good video and an informative update- thanks. However, I notice that you don’t mention the competition. The Ceramic H1 Hip Resurfacing implants currently being developed by the fantastic Prof Justin Cobb and his amazing team at Imperial College, London. Like Recerf, H1 isn’t yet available commercially as it completes the approval process, however, it may be available via a clinical trial. Having personally benefitted from bilateral hip resurfacing with H1 implants just over 3 years ago, I can say that they’ve given me back a full and active life - truly life changing.

Total left hip replacement at age 68. I was playing competition golf after 10 weeks off our back tees. Obviously walking the course which is 6500yds. Got back to 9 index at age 70.

I was told that i had to lose 75 pounds before i could be considered for hip surgery. That was 9 months ago. I have lost 35 pounds on my own. Still struggling and my hip is in very bad shape. I can no longer stand on my right leg, so no exercise except upper body. This is a struggle.

I was diagnosed with AVN 35 years ago (age 30) and had core decompression surgery. I also used an “experimental” electric bone growth stimulation device, I was cured and still have my own hips and am pain free. I guess I was lucky.

I often wonder if the experimental bone stimulation device ever got FDA approval. Although i can’t say whether it was the operation or the device (or perhaps the combination) that cured me. I had to sleep with the gizmo attached for a year.

I have had two total knee replacements & both times experienced restless legs. It is a horrible feeling. Thankfully it disappeared after a few months of being fully recovered

I just found out I have stage 3 avn and have to get total hip replacement on both sides my right side is on the verge of collapse it’s the worst side but my left is really bad too smh please pray for me I’m only 36 never has an injury to my hip my dr says I’m in the rare percentage of ppl who get it because none of the other reasons explain why I have the condition

That's a fab video ! I stayed conscious for my hip replacement so I could hear everything that was going on :-). Good to be able to see what goes on as from my shoulders there was a sheet of blue partition and I was obviously facing the wrong way :-) Obviously a good thing ! Poor Anaesthetist had to chat to me the whole time . He probably was wishing he had sedated me :-) .

hi i'm 5 months post R anterior total hip replacement. i still cannot walk wout the aid of canes or a walker. my quadriceps are still swollen and numb, getting better but still swollen and hurts to walk. The surgeon said i was so "tight" he almost had to break my femur to get the hardware in place. I think he caused severe trauma to my connective tissues in that area. My L hip needs total replacement as well so that's impeding recovery.

That sounds painful. I tell my patients that it can take a year to 18 months for the trauma to settle and complete your recovery. A good PT will be able to help you get better more quickly.

@@Berley_1234 Your surgeon is best placed to advise you on that. The decision to offer surgery needs to be based on a thorough clinical assessment and discussion of the risks and benefits. Having said that, I would be reluctant to do surgery on someone who hadn't completed their recovery from the other side.

My husband is 71 & his hip has been bothering him for the last two years and getting progressively worse. An x-ray showed bone on bone. He is in otherwise good health. Based on this don't you think he'd be better off getting a hip replacement now versus waiting?...

From what you have said, he'd probably do well to consider having surgery sooner rather than later. An experienced hip surgeon will be able to assess him and guide him through the options.

I had my total hip replacement 14 weeks ago and although I've not had a dislocation Ive had it move out of the socket and pop back in about 9 times. Just yesterday it popped again when I went to tie my show lace and today at the gym on the seated calf raise machine it also popped. When it happens Its really distressing because If its moving out of place this easily then it might not be far from dislocation.The first few times it happened I put it down to the muscles not being properly healed but Its now becoming a concern that my mobility will be really restricted. I am a martial arts instructor and was very flexible before the operation. Each time it pops out and in the front of my hip is sore for days.

Update: 4 days after my sudden dislocation, my surgeon went over my scans and x-rays. I was told it looked fine and to go on doing my thing. It dislocated again while I was home alone. I fell to the floor immediately. I had to call out to my phone, to call my family/ambulace. I ended up having a hip revision a week later, this May. My first hip surgery was only in March! My surgeons partner found the problem. The angle of the ball and joint was incorrect for my body. Now I’m healing all over again and it’s much worse this time, I pray to turn a corner soon. I want to have my life back and forget this ever happened, though I know I’ll never forget, it was too traumatic and will take a long time to heal physically and emotionally. I found out that the first dislocation caused me a compression fracture in my spine as well, when I landed on the concrete the first time. That pain is the worst right now. I heard it’ll take months to heal. I cry a lot these days.

@@Lathamhipsurgery Thank you so much. My regular Dr called me yesterday and did an evaluation. After it was all said and done, she said I definitely have PTSD, anxiety, depression etc from all of it, by now he answers I’d given to the tests. She’s called me in meds for the pain in my spine, feeling it’s nerve pain, anxiety meds, something else with serotonin to help me sleep etc and she will monitor me very closely to see how it works and for any side effects etc. I’m praying for some relief, I think it will help me greatly. Thank you again!!!! I so appreciate all your feedback🙏

Im 60yrs old and I fell just thought I had torn muscle! After 2 days walking in pain went to ER and to have surgery right away I had to replaced my hip cause it was broke! I recovered quickly, but I’m having a lot pain in my groin area asked about PT completed that and it just won’t go away! Is this normal and how much longer will it last and should I be concerned! and back of upper thigh! I

Thanks for sharing your knowledge. I have just had TLHR ( 8 wks ago) and miss swimming so much. 😢My physio has told me I can now resume swimming again - with obvious precautions, plus walking in the water. Before my surgery I did a lot of back stroke as I struggled with breast stroke so much. I also bought a float which I used to do front facing kicking. Do you think I could start to do this as well now ?

If you are walking without any aids and you feel confident, then it should be fine. Take it slowly and if it hurts, stop! Hope it goes well.

Hi, If you don’t mind, I’d like to share my story? I’m a 58 yr old mom, wife and Granddma. On March 13th, I had a hip replacement due to osteoarthritis. Everything went well and according to plan. 7 wks into recovery, my hip dislocated out of the Blue as I was standing. Hit the cement, hard falling backward, fracturing my spine. It was beyond a pain and trauma I’d ever known. It took over 6 attempts to re-set my hip. I begged to be knocked out and finally was. My surgeon was shocked when this hospital called him. He requested all scans and x-rays. After looking them over, he was finding no reason for it to have happened? He was sorry and told me to continue on with my PT as I was! 4 days later, it dislocated again as I stood in our bedroom, standing up folding a blanket. I was ALONE this time! I screamed to my phone as my adult kids showed me ‘just incase I ever needed to?!’ Hey SIRI, call so and so!!!! Thank God it worked! My husband, kids and the Ambulance all showed up together. Again, off i go the hospital screaming in agony wanting to die, they knocked me out and set the hip a second time. This time, my surgeon who’d been called again, had me brought to his hospital directly. His partner found what he himself had missed. The new hip ball and socket were at the wrong angle ( for my particular stance). He was devastated and clearly very upset for all I’d gone through. Now, I lay in a hospital bed 4 days, not allowed to really move as he told me it will come out again, and then, I had a revision surgery by him. He used a different size ball/socket at a slightly different angle, in another location, ‘anterior lateral’, and 22 staples vs the 12 sutures from the first surgery in March.This took place May 21st, so I was not even healed yet from the first surgery and had all of this happen one after the other. Today, July 2nd, I’m just now able to walk on a cane ( briefly in my home). I’m in awful pain from surgery AND the spinal compression fracture as well, which will heal over months. On top of it, I have extreme PTSD that is ruining my life. I cry a lot. I’m very depressed because I can’t do anything more than some PT and scoot around my house. I’m exhausted. I can’t sleep because my back hurts and I can’t sleep in any other position yet. I’m a complete mess. I try to be strong and some days I am, other days I just want to give up entirely. I can’t bathe myself. My husband was out of work for one month with no pay because I needed that much care. We have a company coming to change out our bathtub/shower combination, to a walk in shower. I’m scared every time I move. I can’t get it out of my head the ‘what if it happens again?’ I can’t live in fear like this. My surgeon and PT said it’ll be a long road. I won’t turn a corner at least until the end of July? Revisions take longer to heal. Considering it all began in March, it feels forever away. I don’t want to whine about it but I’ve never felt THIS low in my entire life. My quality of life feels non-existent. Do you have anymore words of advice or anything that may pull me out of this dark place I’m stuck in???? I’d really appreciate it🙏 Thank you!!!

Some have very bad pain months after hip replacement 4 months after I had a revision and still in a lot of pain and stiffness

I’ll m 8 wks from a revision. It’s such a slow recovery this time around. I’m a 58 yr old woman. My personal Dr has prescribed me several prescriptions. They help so much! Can you speak with your Dr about it? When my pain is controlled, I do much better with ‘everything’ from PT, to some housework, sleeping at night, cooking dinner etc. I wish you the best!!!!! Me too actually, as we both know, it’s no fun when you’re hurting!

I’m not on Facebook, but I listen to an Australian podcast called Joint Action. It’s run by David Hunter and he invites various experts around the world to share their knowledge and help people. One thing that’s happening in Australia which I wished would happen here is, after diagnosis (hips and knees) people are automatically referred to a pain management programme, but a new thing they are trialling is physiotherapy classes. A physiotherapist will run a class of exercises for about 10 people with oa and the patients love it. 1) they know they’re doing the exercises properly because the physio can correct them if they aren’t. 2) They tend to do more exercise because they are encouraged by the group. 3) They really enjoy it because it becomes part of their social life. 4). The need for surgery is postponed for up to 2 years and in the case of some knee problems, surgery is no longer needed. As someone who spent her life exercising in groups and gradually had to give up all of them, I would have loved to have had something like this to go to. Most GP surgeries are employing physios now, it just needs someone in a position of influence to get it rolling. 😉😉I’m sure it wouldn’t cost much to get rolling and the benefits financially and to patients would be enormous. I had a total hip replacement 2 1/2 weeks ago (in France), and it seems to have gone well, although I am in a lot of pain, especially when it clicks, which it does a lot. I was told by the surgeon I must "walk, walk, walk", which I am trying to do, but some days it's just too painful. Should I keep persevering through the pain, or rest more? I am also having physio, which just consists of strapping my leg to a device which moves the leg backwards and forwards by bending the knee - any thoughts on that? TIA

I advise all my patients to take it easy, particularly in the first 6 weeks. Surgery is a major life event, both physically and mentally. Surrender to it, take plenty of rest and don't push yourself until you feel ready. You have described a Passive Motion machine. Knee surgeons use them. I have never used one after hip replacement surgery. A specialist hip physio will use a variety of techniques focusing on active movements, core stability and balance. Hope this helps.

Had a total hip replacement 4 days ago Walking slowly on crutches but the pain is severe, even when resting up in bed.its pretty rough.hopefully it will be ok once the swelling goes down as my thigh and knee has doubled in size

I can’t wear metal jewelry from chains, watches, even 22 k gold ring, skin got really hard and itchy ant turned a greenish color, I always avoid wearing metal now but my doctor says I need hip replacement and I think my body will react, any advice ?

It’s possible that you are sensitive to base metals like nickel and cobalt. Titanium implants are unlikely to cause reactions. I use them in people who have a history of metal allergies. Ask your surgeon for specific advice.

In my experience it’s anywhere between 2-4 years after surgery. It’s very unusual so sometimes it can take years to reach a diagnosis.

Specsavers Lee! Your hip is not a problem. Titanium is very unlikely to cause a reaction.
