@@Lord_Don the evidence suggests that for a single level, adding a plate does not add much benefit, however the decision to add a plate also depends on other factors related to your imaging and is down to the discretion and expertise of your surgeon.

@@Lord_Don yes. For bilateral symptoms, ACDF is a reasonable and effective approach, but as always depends on the clinical findings, i.e your surgeons assessment of you and your scans.

@ thank you, thank you, thank you!!! I AM VERY AFRAID OF PERMANENT NERVE DAMAGE! If I have ACDF on Sept 12th and a CT on Sept 13th shouldn’t the bilateral stenosis be eliminated from the CT findings?? (I’m just an ordinary analytical guy) I’m not a professional Sept 13th CT scan this was the MAIN trouble area - cord looks good. But am I fully decompressed or no? C5/6: Status post ACDF. No definite evidence of cord compression on this exam. No evidence of retrolisthesis on this exam. Moderate bilateral neural foraminal stenosis. I can’t sleep I’m panicking

@@Lord_Don thanks for reaching out. For bilateral symptoms, ACDF is a very reasonable approach. This video is a generic guide and I would go by the advice given by your surgeon.

Dr Ghosh I did find your descriptions of your surgical procedures interesting! I had many surgeries and have been suffering with severe debilitating fusion back pain!! 2 microdisectomies several post lateral and finally since they can’t seem to remember to tighten Peticle screw caps or install Peticle screws properly and after a yrs working hard physical back punishment chores, it was time to remove screws misaligned installed or too deeply installed screws managing to loosen right up! My life has been a living nightmare of pain on top of fighting leukemia! It’s a very difficult time trying to Sleep much less cut your grass or snow! Then you have every government agency down your throat with much distrust because of terrible narcotic Stigma Agenda all over the United States!! There are many human beings Suffering from very Real Life Altering Pain issues who are very legitimate!! Not in the government’s eyes!! Only your government can handle these issues Not your doctor!!! It’s been a very hard uphill battling the gov and severe debilitating pain issues from very legitimate evidence based back issues! Interesting

I’m doing physical therapy and seeing an ortho but the pain is constant in my lower back and when I sit or lay down it feels like it stems up into my mid back also. MRI showed slightly degradation and bulge in lower back forwards sacrum spine. The pain is not debilitating but it is frustrating and pretty much constant throughout the day but even worse when I put my weight on it. Any advice how to find some sort of relief?

Hi there. Been watching your video. I have had a c5-c6 neck disc replacement 10 weeks ago. When can massage, chiropractor or osteopath treatment recommence after surgery? Is rhere any type of masssage/chiro/osteo that wont help or damage. Can this type of operation thats been done to stop the nerve being pinched be undone Like can do you something to soon that can undo the operation? Im 10 weeks in and have no pain relief, worse than before the op and worse sinfe early days of op. Been told it is stoll early days, but 10 weeks in and nothing, im not sure.

Thanks for your interest. It is best to seek this advice from the surgeon who did your operation. I personally advise my patients to commence some light neck movement exercises in the early days that the physiotherapists on my team advise. Thereafter they are taught some light resistance exercises for the neck based on the ongoing physiotherapy assessments. I personally do not advise on massages adjustments or manipulations from a chiropractor in THIS setting.

Thank you for a balanced approach. I am recovering from a synovial cyst removal surgery. (second one within two years). I am desperate to run again but friends and family keep have some concerns re the impact. My surgeon advised that I can. Your video has reassured me that it will be fine.

Hi Dr, So with Elon Musk……He promoted in having a disc replacement surgery which he did indeed had it. I think sometime after a bit like me now started to get back his symptoms and he revised it for a fusion surgery and now he is doing great. I think he was criticised due to what transpired after the first surgery so he would not promote disc replacement surgery. It’s a common trend that a lot of people are getting out these discs around the word labelling it the next big thing…….but in fact is one of the biggest surgeries to be revised. A ridged Fusion is the best and I regret not listening to my first surgeon and I got a second opinion. I have learnt a valuable lesson session.

Hi Dr, Great videos. I had a disc replacement surgery in sep 2022 as the surgeon recommended it’s superior to fusion due to faster recovery, non loss of motion and avoiding adjacent segment disease. However for me the procedure failed, only after a few months I started to get symptoms back down my left arm, also I know that segment is hyper-mobile, I also have a lot of facet joint pain which I know is caused by the disc as I did not have it before. I found out that the disc through an MRI has broad-based posterior end plate disc osteophyte formation and has calcified and fused. I think fusion is better as it’s rock solid and it’s a ridge cage. Do you do revision surgeries? What can I do surgery? Thank you Raj

My leg weakness is the result of a severe sciatica attack that began suddenly in March. The excruciating pain is gone, but 5 months in my leg is still weak, making my gait unsteady.

I had nerve block injections which helped and then a rhizotomy was done which failed now my pain is back, doesnt shoot down the leg or anything like that. Whats next?

I've been partially bedridden, for years after a fall, I sustained bilateral SLAP tears, bilateral ankle and foot injuries, spine, knee, tbi, and more... I have only had one foot fixed so far and one shoulder repair that failed due to someone tipping at my arm 3 weeks post op. I cannot stand long, sit long, or lay long. I went from elite athlete level to frail and medically deconditioned. I'm having a neuroforaminal bilateral posterior approach TLIF with Nuvasive TDX. I'm so lost about what I can do now and have fusion in July. I'm so scared, and I have No help...

Thanks for this. I am a 70 y,o. male and have developed degenerative scoliosis. Doctor recommends the surgery, which sounds daunting, but I want to lessen the pain and stand up straight again.

Thank you for the video 👍 This last year I have developed a numbness in my right thigh, from my knee to the hip area. This numbness is not like my leg going to sleep, but as if, I had been injected with novicane. As when you go to a dentist and afterwards your face has that numb feeling if you touch it. This can happen any time, sitting, walking, mostly standing. It does not effect my walking. And I am fairly active for my age, 70. I just came in from mowing about a half acre of my driveway. I try desperately to lose weight, am at 185-189 & 5'3". Some mornings I can not stand up straight, until I sit in my hard chair, with my left leg under my back side, after just a few minutes, I can stand up straight. It is the strangest thing. And no one can answer why. I know that you can not answer, but thought I'd share. Ps. I live in rural area, that has no doctors for hundreds of miles.

I had L4-5 fusion in 2019. Pain was unbearable post op. Never got better. Finally found out that on all my scans I had the surgeon left 2 screws in my nerve canal. This was for 4 years. I got the cage out as I'm obviously fused now. Has this ever happened with you or any of your colleagues? Is this common?

I had lumbar surgery a week ago. Per doctor, I’m supposed to walk and make movements. But unfortunately, I had numbness in my lower leg and foot. In this case , do I still need to make effort to walk and move ? Problem is I’m disoriented and movement exert huge discomfiting though not the pain .

Thank you for your interest and well pointed out. I'm going to do a whole separate video on deformity. In my practice I am an adult only surgeon. So the only scoliosis I treat is secondary to worsening degenerative changes that lead to it and the reason I would operate is to untrap nerves as a result or treat any instability as mentioned in the video. Adolescent long segment scoliosis (that continues to adulthood) is a whole separate entity carried out by adult and paediatric spinal orthopaedic teams in subspecialty centres. And yes, I agree the results of surgery are usually really good. I hope this answers your question and thanks for your interest.

I heard McGill say he can’t help those patients sometimes I have a few arachnoid cysts in the lumbar, surgeons I saw all said it’s constitutional but I’ve been experiencing ongoing flexion intolerance for 5 years despite consulting endless physios including McGill practitioners

Thanks for this interview. I’d love to hear O’Sullivan and McGill in the same podcast debating their points of disagreement. It does seem to me based on the various interviews that O’Sullivan doesn’t categorize patients as much as McGill, which makes me think he’s maybe not as good a clinician, using more one size fits all “biopsychosocial” model
