why is the discharge pulsatile after perforation ( light house sign ) , can patient present with pulsatile bulging TM instead prior to perforation

Imagine a tank containing water under pressure. If you put a small hole on the surface of tank, water will leak continously. Here there is collection of pus behind TM.Pus comes out under pressure,and synchronising with each arterial pulse.

I have a question, Dr . If there is otitis media it might have perforation. Then why should we make the myringothoy?

Otitis media means inflammation of middle ear. need not be associated with TM perforation. Patients who have experienced four or more episodes of AOM in the past twelve months should be considered candidates for myringotomy with tube (grommet) placement, according to the American Academy of Pediatrics guidelines
