Is there a problem with having high Free T levels specifcally? If other levels are good? My free T level was 315 and Total was 1,124 at a dose of 160mg a week and my doctor had me go down to 120mg a week even tho other levels were good

Dr Robert Stevens - one of the few who are unafraid to simply be honest and tell the truth - he doesn't sugar coat it (maybe a sprinkle of hCG though!!) - EFFORT = REWARD - it is what I live by and it works. I started lightweight bodybuilding at 19-20. At 63, I was doing 3 hour workouts 4am-7am every single day, but ensuring lots of rest (diet is excellent) - but during my 63rd year, these long workouts seem to have lowered my Total Testosterone to 245 ng/dL (my doctor said that low can be risk of cardiac events). Feb 8, 2024 I started Testosterone Cypionate (100 mg/week, injected twice/week). Some E2 rose up, so switched to 4 x 25 mg injections per week (that helped). After 4 months started Pregnyl hCG (250 IU 3 x week) - now things are getting much better. So 6 months in on the TRT + hCG therapy, and things are starting to settle in ok. hCG has caused another slight rise in E2 - hoping it is temporary, but my doctor is ready to prescribe Arimidex (Anastrozole) - but is Aromasin (Exemestane) really better? I heard you say that in one of your videos I think. In Canada, Aromasin is much more expensive than Arimidex. So to the point of THIS video, yes TRT helps, but it is MY effort that is earning the reward. I was struggling when I was 63, but now I'm flying at 64 thanks to TRT to power my workouts.

Interesting video. I was reading about your gold standard approach to TRT on your website, and utilising micro dosing in patients. Does the daily injections cause scar tissue? Does the patient need to do subcontaneous and or keeping switching injection sites?

Being a man nearing his 50s, and living in the US, the decline felt horrible. Unfortunately getting any TRT treatment, it’s nearly impossible. Most clinics want to see test levels under 250, prior to helping you. At that low of a level, my body would likely begin growing Boobs. That said, your videos have been a Godsend. Daily Sub-Q’s at moderate consistent levels of cypionate have truly brought the energy back felt in my mid 20s to early 30s. My doctor is fully aware of my choices, and has been extremely supportive. He’d mentioned daily sub-Q dosage was the best way to go, but unfortunately the US won’t support the science. That said, he tests my blood levels every 3 months, and has been amazed of the positive results. Admittedly his experience has been limited under the TRT guidelines for prescribed patients, and others are typically taking anabolic levels, thus damaging their bodies, and see him for a solution. So Doc, from the many of us in the US, your knowledge has been a blessing. Since stumbling on your videos, I have been on the daily Sub-Q treatment for a number of years now, and somehow have not needed any estrogen blocking treatment. Not sure if that is a rarity? But my body responds extremely positive with daily testosterone Cypionate. Thank you so much for educating the many of us that life can still feel rewarding, regardless of the number of years we’ve been around.

Hey Doc I'm 45 and have total t at 950-1300 for the last 10 years. I'm not using drugs or steroids. My SHBG is 95-120 nmol/L. All my other markers are in range. Prolactin,e2,lh,tsh,dhea,albumin,thyroid etc. I'm working out at the gym 4 times a week I'm eating healthy with a lot of protein and nothing seems to lower it down. My sex drive is not existent. Proviron is my last chance before I consider TRT ;I really don't want to go this way). Would you think Proviron may help it? Tnx

Dr. Robert, I believe in daily dosing for sure. In my country I am prescribed Test Propionate. Im on Test Prop SubQ daily in the evening. The reason for the shorter half life is to mimic the natural diurnal rhythm of a Healthy Natural Male with this protocol (with appropriate peaks and valleys eg. Rising while sleeping, peaking during morning hours and declining towards bedtime) Why would daily Test Cyp be more appropriate by reaching a steady state? Or in other words, why would you say Test Prop is not appropriate?

Advice please! Right before my TRT treatment total test: 402 free test: 6.1 and estrogen: 21. I had my 1st injection on monday and I asked the doc if I could try without astrazole (misspelled that lol) and he said absolutely NOT, it is required. It was only 21. I have even more fatigue, chest pain (on the left side) i am thinking due to anxiety, libido even lower that it was before with even more ED issues, and i just feel numb to everything. Could crashed E2 be my issue? I go in for my 2nd shot tomorroe and if the doc doesn’t agree to treat me without an AI i might find a new doc. It would make more sense to me to start trt treatment without the AI unless I get high E2 symptoms or super elevated levels at my next blood withdrawal.

When should you test if your on daily micro dosing of cypionate. I inject daily sub Q, usually in the morning. I started this new routine because my estradiol was 50.7 pg/ml and I did not want to be on a AI. Also I inject HCG twice a week, Tuesday and Saturday. So my levels should be stable with no peaks and troughs right? So in that case, does the timing of the test even matter?

I paid for private tests that came back with a TT of 10.3 nmol/l and a FT of 0.20 pmol/l but couldn’t justify the expense of privately funding TRT. I’ve been to my GP who ordered a blood test for TT and this came back at 9.5 nmol/l and I was told that this is in normal range. I booked an appointment to discuss the results and guidances from BSSM off the back of your videos and the GP would not entertain anything I had to say. I asked for a confirmatory blood test to calculate my FT and was told that “there is no mechanism to test for FT”. I sat and tried to reason with the GP for 40 minutes and presented my private results and other studies/documentation from other NHS trusts and got absolutely nowhere. The only way the GP said he would prescribe TRT would be if I presented with ED, despite the fact that I presented with a lot of other symptoms of TD. I’ve done a lot of research on TD based on my symptoms and experiences over the past while and am disgusted that as a tax payer I cannot access the health care that I need. I’m now looking at my finances to see if I can scrape the money together to pay for this privately.

Very interesting video, Ive seen 4 Endocrinologists over last 8 years on NHS, they could all quote normal ranges, tbh, I have a spreadsheet for that, the best guy I saw, treated me as a Patient the others were only interested in numbers, the good Doctor told me every Guy different, and no one size fits all treatment, your experience knows what to look for with this Patient, a follow video would be interesting.

I had clinical Depression for 3 years, Big Pharma dealer(GP) had me on anti depressants and seeing therapists, just masked problem, eventually saw an Endocrinologist, put on TRT problem solved in a week, the NHS is random bad healthcare for Men, I have seen 4 Endocrinologists at the NHS clinic in that time, 1 good, 2 bad, 1 average. Im not a Doctor but I am an Analyst as a day job, so can learn enough about any subject to determine competency of anyone I am speaking to, the current one had never even heard of micro dosing or subq injections. Own your own health, get expert opinions from more than one expert, never be afraid to question, and accept the NHS is about cost savings and mens health is way down their priorities.

I call bullshit as well. Look every patient is different, but if somebody’s suffering from Lerone because they have low luteinizing hormone and their testicles still work just fine simply by just taking hCG. Their testosterone levels are gonna get restored to normal, literally just by taking hCG that’s it and they’ll still have full fertility be able to have children And life is fine. Of course it’s best if possible to the cause of the problem you can diet organics great sleep vitamins, herbs, sunlight, saunas, but after a year if that works and your levels are still low because of low luting hormone HG is a great option in my opinion it’s actually the best option.

If you said (most ppl) it would have been more accurate as not everyone needs a normal (whatever thata is? ) level. is normal 240-699 Total, is it 300-1100 total T? With 100 different ranges what is normal, since those ranges arent based on whats healthy, but a cross section of ppl that are tested healthy or not. Imagine if blood sugar was set in a range based on Populus average. Fasting blood sugar 60-170 could be the range. At least in this case we can agree the range is BS hopefully. The reason HCT gets high is stable T levels which arent (normal) Normal levels fluctuate during the day and are lower at night. 450-1000 24hr a day isnt normal, but 500-1300 diuring the early day and 300-600 during the night is normal IMO. The oral route can lead to a normal-ish level, but the compounded cream with lower dosed HCG would be the most normal approach I believe.

hey doc thanks for making the video and explaining the difference! I do have a question regarding testing for free testosterone. I currently live in Canada, and after calling several labs and doctor offices across the country, I found out that all tests for free testosterone offered across the country, are for INFERED/CALCULATED free testosterone, meaning they measure the total testosterone number, and do the usual calculation of 2%( I am assuming). And as you know this does not show the real picture at all. I am wondering if you wouldn't mind explaining what you think the reasoning behind this practice?
