Thank you for sharing this very important information. I wasn't fast enough on my keyboard to ask this question during the Live. I really love your book and wish that counseling programs would require an entire course around documentation. I finished school in 2018 and have always struggled with completing treatment plans and notes. With that said, since purchasing your book, I have re-done most of the forms I regularly use. My question is, when you are documenting this topics that come up, do you put that information in the "In Session Notes" section or do you find a way to incorporate it into the intervention check boxes (hope that made sense)? I'm really trying to adhere to your mantra of not writing anything twice. 🙂

Are you able to share a copy of the progress note template that you are speaking about? I use paper and then I go into my EHR and click everything on my template on there and then re-write everything into the integrated AI which turns it into a SOAP note. It's so many steps!! I love the idea of printing out a template taking a picture and uploading it. I also love the "in-session notes" idea. This is so helpful thank you!

@ yes, it still applies to Canadians! I mention HIPAA a few times, and there is one chapter on insurance, but the vast majority of the book (even in the insurance chapter) applies across the board. I hope it offers you some peace about your documentation!

Do you have any suggestions, or an example, for a SMART goal that focuses on maintaining a client's current status? I'm facing a roadblock on how to do this while applying the "increase" and "decrease" terminology. For example, the client is diagnosed with Bipolar Dx and has been stable for (at least) 3 months.

@AndreaBlunck ahhh… yes, this can be difficult, and likely only applies for medication providers. Insurance doesn’t cover basic maintenance, but does understand that medication is still medically necessary to maintain what is working. For that you would simply change the language to “maintain reduced symptoms of xx disorder” or “maintain medication compliance of…” For psychotherapy services though, if someone is doing well and no longer experiencing symptoms, it is time to reduce services from an insurance perspective. People can always pay privately at that point, but services won’t be medically necessary beyond the transitioning out period.

The time frame of me being behind on notes is typically 2 weeks and I normally caught up and then it is the cycle of being behind. I work for an agency where it is expected for us to see clients back to back and there is no schedule time for notes and other documentation. My challenge is even if a client cancel they expect us to do notes then but it is still expected of me to make productivity so normally I will try to do some Telehealth sessions. I end up doing notes on the weekends. Any advice to help with this situation. My job will not allow us to have schedule time for notes and additional documentation.

@@miyawrentz7331 in cases like this, collaborative documentation is usually the best, most realistic solution. It takes some practice and getting used to, but most clinicians AND clients love it. Absolutely worth doing in a scenario where you’re not given the necessary time for notes.

Does this apply if you're OON? I have a client that paid for all their session but, ignorer for them to be reimbursed, insurance is asking me for their, treatment plan and progress notes in addition to the superbill.

@@Perspective102 insurance has the same requirements regardless. The difference is that in-network, you’ve agreed to that with the insurance company. Out of network, it’s an agreement with the client and their insurance company. You are not involved in the payment process (unless you’ve voluntarily submitted super bills on their behalf). So it’s more about whether or not your client wants the insurance to see all their records. It’s up to them.

I don't usually recommend just a Response section. Instead, I break it up into categories that are easier to complete. Check out the note templates in the online resources for the book! You'll see what I mean with examples and a few template options :)

As an international student, I am still working on improving my writing skills, and I wanted to ask whether this is a valid reason for being dismissed from the position. Is the ability to write perfectly without errors a critical aspect of the counseling profession? Additionally, I would appreciate your guidance on whether tools like Grammarly or even ChatGPT could be used to assist with writing SOAP notes, or if such tools are considered inappropriate for professional documentation.
