Treat me like I'm dumb: How does this happen? Too hot and tolerances loosen? Excessive wear? Too much limiter? Something else locking up?

Probably excessive wear or rev limiter which caused the valve to break and piston to hit it

the right way to make a fly cutter, multiple carbides. most YT videos are of single cutter heads, not so great.... I think STORM VULCAN when I think fly cutter....

“Did you watch the video” is a trick to get more views from viewers. Making a camshaft out of brass is strictly for a non moving part as it will not work in real life.

It's a link to one of my older videos. People wanted to know how the bottom lobe of the camshaft is milled. Hence the soft material.

This is the hope it's concentric method and that was an absolutely crap fit on that bearing. Kinda hard to eyeball that stuff.
