Thanks for the video, but for LL, Trip pumps when you close discharge vale will carry pumps to shut off. Stop pumps would be correct. Isn't it?

What are the reference documents to prepare cause and effect matrix?

The reference documents are the documents that you used or relayed upon to develop yours C&E. so it may include P&ID, Process description, HAZOP report, Control Narrative. Also it may contains documents that include special instructions that you want the programmer to look into for further details. like Shutdown philosophy, also this would be included in the C&E notes.
