If I'm honest, 1. Greg James. And I think that's because he was shouting about doing a cameo on his radio show but wouldn't give any clues to what he was doing in the episode.

Speaking of City Of Death, it's not a hard-to-spot cameo of course but the greatest cameo HAS to go to John Cleese as the art critic in the Louvre. A classic Python gag INSIDE a Who episode :D Merry Christmas all!

I'm not quite sure it counts as a cameo, since he's actually listed in the credits, but Keff McCulloch, who composed the Sylvester McCoy version of the theme and provided music for six stories with McCoy's Doctor, appeared on screen and on stage as a member of the Lorells, the band at the holiday camp in "Delta and the Bannermen". Of course that was also one of the stories he provided the score for as well.

About Mark Gatiss, I think it's rather intruiging that he appeared in Twice Upon a Time alongside his own casting choice for The First Doctor, now acting alongside him. To me, there is something quite meta about that.

You forget another Peppa Pig/Who connection. Grampy Rabbit is voiced by Brian Blessed. Who aside from being Yrcanos and Omega in an audio story, was considered for the Second Doctor along with a number of other subsequent roles. Having seen plenty of Peppa Pig from the appropriate years and not knowing what the actress who voiced her looked like, I had the image of a child looking somehow quite British and cheery with chubby cheeks. Turns out I was pretty on.

He went on to appear in various comedy shows including Sirens, Bad Sugar, PhoneShop, Skins and The Increasingly Poor Decisions of Todd Margaret all before Doctor Who. In 2010, he was one of the main cast of Chris Morris' hugely acclaimed comedy film Four Lions. Since Doctor Who, he's appeared in films like Paddington, Prevenge, Cuban Fury, Men in Black International and Cruella... As well as TV shows like Inside No. 9, Uncle, SunTrap and Toast of Tinseltown. However he is probably best known for his main role as Nador in What We Do In The Shadows. So yeah. He's pretty famous.
